The NSW Health Vaccination Portal is where you can book your COVID-19 vaccination appointment if you are receiving your vaccine at a NSW Health clinic. From the portal you can view your appointment details, reschedule or cancel appointments, and book appointments on behalf of a family member. Booking process overview During the booking process you will need the following: 1. Current mobile number and email address – you will receive appointment confirmation details and reminders by SMS and email. 2. Verification code – you will receive an SMS and email with a 6-digit verification code anytime you log in to the NSW Health Vaccination Portal. What is the NSW Health Vaccination Portal? The NSW Health Vaccination Portal is where you can book your COVID-19 vaccination appointment if you are receiving your vaccine at a NSW Health clinic. Book your COVID-19 vaccination appointment Part 1 – Receive your unique booking code.

1. You will receive your unique 8-character booking code via SMS. You will need this code to book your appointments.

Part 2 – Create your account in the portal and verify your details

2. Visit the NSW Health Vaccination Portal to create an account and verify your details. If you already have an account, proceed to part 3.

3. From the NSW Health Vaccination Portal home page, click Create your account.

The Create your account form will display.

4. Complete the Create your account form. In the Phone field, it is critical that you register the same mobile phone number that you received the booking code on.

A valid email address is also required to complete the verification process.

5. Confirm that you have read the use and disclosure of information statement.

6. Click Submit.

Your request to create an account will be submitted and an email will be sent to the email address you provided to verify your account.

7. Find and open the verification email sent to your inbox and click the Verify email link.

On the portal, a confirmation message will display that the registration is complete.

Part 3 – Log in to the portal and complete the two-step authentication process

8. Click Login to continue.

If you are returning to the portal and already have an account, click Log in (appears in the top right-hand side of the screen).

The Log in window will display.

9. Enter your user name (email address) and password.

10. Click Log in.

A Two-step Authentication screen will display and you will be sent a 6-digit one-time verification code.

11. Return to your inbox and find the email which contains the 6-digit one-time verification code (you will also be sent this in an SMS) and enter it in the Two-step Authentication screen on the portal.

12. Enter the verification code and then click Log in.

The NSW Health Vaccination Portal home page will display.

Part 4 – Enter your personal information

13. Click Enter personal information.

The Enter your personal information form will display.

14. Complete the Enter your personal information form. If you have a Medicare card, you must enter your full name, date of birth and residential address as you have it registered with Medicare.

15. Complete the Vaccination history section. If you have previously received a dose of the COVID-19 vaccine, you will need to provide the vaccine type and the date the dose was administered.

This information is used to determine the next dose that you need to book.

16. If you have already received Doses 1-2 or Doses 1-3 of the vaccine, you will need to complete section a) Booster or Dose 3 details.

If you have not had any doses of the COVID-19 vaccine you will need to complete section b) Preferred vaccine type. The selection you make will determine the list of clinics that you can book your appointment at.

17. Enter your address in the Enter address here field. As you start typing this will bring up matching addresses and you can select the correct address from the results displayed in the list.

This will populate the address fields.

18. If your address doesn’t appear in the list, clear the search and select the I want to enter manually checkbox to complete the address fields manually.

19. Complete the Medicare details section. If you have a Medicare card, please provide your card number, IRN (Individual Reference Number) and card expiry date. If you do not have a Medicare card, you are still able to register and book your COVID-19 vaccination.

20. Complete the Additional Details, Other details and Emergency contact details sections, and then click Submit.

A confirmation will display that you can now book your vaccination appointments.

Part 5 – Use your booking code to book your appointments

21. Once you reach the You’re ready to schedule your vaccination screen, you will need to make sure you have your unique 8-character booking code which was sent to your nominated mobile phone number via SMS.

22. Click Book vaccination.

The Schedule your vaccination form will display.

23. In the Choose a vaccination site section, select the booking code checkbox.

24. Enter your unique 8-character booking code that you received via SMS (part 1 / step 1).

The code is case sensitive, so ensure you enter it in exactly as it displays in the SMS.

25. Click Validate my booking code to verify the code. Booking codes can only be used once.

26. The system then validates the booking code. If the code is valid, a message will display that the clinic you have been advised to attend is selected.

If a message displays that your booking code is not valid, check that you have entered it in correctly (the code is case sensitive) and try again. Call the NSW Health Vaccination Booking Support Service on 1800 57 11 55 if you are still unable to proceed.

27. From the calendar select a) a date that suits for the first dose, b) followed by an appointment time from the options available.

Note: If you have already received the Dose 1 of the vaccine, you will only have to book an appointment for Dose 2.

28. From the calendar select a) a date that suits for the second dose, b) followed by an appointment time from the options available.

The date range available for the second dose is a set number of days apart from the first dose.

29. The Vaccine Screening and Consent section allows you to tell us about any medical conditions before you attend a clinic so that a Clinic Medical Eligibility Reviewer can ensure the vaccination is safe for you.

30. Indicate if you consent to receiving the vaccine.

31. Click Submit.

32. A confirmation page will display with your appointment details.

You will also receive this information in an email, which includes a QR code for check in. You will need to bring this with you to your vaccination appointments.
