Mission

The mission of the Dr. Allan L. and Mary L. Graham Clinical Performance Center (GCPC) is to

help educators in the health professions improve clinical performance and patient safety by using

simulated clinical encounters for

Instruction

Assessment

Quality Improvement

Research

Who We Are and What We Do

The GCPC supports the learning and assessment of communication skills, data gathering, clinical

reasoning, decision-making, procedural skills and team skills through simulation-based

educational programs. Our programs leverage standardized patients, task trainers, mannequins

and other simulations to provide learners with opportunities for safe, repeated practice of clinical

skills, in an environment rich with feedback and coaching.

GCPC facilities include two eight-room “clinics” for standardized

patient encounters, two mannequin simulation suites, two procedural

skills labs, and two classrooms, all equipped with AV recording and

playback capability; as well as office and administrative spaces.

The GCPC is affiliated with the College of Medicine’s Surgical Skills

Lab and Perinatal Simulation Lab, together comprising the UIC Health

Sciences Simulation Consortium. The Consortium was re-accredited by

the American College of Surgeons as a Comprehensive Accredited Educational Institute in 2017.

The GCPC is a unit of the Department of Medical Education (DME) in the UIC College of

Medicine. We serve medical students and residents, nursing and nurse practitioner students,

pharmacy students and residents, hospital staff including nurses and respiratory therapists,

physicians in practice, and mid-career professionals in the DME’s Masters of Health Professions

Education (MHPE) and Masters in Patient Safety Leadership (MPSL) Programs. Health

Professions educators from around the world attend certificate and short-term fellowship

programs at the GCPC to learn how to implement simulation activities effectively in their own

schools.

GCPC faculty and staff serve as workshop instructors, plenary speakers and Special Project

faculty for health professions students and residents at UIC. We disseminate knowledge gained

through our activities in peer reviewed publications, at workshops across the US and abroad, and

at invited and peer reviewed presentations at national and international conferences, including

the American Association of Medical Colleges (AAMC), the International Meeting on

Simulation in Healthcare (IMSH), the Association of Standardized Patient Educators (ASPE),

and others.

GCPC Highlights - 2016 and 2017

GCPC Expansion

GCPC staff have been working closely with

architects from Holabird & Root + SLAM to design

the new College of Medicine simulation facility on

the fourth and fifth floors of the building housing the

Mile Square Health Center on Roosevelt and Wood.

The move will more than double the space available

for simulation activities, allowing the GCPC to meet

the growing demand for simulation-based

educational programs for learners at all levels of

training and practice. The new center is projected to

be complete in 2019.

Staffing Updates

The GCPC welcomed Dr. Christine Park as Co-Director In the fall of 2017. Bob Kiser rose to the

position of Associate Director in the spring of 2016 while Laura McKenzie became Assistant

Director. Raina Long joined the GCPC team as an SP Coordinator and Angelo Dindia as a

Technology-Based Simulation Coordinator. Read more about our new team members in the

Faculty and Staff update below.

American Heart Association Training Site

The GCPC was accredited as an AHA Training Site in 2015, enabling us to provide training in

Basic Life Support, Advanced Cardiac Life Support, and Pediatric Advanced Life Support at

both the learner and instructor level for students, residents and staff. In 2017 we took on the

majority of CPR training from the UIC Safety Department, and became the main BLS training

site for University of Illinois Hospital physicians.

We are currently applying for AHA Training Center status.

Professional Development Programs for Simulation Educators

The GCPC Certificate Program in Standardized-Patient-Based Education is an intensive one-

week program that provides faculty and simulation educators with the knowledge and skills

needed to create effective instruction and assessment programs utilizing standardized patients

(SPs). The certificate program has taken place annually since 2012, and is one of only two

programs in the US dedicated to teaching faculty about SP methodology. National and

international participation, which has included participants from Switzerland, Ethiopia, Antigua,

and Nigeria, provides a stimulating peer group and promotes networking opportunities.

In 2016 and 2017 we added two 2-day advanced workshops for SP educators: Training SPs to

Debrief and Provide Feedback, and Advanced Instruction and Assessment with SPs.

In May 2017 Dr Rachel Yudkowsky, Director of the

GCPC, and Bob Kiser, Associate Director of the

GCPC, conducted a series of professional

development workshops based on the SP educator

certificate program for faculty and SP educators at

Plymouth State University in Plymouth, NH.

Participants improved their skills in writing SP cases,

training SPs, managing an SP program, and

developing SP-based assessments.

In October 2016 Bob provided a 2-day workshop on

SP debriefing at Baylor University in Houston, Texas.

Rachel also provided workshops and presentations at

the Sentara Center for Simulation and Immersive

Learning, Eastern Virginia Medical School, Norfolk

VA; University of Minnesota College of Medicine, Minneapolis MN; Oakland University

William Beaumont School of Medicine, Rochester MI; Northwestern University Feinberg

School of Medicine, Chicago IL; and SUNY Downstate Medical School, Brooklyn NY.

The GCPC’s professional development programs have had an impact both globally and

nationally. For an interactive map of GCPC programs go to http://tinyurl.com/kolhu69

Resident Orientation Boot Camp

All new UIC PGY1 and PGY2 residents come to the GCPC during their orientation week to be

coached in four basic procedures: IV insertion, arterial puncture (ABG), lumbar puncture (LP),

and basic airway /Oxygen delivery. This “boot camp” ensures that they are well prepared to

perform procedures under supervision on their first night on call. In 2017, at the request of the

hospital and GME office, we added a “patient handoff” station in which new residents practiced

giving and receiving patient transfer information according to a standard protocol.

Medical Students and Physical Therapist Students learn from each other with the help of SPs

In an innovative interprofessional education (IPE) collaboration between the College of

Medicine and the College of Applied Health sciences, physical therapy students and faculty, in

conjunction with GCPC SPs, teach second-year medical students the musculoskeletal physical

exam. In a return visit, fourth-year medical students, again with SP assistance, teach PT students

to perform the abdominal exam. In addition to learning physical exam skills, students learn to

work with and appreciate other professions.

High School Simulation Immersions

High school students from around Chicago came to the GCPC for a half-day simulation

experience as part of the UIC Emergency Medicine sponsored "Champions" high school

program, in which high school juniors and seniors from low-income neighborhoods are trained to

be health advocates in their own communities. At the GCPC high schoolers learned to obtain an

EKG, conduct an ultrasound exam, and provide CPR, and put their new skills into practice in an

exciting high-fidelity mannequin simulation. In 2018 we will be repeating this project for UIC’s

Urban Health High School program.

A similar simulation immersion was conducted in 2016 and 2017 for Northwestern University’s

National Student Leadership Conference, in which high-school students from around the world

come to NU to explore the qualities of effective leadership and gain an insider's perspective on

some of today's most sought-after professions. Students enjoyed the challenge of performing

procedures on task-trainer models and engaging in high-fidelity clinical scenarios.

Research and Development Projects in the GCPC

Meeting Service Needs of Veterans Experiencing Homelessness: A Process

Improvement Initiative Using USV's: In this collaboration with Drs Saul Weiner

(Internal Medicine, JBVA) and Alan Schwartz (DME) and their research team, the GCPC

is sub-contracted to recruit, train and manage sending incognito SPs around the country

as unannounced standardized veterans to assess the services provided to vets. Phase 1

was completed successfully in 2017, Phase 2 is currently in progress. Funded by the US

Department of Veterans Affairs.

Family-Physician Communication Study: Drs Michelle Barnes and Amanda Osta

(Pediatrics), Alan Schwartz (DME), Linda Gilkerson and Rachel Justice (Erikson

Institute) and Alisa Seo-Lee (Stroger Hospital) are evaluating the impact of a training

program to improve pediatric residents’ empathy and communication with parents and to

increase family satisfaction with residents’ communication in the clinical setting. The

UIC pediatric resident communication OSCE provides the context for outcome measures

for the study. The study is funded by a grant from the Erikson Institute and the

Association of Pediatric Program Directors. Previous pilot data showed an increase in

empathy and mindfulness and satisfaction with communication as reported by the

residents. The current study expands the mixed methods design to include a simulated

patient protocol as well as self-report measures and resident and program director

interviews.

Immersive Touch Module Development: Drs. Cristian Luciano and Rachel

Yudkowsky (DME, GCPC) worked with Dr. Pat Banerjee of the UIC College of

Engineering and Immersive Touch Inc to develop new simulator modules and curricula

for the Immersive Touch augmented reality/haptic simulator, focusing on Lumbar

Puncture.

Core + Clusters Physical Exam: The GCPC and Dale Lorens (DME), Dr Asra Kahn

(IM) and Dr Ananya Gangopadhyaya (IM), directors of the Essential of Clinical

Medicine (ECM) Program are collaborating with faculty from Columbia, George

Washington University, Harvard, University of Connecticut, and Texas Tech to develop a

new approach to teaching and assessing the physical exam while promoting clinical

reasoning.

Telemedicine Curriculum for Internal Medicine Residents: Drs Ananya

Gangopadhyaya and Mahesh Patel (IM) and Rachel Yudkowsky (DME, GCPC), along

with Jerry Stapleton (DME) and Bob Kiser (GCPC), continue to develop an online

curriculum to teach residents to work effectively in an online environment. The

curriculum includes live tele-encounters with standardized patients to allow residents to

experience telemedicine and practice telemedicine communication skills. This work

begun with funding from the UIC Center for Excellence in Teaching and Learning

(CETL).

C.A.R.E.S.: Clinicians Acting to Reinforce Empathy Skills. This curriculum was

designed to strengthen medical students’ empathic skills through the use of acting

exercises and techniques. Students learn to identify empathic moments through role-

playing both patients and clinicians within simulated clinical encounters. Project

collaborators include Drs Josh Nathan, Amber May, Kelly Volpe, Sean Blitzstein and

Geri Fox (all from Psychiatry), Dr Fred Zar (IM), GCPC staff Martin Hurm, Bob Kiser,

Laura McKenzie, Lynda Shadrake and Shole Milos, and medical student Katherine Bock.

The GCPC team members continue to be instrumental in developing the course content,

including acting exercises and patient characteristics and scenarios, and teaching the

course alongside a physician co-educator. The pilot project was originally funded by the

UIC Center for Excellence in Teaching and Learning (CETL).

Biomedical Visualization Projects. The GCPC collaborates with the Biomedical

Visualization (BVIS) masters degree program of the College of Applied Health Sciences,

providing a venue for BVIS students to observe and illustrate medical procedures as well

as opportunities for thesis projects.

o 2016 BVIS student Carrie Shaw collaborated with faculty of the COM Essentials

of Clinical Medicine geriatrics curriculum to create The Alfred Lab, an interactive

virtual reality experience to teach health professions students about aging.

Wearing a VR headset, students can experience a taste of what it’s like to be an

elderly man with visual and hearing impairments. The Alfred Lab was piloted in

the geriatrics curriculum for second-year medical students.

o 2017 BVIS student Emma Chai is working with Drs Yudkowsky and

Gangopadhyaya to create an online resource for COM students to practice

conducting a virtual pelvic speculum exam.

o Additional BVIS student projects are in progress in the GCPC Mixed Reality Lab,

Application of Haptic Virtual Fixtures in Psychomotor Skill Development for Robotic Surgical Training

Cecilia Gatti

MS (May 2017)

Visual, auditory, and touch senses have

been widely used in training. In particular,

tactile (cutaneous) and kinesthetic

(muscle) sensing are the main haptic

feedback modalities used by human beings

to interact with the world around them.

Recently, the combination of haptics and

virtual reality (VR) has been able to

improve motor skill training. The

technique of visually and/or haptically

magnified errors (known as error

augmentation) has shown positive results

in skill development. However, the use of

haptics in psychomotor learning is still not

completely understood and thus underdeveloped. The goal of this project was to provide a

platform for conducting research on psychomotor skill learning to better understand how haptic

error augmentation can provide more effective methods to improve training and facilitate re-

learning.

Haptic and Virtual Reality Surgical Simulator for Training in Percutaneous Renal Access

Andrea Faso

MS (May 2017)

Percutaneous renal access (PCA) is a

technically challenging surgical procedure that

requires extensive training and presents a steep

learning curve for residents. Traditional PCA

trainers using manikins and animals are

inaccurate, highly priced, and subject to quick

tissue degradation. Moreover, prolonged

training with fluoroscopic guidance exposes

trainees to high amount of radiation, limiting

its benefits. Currently-available virtual reality

simulators (VRSs) for PCA are unable to

realistically recreate the tactile sensation

perceived when puncturing real patient anatomy. Thus, a novel haptic VRS for PCA was

developed using a low-cost 3D Systems TouchTM 3D Stylus haptic device. A preliminary

evaluation of this system suggests the simulator is able to improve the performance of medical

students developing the psychomotor skills needed for conducting PCA.

Autonomous Training for Central Line Placement Using Mixed Reality and Machine Learning

Nihar Sheth

PhD (expected July 2018)

This project aims to develop and evaluate

an autonomous mixed reality simulator

for practice and performance assessment

of CVC placement. Trainees often forget

some of the 23 surgical steps, perform

them out of order or incorrectly, lack

proper technique for hand and instrument

manipulation, and are unable to correct

their own errors. The “instructor-less”

mixed reality proctor, which will be

tested in two teaching hospitals, uses a

patient-derived physical manikin, a

tracking system of trainee hand and

instrument movements, a 3D display to show virtual internal anatomy and ideal instrument

guidance, and an artificial-intelligence-based algorithm to autonomously coach and score

medical residents in learning this procedure without risk to patients.
