I have always enjoyed looking at a person’s face to determine what makes them look youthful and a�ractive versus aged, unhealthy or “operated on”. As we age, most of the changes seen in the face are the result of loss of support and fullness in combination with hereditary factors, smoking history, and sun exposure.

Facial surgery has really evolved over the past ��een or so years. Gone are the days of the overly tight wind tunnel appearance. In rejuvenating the face we now strive for a so�, natural appearance, making the skin-only removal type of faceli� a thing of the past. �e skin has no structural support and in addition lasting only a short time, a skin-only li� initially produces an excessively tight appearance that is unacceptable to most people.

Newer faceli� techniques involve a two layer approach, called a deep plane or SMAS faceli�. For this the facial skin is li�ed, but an additional second layer deep to the skin, is also li�ed. �is second layer, the SMAS, is a �brous fa�y layer that a�aches to the lower facial muscles. Tension is diverted to this deeper layer during the li�ing process which results in a more natural look and longer lasting results. With this technique and proper incision placement, once the healing process is complete there will be li�le, if any, tell-tale signs that surgery has been performed.

A newer procedure that has really enhanced my face li�ing results is fat injection. Fat injections known as lipostructure, adds volume to the face on a more permanent basis. We borrow fat from areas of excess – saddle bags, love handles, or inner knees; process it, and actually re-inject it into the face to �ll it out. If you look at a child’s face, it is a full face. As we age, we lose volume in the face and remaining facial fat tends to redistribute, depositing in the jowl, under the chin and the area above the nasolabial fold (the fold between the nose and the upper lip) with subsequent disappearance of surrounding fullness. Deep valleys in the face, volume loss, and sagging skin create the appearance of aging or poor health.

Typical areas for fat injections are the cheeks, temples, nasolabial folds, jaw line and even in the hollows underneath the lower eyelids. It never ceases to amaze me to see how adding back facial volume makes one look so much younger. I frequently recommend fat injections in combination with SMAS face-li�ing, it takes my result and makes it just one step be�er. Interestingly current scienti�c research has demonstrated that harvested fat cells actually also contain an abundance of stem cells and various growth factors. Stem cells and growth factors are showing promising results for mending the sun- damaged and aged skin by improving skin texture, elasticity and color. Stem cells are the new buzz in facial rejuvenation and I look forward to exciting new procedures combining them in the rejuvenation process.

When properly planned and executed, modern faceli� procedures can provide long lasting, natural results. Ancillary procedures such as fat injections, eyelid surgery, skin resurfacing, chin augmentation, rhinoplasty and forehead li�s all combine to improve the �nal outcome.

My practice is unique in that we only operate and perform services on the face. We therefore are quali�ed to tailor each procedure to achieve an outcome that is harmonious with age and appearance. It is my �rm opinion that no surgeon performing full cosmetic surgery can be an expert at every procedure, and that is why I chose to specialize in plastic surgery isolated to the face and neck. Are your teeth sensitive or mobile? � ese are a few of the commonly overlooked symptoms of a condition called occlusal disharmony. � e term occlusion refers to the way your teeth and jaw joint come together when you bite. If this relationship is not ideal, there can be many painful and costly repercussions. It is important to have this diagnosed before you have any dental work completed, but unfortunately many dentists do not take the time or have the secondary education to identify and treat occlusal disharmony. Most people understand that x-rays are used to diagnose cavities and that periodontal gum measurements are used to diagnose gum disease, but did you know that there is a device that can diagnose occlusal disharmony? It is called a Kois Deprogrammer.

Over the last twenty years, a strong emphasis has been placed upon prevention in the dental � eld. Fluoride is used to prevent cavities, professional teeth cleaning and proper home care is used to prevent gum disease and there are technologies used to detect oral cancer in its early stages. Because occlusal disharmony can cause many oral problems, it should be approached in a preventive method as well. � is is a relatively new way of thinking as many of the symptoms of occlusal disharmony don’t appear to be linked to the cause. If the occlusal disharmony is not addressed, eventually teeth and � llings will wear and fracture again. � is is why a dentist is like a “Crime Scene Investigator.” � ere is evidence in the oral cavity that needs to be collected and examined in order to identify the cause of symptoms and then an individualized treatment can be prescribed.

� e following article will discuss the role that occlusal disharmony plays in relationship to many of the most common dental complaints, how it can be diagnosed through a few simple steps and the di� erent options for treatment.

HOW DO I KNOW IF I HAVE OCCLUSAL DISHARMONY?Many of the most common oral complaints can be attributed to occlusal disharmony. WHAT IS A KOIS DEPROGRAMMER?A Kois Deprogrammer is a removable dental appliance, similar to an orthodontic retainer that is worn over the teeth to prevent them from touching when you bite together. It is designed to

aid in the diagnosis and initial treatment of occlusion related problems. Because it prevents the top and bottom teeth from touching, you cannot fully engage the muscles that move the jaw joint. When these muscles have an opportunity to rest, they “deprogram” and relax into their physiologically ideal position without any in� uence from biting or clenching. � e Kois Deprogrammer is both a diagnostic tool and a way to temporarily relax the jaw and reduce muscle tension, headaches and excess wear on the teeth. � e Kois Deprogrammer should be worn as much as possible and always removed when eating or cleaning teeth.
