Part 1 - Registration Requirements
The parent should use the Humble ISD Home Access Center, described in the Part 2 and Part
3 of this document, to provide an annual update for the student.
Once the Annual Student Online application for a student is submitted, please bring proof of
residency to the campus registrar to complete enrollment.
If applicable for the student, bring updated immunizations, military dependent documentation,
and the Department of Family Protective Services documentation.
If applicable for the student, fill out and return the following health forms:
Diet Modification Registration
Authorization to Administer Medication
Immunization Records
Immunization records and all documents claiming exemption are required prior to enrollment in
Humble ISD. This includes all special education programs, inclusive of speech, SPEAK classes,
PPCD and ECI services, whether the services are on or off campus. The immunization
requirements are listed in the Required Immunizations section of the Humble ISD Student
Handbook. Reasons of Conscience forms must be completed, notarized and submitted to the
school before enrollment.
Legal Custody for Enrollment
Students under eighteen (18) years of age must be living with their parents, legal guardians,
managing conservators, any other persons acting under a valid court order, or other persons to
whom placement has been made by a child placement agency currently licensed by the State of
Texas.
Custody Documents
It is the parent’s responsibility to provide the school with current legal documents when custody
rights have been determined by a court of law. Legal documents are those with a judge’s
signature and a stamp of the court.
Verification of Residence
The enrollment process in Humble ISD includes a Student Residency Questionnaire and
documentation to verify residence. For students residing in a house, the lease, deed, utility bill,
etc., will suffice. For apartment dwellers, either the lease with parent and student name listed or a
letter from the apartment manager is required. If both parent and student live with a relative or
friend in Humble ISD whose residence is not an apartment, the parent and the person with whom

they reside must submit a notarized statement verifying the residence. A form is available at
every campus.
Homeless Students
The enrollment process in Humble ISD includes a Student Residency Questionnaire as part of
our comprehensive efforts to identify students and families currently experiencing homelessness.
If your current address is a temporary living arrangement and it is due to loss of housing or
economic hardship, you may meet the definition of experiencing homelessness. If, due to a lack
of housing, you must live in a shelter, motel, vehicle, or campground, on the street, in abandoned
buildings or trailers, or doubled-up with relatives or friends, then according to the McKinney-
Vento Act, you are considered homeless.
The educational rights homeless students have under the McKinney-Vento Act include the right
to:
go to school,
enroll in school without giving a permanent address,
continue attending the school in which they were enrolled before the homelessness
occurred when feasible; and
enroll in the Texas public school district of their choice with the campus designation
being determined by the school district when feasible.1
Humble ISD can also offer additional assistance to students and their families experiencing
homelessness. If you are in need of additional assistance, please speak to the campus registrar or
contact the Humble ISD Homeless Liaison at (281) 641-8408.
DFPS
If the student is EE, K-12 and is in the conservatorship of the Department of Family and
Protective Services (DFPS), please provide one of the following acceptable documents:
1. All forms in the 2085 series
Foster Care/Residential Care – 2085 FC
Kinship or Other Non-Foster Caregiver – 2085 KO
Verified Kinship Foster Caregiver - 2085 KF
Legal Risk - 2085 LR
Home and Community-based Services (HCS) - 2085 HCS
Supervised Independent Living - 2085 SIL
Designation of Education Decision-Maker – 2085 E
Designation of Medical Consenter – 2085 B

2. DFPS Kinship Caregiver Agreement – 0695
3. Court-Order naming Texas DFPS as the Temporary Managing Conservator (TMC) or
Permanent Managing Conservator (PMC).
If the student is pre-kindergarten you must provide a copy of the verification letter sent by the
Texas DFPS and Child Protective Services.

Part 2 - Log into the Home Access Center
Each year a parent or legal guardian must update a student’s enrollment.
Use one of the supported browsers: Internet Explorer 9, 10, 11 (not in compatibility view),
Firefox (current release plus 4 prior versions), Chrome (current release plus 4 prior versions),
or Safari 5.1.7 to navigate to the Humble Enrollment site located at
http://www.humbleisd.net/enrollment
Click on “Returning Student” button to navigate to the Online Annual Student Update page.
Please review information on the page for additional instructions.
A guardian must have a HAC account established and be listed as the guardian who lives with
the student to launch and open the Annual Student Update Online Registration Module. If you
do not yet have a HAC account, click on the “Setting Up a HAC Account” link and follow the
instructions.
When ready to begin the Online Annual Student Update click on “Home Access Center” button.
Following are instructions for completing the annual student update enrollment.
1. Log into Home Access Center.

2. Click on the “Registration” button.
3. Click on the “Update Registration” tab.
4. Click NEW to open the new annual update application.
Part 3 - Annual Student Update Application
At the top of the Annual Student Update Application form, you see a hyperlink “Show All
Sections” which opens each section of the form to allow you to view all sections of the document
at once.
You may also open and close sections by clicking on the plus sign in each section header.
Please be aware the enrollment session times out after 90 minutes. To save information, click on
the SAVE button located at the end of the form.
1. The Student Information section contains the most recent information for your student.
Student name or birth date requires legal documentation in order to update. Please contact
the campus registrar to update information that requires supporting documentation or to
request an update to ethnicity or race.

2. Set checkbox ON to give student access to Home Access Center where they may view
their grades and attendance online.
3. Click on the ‘-‘ sign to close the Student Information section then click on the ‘+’ sign to
open the Address and Phone section.
4. The Address and Phone section contains the most recent information for your student.
Student addresses must be verified each year by providing supporting documentation to the
campus registrar. A document such as a current utility bill, residential lease, etc. is required
to establish residency in Humble ISD. See district web page or student/parent handbook for a
list of acceptable documents.
You may modify the Student Phone information.
5. Click on the ‘-‘ sign to close the Address and Phone section then click on the ‘+’ sign to open
the Contact Information section.

6. In the Contact Information section, you are able to view and edit parent/legal guardian and
emergency contacts.
Click on the black arrows to adjust the priority of the contacts.
Click on the Edit/View icon to review and update contact information.
Click on the red “X” in the Delete column to remove a contact.
Click on the Add Contact button to add a new contact.
Only chose one contact type for each contact. (Guardian OR Emergency)
It is recommended at least 1 Guardian and 1 Emergency contact be provided.
If you wish to view HAC screens in Spanish, set the Language of Correspondence to
“Spanish”.
You must SAVE the application after inserting or editing each contact.

7. Click on the ‘-‘ sign to close the Contact Information section then click on the ‘+’ sign to
open the Additional Information section.
In this section, the right column shows the Existing Information on File for the student.
Note: If you enter new information that does not match the Existing Information on File, this
column will not show the new response until the registrar has accepted your application.
8. Provide responses to the Student Health Inventory Questionnaire.
Each select box requires a yes/no response.
N indicates NO.
Y indicates YES.
When Y is selected, please provide additional information in the explanation for the
disease/condition.
Please enter your name at the end of this section acknowledging all health information is
correct.

Remember you must SAVE the application within 90 minutes or session timeout
will occur and information will not be saved. Use the TAB key to navigate between
fields and type N or Y instead of using a mouse to select value from SELECT drop down.
You might also fill in required information, SAVE the application, then RETURN to this
section to fill in explanations.
8. Provide responses to the Military Connection and Foster Care Questions.
Each select box requires a yes/no response.
N indicates NO.
9. Provide responses to the Family Survey.
Each select box requires a yes/no response.
N indicates NO.
Y indicates YES.

10. Provide responses to the Student Residency Questionnaire.
If student’s current housing situation is TRANSITIONAL please fill in the best
description of the living situation, the background and services needed.
Fill in this section if student is living in a TRANSITIONAL situation.
Please enter your name at the end of of the section acknowledging all information is
correct.

9. Provide responses to the Permission and Consent Questions.
Each select box requires a yes/no response.
N indicates NO.
Y indicates YES.
Please indicate if your address has changed.

10. Click on the “-“sign to close the Additional Information section then click on the “+” sign to
open the open the Additional Questions section.
11. Please enter your name next to each question acknowledging you have read the statement.

11. Save the application.
12. Click the “I Agree” checkbox to acknowledge that you are the parent or legal guardian of the
student.
13. Click the Submit button to send your enrollment application to the campus.
14. After the application is submitted, you may return to the Update Registration page where the
status of the student’s enrollment application is displayed. Use the “Edit” link to submit any
additional changes to the application.
15. After submitting the online enrollment application, visit the student’s school campus to
complete the enrollment process. Please bring documents for Residence Verification.
If applicable, bring updated immunizations, military dependent documentation, and the
Department of Family Protective Services documentation.
Part 4 - Accepted Application
After an application has been submitted, a Humble ISD registrar will review and accept the
submission.
Parents are encouraged to visit the District’s Child Nutrition website,
http://www.humbleisd.net/childnutritionregistration, where you may:
Set up a “mySchoolBucks” account to pay for meals
Report student Food Allergies
Register for Free and Reduced Price Meals
