Central Information
To gather information regarding our Mental Health services, please call us at the information listed
below. The Mental Health Intake Coordinator will provide program information to assist referral
sources in completing the necessary paperwork which will help determine the most appropriate
and beneficial services for the individual referred.

Credo Community Center is a New York State Office of Mental
Health licensed agency.
The Credo Outpatient Mental Health Clinic is staffed by clinical therapists who are licensed, master’s level professionals committed to providing quality care and focused on helping patients and their families. Counselors provide assessment, evaluation and psychotherapy. Our psychiatric Nurse Practitioner provides assessment, evaluation, consultation and medication management services. Respect for client confidentiality, and safeguarding all information, as mandated by law, is of utmost importance among our practitioners. Our clients are children and adults age 16 and over. Credo fosters strong self-esteem, respect for community, a sense of purpose and the development of coping skills necessary for the achievement of their highest potential, for all patients. All professional mental health services are provided in a manner consistent with ethical principles, accepted professional practices and individualized to meet the needs of our patients. Sessions are conducted in confidential offices and group spaces.

Seeking Treatment Seeking help from a mental health professional could lead to reducing or dealing more positively with life’s difficulties. Those in need of the treatment could exhibit any combination of the following: Adolescents • Physically and verbally striking out • Excessive crying and tearfulness • Negativity • Problems with concentration • Inability or refusal to take direction • Isolation or lack of communication with family and peers • Dramatic changes in grades at school • Behavioral or attendance referrals from schools • Secretiveness about friends or running with “wrong” peers • Self-harming activities • Bullying • Unusual irritability • Changes in personal hygiene or appearance
