Personnel Training and Experience CHDT personnel have been conducting Naturally Occurring Radioactive Materials disposal operations since December 2006. All staff members have been trained on the basics of radioactive materials and basic radiation safety. All staff members have been trained in CHDT Operational SOP’s. Ongoing training occurs at least annually. Operations staff are trained and experienced in the use of radiation instruments. Staff training in radiation safety is summarized in this document. Most CHDT Operations staff members have more than ten years experience in landfill waste operations and they are expert at safely handling many types of hazardous materials. CHDT Key Personnel have many years of technical education and experience. Staff organization and responsibilities are detailed in SOP 15.RPP.01 Radiation Protection Plan. A current organizational chart is included in this attachment. Resumes of Key Personnel are also included.

Provided worker training, Radiological Control Technician procedural and field guidance, and oversight of radiological survey results and site control. Site was successfully remediated with identification and removal of radiological commodities and final closure of the site. Former Alameda Point Naval Air Station Radium Controls - Radiological Safety Officer for six-phase heating system volatile organic sub-slab remediation at a closed military airbase. Defined, implemented, and oversaw radiological controls for radium-contaminated soil during installation, remediation, and removal activities for three different locations at the site. Boeing Reality Corporation - Project Manager for contract to survey closed aircraft manufacturing facilities and reclaimed property for real estate transfer. A historical license and record review was performed to identify radionuclides of concern. Facility surface survey and soil sampling to verify absence of depleted uranium, potentially from aircraft control surface counterweight alterations; and natural uranium potentially contained in aircraft paints. Both evaluations were performed in the presence of natural background uranium. Final report documenting absence of contamination provided the regulators and client documentation for use in property transaction decisions. SPRU Land Areas Clean Up - Provided radiological readiness evaluation for DOE site soil removal project. Member of a Readiness Evaluation team that performed onsite review of documentation, personnel and equipment readiness, and operational readiness. Mockup performance of critical project tasks were observed and evaluated. A Readiness Assessment was completed to support authorization to begin onsite remediation work. Naval Station ? Puget Sound (Sand Point) - Certified Health Physicist providing project design, estimating, execution, and documentation for radiological characterization of radium-contaminated aircraft instrument shops at a closed military facility. Provided onsite radiation protection supervisor services, managing radiological control technicians as scoping and characterization surveys were performed and documented. Significantly more contamination was identified than initially anticipated, resulting in extended field work. Managed radioactive material license necessary to perform the work and managed the investigation derived radiological waste generated during the characterization. 05/2002 - 10/2005 Senior Consultant I, Shaw Environmental & Infrastructure, Inc., Federal Services, Centennial, Colorado 08/2001 - 05/2002 Senior Consultant I, IT Corporation (The Shaw Group In., acquired substantially all of the operating assets of The IT Group, Inc., on May 23, 2002.), Englewood, Colorado 04/1997 - 08/2001 Certified Health Physicist, Rocky Mountain Remediation Services, L.L.C., Golden, Colorado 12/1993 - 04/1997 Senior Health Physicist, Chew & Associates, Golden, Colorado 03/1990 - 11/1993 Health Physics Manager, Dames & Moore, Denver, Colorado 11/1977 - 03/1990 Progressive Technical and Management Position, EG&G Rocky Flats, Inc., Rocky Flats, Colorado
