Met dit formulier vraagt u een beschikking Algemeen nut beogende instelling (ANBI) aan. Met deze beschikking merkt de Belastingdienst een instelling of meerdere instellingen aan als ANBI.

Giften aan ANBI’s zijn aftrekbaar voor de inkomstenbelasting of de vennootschapsbelasting. Ook komen ANBI’s onder bepaalde voor-waarden in aanmerking voor een vrijstelling van de erf- en schenk-belasting, de overdrachtsbelasting en voor een vermindering van de energiebelasting. U kunt dit formulier gebruiken voor zowel één instelling als een groep instellingen.

Groepsbeschikking aanvragen?Vraagt u een beschikking aan om meerdere instellingen in één keer aan te wijzen als ANBI? Zij krijgen dan een zogenoemde groeps-beschikking. Dit kan alleen bij een specifieke categorie instellingen, of een groep met elkaar verbonden instellingen. Bij de aanvraag van een groepsbeschikking gelden de vragen voor elke instelling die bij de groep hoort. Vermeld in een bijlage de volgende gegevens van alle instellingen die bij de groep horen:– de naam van de instelling– het vestigingsadres– het RSIN of fiscaal nummer– het internetadres

Wilt u ook voor de overkoepelende instelling een beschikking aan-vragen? Vermeld dan ook de gegevens van deze instelling in de bijlage.

Ook status culturele ANBI aanvragen?Met dit formulier kunt u ook de status culturele ANBI aanvragen. Uw instelling moet dan voor minstens 90% op cultureel gebied actief zijn. Wilt u naast de aanwijzing als ANBI ook de status culturele ANBI aanvragen? Vul dan ook vraag 2 in.

Aan welke voorwaarden moet een ANBI voldoen?Behalve aan de voorwaarden uit dit aanvraagformulier en de toelichting moet een ANBI ook voldoen aan de volgende belangrijke voorwaarden:– De kosten van beheer moeten in een redelijke verhouding staan

tot de uitgaven die de instelling doet voor het doel van de instelling. Wat een ‘redelijke verhouding’ is, is gezien de diversiteit van de instellingen maatwerk. Deze verhouding moet wel blijken uit de administratie.

– De instelling mag niet meer vermogen aanhouden dan redelijker-wijs nodig is voor de continuïteit van de voorziene werkzaamheden van de instelling. Het vermogen dat in elk geval mag worden aangehouden is:

– Vermogen verkregen uit nalatenschappen of schenkingen, waarbij is bepaald dat dat vermogen in stand moet blijven als stamvermogen. De rendementen uit dit vermogen moeten wel worden aangewend voor het beoogde doel.

– Vermogen waarvan de instandhouding voortvloeit uit de doel-stelling van de instelling. Voorbeeld: de doelstelling van de instelling is het in stand houden van een kerkgebouw of land-goed. Het bezit van een kerk of landgoed komt dan voort uit de doelstelling.

– Vermogen dat nodig is om de activiteiten van de instelling te ondersteunen, zoals een pakhuis of kantoor pand.

– De instelling moet een administratie bijhouden waaruit ook het volgende blijkt:

– de aard en omvang van de toegekende onkosten vergoedingen en/of vacatiegelden aan de afzonderlijke leden van het bestuur

– de aard en omvang van de kosten van beheer, en de andere kosten die de instelling heeft gemaakt

– de aard en omvang van de inkomsten en het aangehouden vermogen van de instelling

Wij toetsen de instelling aan alle voorwaarden. Op belastingdienst.nl/anbi kunt u de voorwaarden nalezen.

Gegevens instellingOp belastingdienst.nl staat een overzicht van instellingen die zijn aangewezen als ANBI. In dit overzicht staan de instellingen onder hun officiële naam. Misschien is de instelling bij het publiek beter bekend onder een andere naam. Als u deze andere naam ook opgeeft, nemen wij die ook op in ons overzicht. Donateurs kunnen dan makkelijker de instelling vinden.

Vraagt u een groepsbeschikking aan? Vermeld dan in een bijlage behalve de officiële namen ook de andere namen waaronder de instellingen bekend zijn.

Groepsbeschikking aanvragenVraagt u een beschikking aan om meerdere instellingen in 1 keer aan te wijzen als ANBI? Zij krijgen dan een zogenaamde groeps-beschikking. Elke instelling moet aan alle voorwaarden voldoen. U kunt alleen een groepsbeschikking aanvragen bij een specifieke categorie instellingen, of een groep met elkaar verbonden instellingen. Bijvoorbeeld de instellingen die horen bij de Vereniging Openbare Bibliotheken. Bij het aanvragen van een groepsbeschikking moet aangegeven worden welke instellingen onder de groep vallen.

Toelichting bij de vragenVraagt u een groepsbeschikking aan? Dan gelden de vragen voor elke instelling die bij de groep hoort. Lees in dat geval ‘instelling’ als ‘instellingen’.

Bij vraag 1aAls u een aanvraag doet voor een groepsbeschikking, moeten de instellingen die tot de groep behoren, dezelfde doelstellingen hebben. Instellingen kunnen ons vragen ze aan te wijzen als ANBI. Instellingen die hiervoor in aanmerking komen, zijn kerkelijke, levensbeschouwelijke, charitatieve, culturele en wetenschappelijke instellingen en instellingen die op een andere manier bijdragen aan het algemeen nut.

Bij vraag 1bEen publiekrechtelijk rechtspersoon is een overheidsinstantie, bijvoorbeeld een gemeente of een waterschap.

Bij vraag 1cEen ANBI mag geen winst maken met haar algemeen nuttige activiteiten. Een instelling die commerciële tarieven vraagt voor deze activiteiten kan geen ANBI zijn.

Een ANBI mag wél winst maken uit een commerciële activiteit, naast de algemeen nuttige activiteiten. Voorwaarde is wel dat de winst uit de commerciële activiteiten ten goede komt aan het algemeen nut.

Bij vraag 1dTen minste 90% van de werkzaamheden van de instelling moet gericht zijn op het algemeen belang. Als de instelling de belangen van één of enkele families behartigt of die van een beperkte groep leden, dan dient de instelling niet het algemeen belang. Dit is bijvoorbeeld het geval bij een personeelsvereniging.

Bij vraag 1eOver de integriteit van een ANBI en de mensen die daarbij betrokken zijn, mag geen twijfel bestaan. Als de instelling, een bestuurder, een leidinggevende of gezichtsbepalend persoon de afgelopen vier jaar onherroepelijk is veroordeeld voor een misdrijf dan wordt de instelling niet aangemerkt als ANBI.Het gaat dan om een veroordeling voor het opzettelijk plegen van een misdrijf als bedoeld in artikel 67, lid 1 Wetboek van Strafvordering;en:– het misdrijf is gepleegd in de hoedanigheid van bestuurder, feitelijk

leidinggevende of gezichtsbepalend persoon van de instelling;– nog geen vier jaren zijn verstreken sinds de veroordeling; en– het misdrijf gezien zijn aard of de samenhang met andere door de

ANBI of genoemde personen begane misdrijven een ernstige inbreuk op de rechtsorde oplevert.

Het genoemde wetsartikel omvat alle misdrijven waarvoor een bevel tot voorlopige hechtenis kan worden gegeven.

Bij twijfel over de integriteit van een persoon of de instelling kunnen wij vragen om een Verklaring Omtrent Gedrag (VOG). Voor informatie over de VOG, zie www.justis.nl.

Bij vraag 1fEen natuurlijk persoon of een rechtspersoon (bijvoorbeeld een bv) mag geen doorslaggevende zeggenschap hebben over het vermogen van de instelling. Een bestuur van minimaal drie onafhankelijke personen waarborgt dat die persoon niet over het vermogen kan beschikken alsof het zijn eigen vermogen is. Er is sprake van onafhankelijke personen als zij geen enkele onderlinge relatie hebben, zoals een familieband of een zakelijke relatie.

Ook is het niet toegestaan dat er onvoldoende bestuurders zijn. Is er een vacature? Dan moet die binnen 3 maanden zijn vervuld.

Al deze zaken moet u vastleggen in de statuten van uw instelling.

Bij vraag 1gHet bestuur van de instelling bepaalt het beleid. De bestuursleden mogen voor de beleidsbepalende werkzaamheden geen beloning krijgen. Zij mogen eventueel wel een onkostenvergoeding en vacatiegeld krijgen. Als de instelling een raad van toezicht heeft die het bestuur controleert, bepaalt de raad van toezicht het beleid.

In de statuten van de instellingen moet dit beloningsbeleid ook zijn vastgelegd.

Bij vraag 1hEen ANBI heeft een publicatieplicht. Zij moet reeds bij de aanvraag via een internetsite haar volgende gegevens van het afgelopen boekjaar openbaar maken:– de naam van de instelling– het RSIN/fiscaal nummer– de contactgegevens– de doelstelling– het beleidsplan– de bestuurssamenstelling– de namen van de bestuurders– het beloningsbeleid– een verslag van de uitgeoefende activiteiten– een financiële verantwoording

Let op! De gegevens die de instelling publiceert mogen ook

in het Engels, Duits of Frans zijn.

Bij vraag 1iIn de statuten van de instelling moet zijn vastgelegd dat bij opheffing het vermogen dat overblijft wordt besteed aan een ANBI met een soortgelijk doel.

Regelmatig zien wij in statuten staan dat het geld dat overblijft na opheffing ‘zoveel als mogelijk’ wordt besteed aan een ANBI met een soortgelijke doelstelling. Wij wijzen de aanvraag af als uw instelling een dergelijke formulering in de statuten heeft opgenomen. Het is van belang dat het volledige bedrag dat overblijft wordt besteed aan een ANBI met een soortgelijke doelstelling. Vermogen van bijvoorbeeld een theater moet na opheffing van de ANBI worden besteed aan een theater. Twijfelt u of de statuten aan alle voorwaarden voldoen? Dan kan een notaris u misschien helpen.

Bij vraag 1jAls de instelling al bestond op 1 januari van het jaar waarin u de aanvraag doet, kan de instelling met terugwerkende kracht tot 1 januari van datzelfde kalenderjaar worden aangemerkt als ANBI.

Een nieuw opgerichte instelling, die binnen één jaar na oprichting een verzoek doet om aangemerkt te worden als ANBI, kan met terug-werkende kracht tot de datum van oprichting worden aangemerkt als ANBI. In beide gevallen geldt dat de instelling pas een ANBI is vanaf het moment dat de instelling voldeed aan de eisen voor een ANBI.

Een afwijkende ingangsdatum is ook mogelijk bij bijzondere omstandigheden, zoals bij een oprichting van een instelling bij testament.

Pas als wij de instelling hebben aangemerkt als ANBI, kunt u gebruikmaken van de fiscale voordelen. Het is mogelijk dat niet alle instellingen in een groep aan de ANBI-voorwaarden voldoen. Een instelling die niet aan de voorwaarden voldoet, nemen wij niet op in de groepsbeschikking.

Bij vraag 2 De vragen 2a en 2b hoeft u alleen te beantwoorden als u naast de aanwijzing als ANBI ook de status culturele ANBI wilt aanvragen. Uw instelling moet daarvoor minstens 90% actief zijn op cultureel gebied.

Voorbeelden van culturele instellingenCulturele instellingen zijn instellingen die actief zijn in beeldende kunst, bouwkunst, erfgoed, dans, film, letteren, (pop)muziek, theater of vormgeving. Hieronder vallen bijvoorbeeld musea, archieven, archeologische instellingen, monumentenzorg, theatergroepen, schouwburgen, bibliotheken, filmhuizen, (pop)podia, letteren-, pop- en filmfestivals en architectuurcentra.

Hardware is leased. Software licenses are spent mainly on WordPress plugins and Articulate 360 for businesses. Perhaps a collaboration with the AMC/UvA could save some costs. When the team expands in year three, an office is required. Preferably in a start-up building across the street from the AMC or a University building.Aging of the online product is a pitfall that needs attention from the start of content creation through redacting cycles. Marketing focusses on online presence, mainly new medical students and faculty should be reached.

The foundation will set out with a founder responsible for early content and overall management, in possession of aMedical Degree. Initially, an early build is developed through existing code and plugins in a WordPress-based learningmanagement system (LMS). In year three, a site content migration away from the early build is a possibility. The purposewould be to adopt a more solid and scalable platform. To satisfy users however, the platform needs quantity and qualityof content. This is why the team expands with a medical content developer every year.Please note that this projection is for the minimum personell required to be professionaly operational. Scalingdepends on income, Project Tulip hopes to quickly expand in staff and through outreach programs towardsvolunteer content creators. We want to build!

These costs of goods and services sold are mostly reserved for server costs and other IT infrastructure. The costs per user should decrease significantly as the user base grows. Back-end development should focus on scaling.

In this projection, income is solely based on donations from academic institutions, medical centers, charities, NGO's and large businesses. Project Tulip will aim for yearly contributions starting from €25,000. Naturally, Tulip will prioritize pursuing grants from different sources as well.

The user growth is directly related to the completeness of the platform. An incomplete platform will not draw many users, so the initial barrier is high. However, the platform user base may grow ahead of this projection depending on collaboration with external partners on the question bank and textbook-like content.

heavilyononlinestudytools.Imostlyusedmyuniversity’sweb-

lectures so I couldpauseandaddnotes, images,diagrams,etc.

Physical classes in massive lecture halls usually cover basic

materialandhaveverylittleinteractionanyways.

Istarted‘blending’thecurriculumtomyneeds,andgotinvolved

inseveraldigitaleducationinitiativesatmyuniversityandmedical

faculty.Here Iencounteredtheproblemsoutlined inchapter2,

and sawmedical faculties struggle to create andpublish digital

contentwhileblendedlearningandflippingtheclassroommade

their definitive entrance. If universities are cutting back on

physical education and instruct students to learn the basics in

theirowntime,studentsshouldbeprovidedanadequate(digital)

alternative.Duringmyfinalyearsofmedicalschool,Ienrolledin

thehonorsprogram,spent6monthsintheUSAtodoresearchat

Harvard, co-published in Science, built a home-studio for

educational videosandcreated loadsof studymaterial through

videos,writingexamquestionsanddevelopingonlinecourses.Six

months before graduation I had to choose between Pediatrics,

pursuing a PhD and a career in education. My passion lies in

Toelichting op de Balans

1. Een boekjaar van de Stichting Project Tulip loopt gelijk met een kalenderjaar. Stichting Project

Tulip is opgericht op 13 april 2018 en zal in haar eerste jaar gebruikmaken van een verlengd

boekjaar eindigend op 31 december 2019.

2. De balans per Q2-2018 weergeeft een momentopname van de financiële positie van Stichting

Project Tulip per 30 juni 2018.

3. Overige Kortlopende Schulden bestaan uit nog uit te betalen vacatiegelden ten behoeve van het

bestuur van Stichting Project Tulip.

4. Overige Langlopende Schulden bestaan uit een lening met een looptijd uiterlijk eindigend per 31

december 2022 tegen 1.0% rente op jaarbasis ten behoeve van de opstartkosten van Stichting

Project Tulip.

Toelichting op de Baten en Lasten

1. Een boekjaar van Stichting Project Tulip loopt gelijk met een kalenderjaar. De stichting is opgericht

2. De baten en lasten per Q2-2018 weergeven een overzicht van de baten en lasten van Stichting

Project Tulip in de periode van 1 januari 2018 tot en met 30 juni 2018.

3. Depreciatie bestaat uit eenmalige oprichtingskosten van Stichting Project Tulip en zijn

voornamelijk gedreven door gemaakte kosten bij inschrijving bij de notaris en de Kamer van

Koophandel.
