    Abstract--A main component of current malaria control strategies to reduce malaria-related mortalityand severe morbidity is early diagnosis and treatment at peripheral health services such as village healthposts and dispensaries. This strategy has been promoted mainly by sensitising the population withregard to the available service offered and by providing classical biomedical descriptions of symptomsand signs of malaria. This strategy represents important challenges for successful implementation andmaintenance. Early treatment depends upon prompt recognition of symptoms and signs of malaria inthe household, i.e. mainly by women. Early treatment also requires that appropriate health services andmedication are accessible and used. In this paper we argue that the success of malaria control dependsupon an approach that is gender-sensitiveand takes into account the level of endemicity in a given set-ting. The level of endemicity determines which group of the population is at highest risk for infection,morbidity and mortality, and is strongly related to gender considerations. The paper develops a typol-ogy that combines the key factors of gender variables with epidemiological features. It consequentlyoutlines an approach to community-based, effective malaria control tailored to a given endemic setting.Finally, we suggest that the proposed framework could be validated for its potential application to thecontrol of other communicable diseases. 1997 Elsevier Science Ltd.     INTRODUCTIONAND CONTEXTMalaria is the most important parasitic disease andremains of highest public health importance. Thestatistics are frightening with an estimated 300-500million people infected of which 2 to 3 million dieevery year (WHO, 1995a). The global impact o fmalaria is estimated at 35 million DALYs (disabil-ity adjus ted life-years) (World Bank, 1993), i.e. anannual loss of 35 million future life-years due topremature mortality and disability. Notably, 90%of the world's malaria burden is found in sub-Saharan Africa. The control of malaria remains oneof the wor ld's greatest public health challenges, par-ticularly as we are still waiting for an efficaciousand applicable vaccine (Tanner e t a l . , 1995).Moreover, some of the established tools such asanti-malarial drugs and insecticides are showingreduced efficacy in most endemic areas owing to thedevelopment of resistance among parasites and vec-tors. The 1993 Global Conference on Malaria re-iterated these points and outlined concepts andstrategies for each of the eight major endemic set-tings and malaria paradigms (WHO, 1993).Emphasis was placed on tailoring malaria controlto the local situation, i.e. considering the social,ecological and political context of a given area andits overall health and development plans. The reasons stem from the failure to incor-porate an interdisciplinary perspective in the designof malaria control strategies and from insufficientconsideration of the social and cultural context ofinfection and disease.The importance of social, cultural and economicfactors in malaria control for successful treatmentand health promoti onal interventions has beenemphasised an d illustrated frequently (reviewed inGomes, 1993; Agyepong, 1992a,b; Mwenesi, 1993b;McCombie, 1996; Agyepong e t a l . , 1995).
