Community and mental health services update For GPs and Practice Managers

This regular newsletter is to keep GPs and Practice Managers informed of news and updates relating to Oldham community and mental health services delivered by Pennine Care NHS Foundation Trust.

It is divided into ‘need to know’ and ‘good to know’ sections, to differentiate between updates about operational matters, such as changes to service delivery, referral routes or care pathways, and general service updates, such as news of good outcomes or a patient story.

UPDATE re: referrals into Oldham Audiology Service from January 2016 In the last newsletter we notified GPs that from Friday 1 January 2016, the Oldham Audiology Service has changed the way it receives referrals via the online e-Referral Service, which means GPs have to book a telephone appointment and give the patient a copy of the booking.

This appointment is a time at which the patient can telephone the audiology service to select and book a face-to-face appointment, it is not a time for them to go to the department to be seen.

There has been some GP feedback that the changes make it difficult for those who are deaf and cannot use the telephone to book an appointment.

We would like to assure GPs that steps have been taken to ensure the service is easily accessible to everyone, particularly those with hearing problems:

Patients are able to give consent to a friend, relative, carer or other health professional to telephone on their behalf to complete the referral

Where this is not possible and a patient is unable to telephone, the service will accept a traditional referral letter from a GP

The Oldham Audiology Service strongly encourages the use of e-Referral wherever possible, as this system allows referrals to be processed more efficiently.

Have your say on the Audiology Service: Pennine Care Audiology services at Bury, HMR and Oldham have been providing AQP audiology and hearing aid services for the past three years and will be continuing the service into the next financial year. This will enable the team to improve service delivery, ensure quality and build on partnership working with our GP colleagues in primary care. Thank you.

Developing the Appointment Centre The Oldham Appointment Centre is running a survey to gather GP views about the service.

The quick questionnaire - made up of a single question - asks GPs to rate how responsive the Appointment Centre Service is to their needs. There is also space for participants to leave comments and additional feedback if they wish.

The success of the survey is dependent on contribution from GP colleagues and the Appointment Centre team would welcome any honest feedback that may be helpful to future development of the service. The results of the survey will be shared and views will then be sought to create a development plan for anything highlighted as an area that can be improved upon.

Oldham Urgent Care Alliance outcomes

The Oldham Urgent Care Alliance is a high level strategic and operational multi-agency

partnership. The Alliance members work together to determine the strategic direction of the

health economy in Oldham, particularly in relation to integration, to achieve better outcomes

for local people.

Diabetes training options for GPs and Practice Nurses

The Oldham Diabetes Service, provided by Pennine Acute Hospitals NHS Trust and Pennine

Care NHS Foundation Trust, is delivering diabetes training modules open to GPs, practice

nurses and health care assistants.

Older people benefit from community mental health partnership A pilot community mental health service has supported more than 100 older people, helping to prevent many from having to be admitted to hospital.

The Oldham Social Care Liaison Service, provided by Pennine Care, is a team of mental health nurses that works in partnership with Oldham Council Adult Social Care Neighbourhood Teams, to provide mental health assessment and support to people aged over 65 in the community.

Social workers are able to contact the Liaison Service if they have concerns about any of their clients, who they feel may need some mental health care or support. By working together to ensure people get the right mental health support for their needs at an early stage, the service helps to prevent crises and reduce admission to hospital.

Oldham Social Care Liaison is a new community-based component of Pennine Care’s RAID (Rapid Assessment Interface Discharge) service, which comprises of liaison teams of mental health professionals who work mostly in hospitals to provide assessment and support for people on wards or presenting at A&E with mental health needs.

The service, which was launched in October 2015 and is running as a 12 month pilot, until September 2016, has received 103 referrals in its first four months. It is has prevented and estimated 31 hospital admissions and 27 A&E attendances, by providing mental health assessment and support in the community.

The Five Year Forward View for Mental Health – Pennine Care response On Monday 15 February, the NHS in England committed to a transformation of mental health care, pledging to help more than a million extra people and invest more than a billion pounds a year by 2020/21. This is in response to an independent taskforce report: ‘The Five Year Forward View for Mental Health’.

Pennine Care welcomed the publication of the Five Year Forward View for Mental Health report and its endorsement by Government and NHS England. Chief Executive, Michael McCourt, said: “As a Trust, Pennine Care fully supports the report and we agree mental health requires greater investment. We look forward to working with commissioners, partners and all stakeholders to develop and implement a new strategy for Greater Manchester as part of the Greater Manchester Health and Social Care Devolution project. “As a provider of mental health services across Bury, Oldham, the Rochdale borough, Tameside and Glossop and Stockport, we are always looking at ways in which we can improve the services and care we provide to patients and people who use our services, through innovation and development, within the budgets we have available.” Michael was live on BBC Radio Manchester discussing mental health in Greater Manchester on the day the Five Year Forward View for Mental Health report was published. Become a Pennine Care Governor Elections for Pennine Care’s Council of Governors will be starting soon, giving local residents the chance to influence the way health services in their area are run.

The Council of Governors is responsible for ensuring that community views are considered when Trust decisions are made and services are developed.
