CHW reaches a patient who is not feeling well
CHW calls in the operator
Operator takes call and begins registering data in the computer
The conversation…• Hello, what is your name?
– CHW gives his own name• What is your patient’s name?• In what village is the patient located?
– Pulls out the specific form for the clinic in the village• What are the patients Symptoms or signs?
– Record vitals as well• Does s/he have any Allergies?• What Medications is s/he taking?• What is the patient’s Past history?• When was the Last oral intake (meal)?• Thank you, can you please hold?
– Operator calls clinic, verifies waiting times and • I will send this information to the clinic before the patient arrives
The information is printed out and given to a messenger. Only the top part of the form is filled, the bottom is reserved for the clinic to fill out.
Messengers visit clinics on rounds every morning to deliver / pick up forms. If it is an emergency, then a special messenger will be on call to service the papers.
Messenger uses Boda Boda to transport forms to clinic
Clinic receives information. When patient is taken care of, the nurse fills lower part of the form.
Patient arrives and his information is already
waiting for him.
Once patient is treated, nurse fills out the form with the diagnostic and indications for treatment.
Messenger uses Boda Boda to pick-up forms from clinics and delivers them to the operator. CHW visit the operator to pick up forms and follow up patient
treatment.
