Preplanning eases burden for loved ones

Ashley, a vibrant, ac-tive 37-year-old lawyer,mother of three youngchildren and part-timePilates instructor has ato-do list longer than hercombined client caseload. When she learnedthat her next-door neigh-bor, a mother of fourchildren who pal aroundwith her kids, had termi-nal cancer, her mind wentin many directions.

How could she help?What should she say?How will the childrenrespond? How was herfriend handling this cri-sis? But one thought in-truded regularly into herdaily routine – if thishappened to her, howwould she and her familyhandle the impending endto her life? She was clue-less on how to approachthe subject, but day byday, as she realized the

fragility of life, she be-came more convincedthat a conversation wascritical regarding herwishes and her hus-band’s.

This is both an ordi-nary and an unusual sce-nario. Ordinary becausefew people younger than40 have thoughts of plan-ning for dying. Unusualbecause Ashley didn’tdismiss her concerns.Rather, she wanted totackle the issue and getto-do items done.

Hospice of the West-ern Reserve recognizesthe courage it takes toapproach one’s end oflife. As one of the coun-try’s best-known hospiceand palliative care pro-viders, the agency offersthis kind of advice for allages in a booklet called“Courage in Conversa-tion: A Personal Guide.”

The guide tackles notonly the care you want inthe event that you are nolonger able to speak orthink for yourself, butalso how to begin thediscussion – as early inyour adulthood as pos-sible. There are a fewpoints to consider as youbegin this process:

1. Have a plan as tohow you will share your

wishes. Will you havethings written down?With whom will you betalking?

2. Create an environ-ment that is conducive tolistening. It is usuallyhelpful to sit down withyour loved ones and tryto be at the same eyelevel.

3. Share the informa-tion in small segments.

Avoid apologizing for theinformation you are shar-ing. These are your wish-es for one of life’s mostimportant moments.

4. Allow time for yourloved ones to processinformation and respond.This is one of the mostimportant things you cando. They may have ques-tions or feelings to sharewith you, but may needtime to process yourdesires.

Next steps may in-clude researching re-sources to help supportyour loved ones, such asfuneral and financialarrangements, creationof advance care docu-ments – your living willand your powers of at-torney - or simply statingwhere your advance careplanning documents willbe stored.

There are some whothink that the word “hos-pice” means that death isimminent. They equatehospice with the last daysor even hours of life, andthat can be a frighteningthought. There are otherswho have the perceptionthat receiving hospicecare hastens death. Butthe truth is, that’s a mis-conception. Hospice ser-vices are, indeed, avail-able to those who have adiagnosis of six months orless to live. But manyhospice patients live long-er than that and, in fact,several studies haveshown that those receiv-ing hospice care live long-er and with an enhancedquality of life.

Why hospice and whatdoes it provide? Hospiceservices have existedsince the late 1960s, whenDame Cicely Saundersfounded the first modernhospice in a suburb ofLondon, England. Hospicecame to the U.S. just afew years later and was

publicly supported in herbreakthrough work, “OnDeath and Dying,” by Dr.Elisabeth Kubler-Ross.Kubler-Ross testified insupport of hospice ser-vices before the U.S.Congress, stating, “Welive in a very particulardeath-denying society. Weisolate both the dying andthe old, and it serves apurpose. They are re-minders of our own mor-tality. We should not in-stitutionalize people. Weshould give families morehelp with home care andvisiting nurse, giving thefamilies and the patientsthe spiritual, emotionaland financial help in or-der to facilitate the finalcare at home.”

Although hospice bene-fits became legislativelycovered in the late 1970s,hospice is far more than aMedicare-funded pro-gram. Hospice servicesincorporate the use of amultidisciplinary teamwhich includes physi-cians, nurses, social

workers, chaplains andvolunteers. The emphasisis on pain control, pallia-tive care rather thancurative treatment. Andthe goal of hospice is toprovide an optimal qual-ity of life for whatevertime the person has re-maining, meeting theirneeds as a whole person –physically, emotionallyand spiritually.

The team not onlyworks with the patient butalso with the loved ones,both family and friends,as appropriate. Helpingthe loved ones to supportthe patient through thejourney can make a tre-mendous difference.Family care continueswith a full year of be-reavement services fol-lowing the patient’s deathto help with grief andmove loved ones forward.
