About our Trust

Norfolk and Suffolk NHS Foundation Trust provides inpatient and community-based mental health

services in both Norfolk and Suffolk.

It is also commissioned to provide in some of its localities, substance misuse and learning disability

services, along with other specialist services including forensic secure mental health and wellbeing.

The Trust believes in recovery and wellbeing, and understands the importance of good physical health,

maintaining relationships and achieving a balance between treatments and continuing an active life.

· Formed in January 2012 following the merger of Norfolk and Waveney NHS Foundation Trust and Suffolk Mental Health Partnership NHS Trust

· NSFT is the 7th largest mental health trust in the UK, covering 3,500 square miles across Norfolk and Suffolk, including Great Yarmouth and Waveney – equivalent to 7% of the country’s landmass

· The Trust provides more than 70 services, in more than 20 locations, and at over 40 sites, as well as in community settings including GP practices, places of education and people’s places of residence such as care homes, places for looked after children, and people’s own homes

· Services are offered to more than 55,000 service users per annum, from a population of around 1.6 million people spanning Norfolk and Suffolk

· Services include support for a wide range of low level and highly complex mental health and neurodevelopmental conditions, from anxiety and depression, to ADHD and autism, schizophrenia, eating disorders, personality disorders and dementia

· The Trust’s Childrens, Families and Young People’s services provide care for over 10,000 young people

· NSFT employs just under 4,000 (3,968) members of staff

· It has c400 inpatient beds across Norfolk and Suffolk

· NSFT has an annual turnover of £202 million

How we deliver local services

Our operating model provides a framework for the management, governance and delivery of all our services.

While the naming of the teams is different in Suffolk they follow the same basic structure. The locality

structure is co-terminus with our CCGs (and local authorities) but we are now starting to fully focus on clinical

pathways as well. Our recent work on our Clinical Strategy has followed and developed this pathways

approach, meaning that we are moving towards a matrix structure.

In line with our strategic goal of being ‘one Trust’ we are looking for opportunities to move to single

management structures. An example of this is the current move to create a single management structure

overarching our Wellbeing services, commissioned independently in both Norfolk and Waveney and in

Suffolk, offering a single structure across the two counties.

Our localities consist of:

· West Norfolk

· Central Norfolk (Norwich, South and North)

· Great Yarmouth and Waveney

· West Suffolk

· East Suffolk

· Suffolk wide

We also have a further definition of clustered services called Specialist Services. This incorporates Norfolk

Recovery Partnership (substance misuse); Secure Services (Norfolk and Suffolk); and Wellbeing (IAPT -

Norfolk and Suffolk).

Within our clinical services, working as a team to manage services locally, this operational model is led in each locality by a:

· Locality Manager – ensuring resources available and well-managed

· Modern Matron – monitoring and promoting quality standards

· Lead Clinician(s) – setting the clinical direction

· Supported by corporate services – including HR business partners, management accountants,

governance team, PMO, estates, etc

This team works together to ensure the delivery of:

· Quality care

· Staff engagement

· Financial balance

· Stakeholder management

· Contract requirements

Our operational model – incorporating corporate support and specialist services - has six objectives:

· Simplify and standardise structures

· Specify responsibilities so lines of accountability are easily visible

· Align governance and support structures to provide effective support to front-line services

· Strengthen how Trust objectives are cascaded to align everyone’s efforts

· Strengthen how issues are escalated for additional support or intervention

· Support devolved / earned autonomy by providing a risk-responsive assurance system that encourages innovation and safeguards against quality failure

Adult Acute Services These services provide a range of intensive support, admissions and psychiatric intensive care.

Inpatients NSFT provides several acute wards across the two counties taking adult admissions. We provide high standards of treatment and care for patients requiring a safe and therapeutic setting to promote recovery. All operate 24 hours a day, seven days a week.

Psychiatric Intensive Care Units (PICU) These wards provide an age inclusive service for people who have acute mental health issues, offering assessment and treatment for those detained under the Mental Health Act and those whose risks are such that they cannot be managed safely on an open acute ward. Both operate 24 hours a day, seven days a week.

Crisis Resolution Home Treatment (CRHT) The service provides intensive support for people during a mental health crisis, delivering prompt and effective home treatment, including medication, and brief psychological interventions in order to prevent hospital admissions and give support to carers. It also acts as a gatekeeper to other mental health services such as acute inpatient care, referring people to other services if longer term follow up is indicated. All operate 24 hours a day, seven days a week.

Home Treatment The home treatment teams provide an alternative to admission for adults experiencing acute mental health problems. With both the use of psychiatric medication, and an emphasis on the benefits of exercise and healthy eating, patients are offered a creative, forward thinking approach to achieving good mental health. Support is also given to those discharged from inpatient wards as required.

Adult Community

NSFT provides a wide range of community focused mental health services for adults across Norfolk and Suffolk, where care can be offered within a person’s home or in a community based facility. Our services focus on providing effective interventions using the best evidence available and reducing reliance on inpatient services.

These services are for adults over 25 with moderate to severe mental health needs who require support in the community. The teams provide assessment, treatment and support and follow the principles of recovery and social inclusion, and work closely with voluntary and statutory partner agencies. They also provide carer assessments and community care assessments.

Integrated Delivery Teams (IDTs) In Suffolk, the NSFT operational model is based on integrated teams. The IDTs are responsible for the coordinated delivery of community mental health services, providing support for people of all ages with mental health within the designated locality. This includes early intervention and support for children and their families, adolescents, working age and older adults, and individuals with a learning disability and attention deficit hyperactivity disorder (ADHD) . All teams operate Monday to Friday, 9am to 5pm but link with other service areas such as the Access and Assessment, and Home Treatment Teams and Acute Inpatient Services to provide a 24-hour assessment and intervention service.

Access and Assessment The Access and Assessment service is designed to make it easier for patients to get the right mental health and social care services as quickly as possible. GPs, and other health and social care referrers, can refer via a central number for triage and direction to the most appropriate service.

Learning Disability and Neurodevelopmental Services Learning Disability The NSFT LD Service is commissioned in Suffolk only. In Norfolk we work alongside other NHS partners who are commissioned to provide these services.

The Suffolk service offers specialist interventions to people with a learning disability and a mental health disorder. Our range of professionals can cover physical health, mental health, behaviour, communication, physiotherapy and daily living skills. We also have dedicated Social Work support. We offer both clinic-based and community-based interventions and rehabilitation for people with learning disabilities who have had an extended stay in a more restricted environment.

Veteran’s Response Partnership This service takes expert help and peer support directly to veterans facing a mental health crisis across Norfolk and Waveney. Liaison and Diversion These teams provide mental health services to police investigations centres and magistrates courts. In addition the service runs personality disorder projects within HMP Wayland and through the Norfolk and Suffolk Probation Service. Children, Families and Young People’s Services

The aim of the various children’s services teams is to support and help children and young people who are experiencing moderate to severe emotional and mental health problems. It also offers support to their families and carers, and provides advice and support into places of education, as well as working with agencies which look after children’s services.

Early Intervention in Psychosis (EIP) The EIP teams support people between 14 and 35 in Norfolk and Waveney who are experiencing symptoms of their first episode of psychosis. This support is intended to help patients feel better and to reduce the impact of the symptoms and also aims to help patients and their families understand more about the symptoms of psychosis and what treatments are available. In Suffolk the EIP function is contained within the IDTs.

Intensive Support This team offers services to young people aged 12 to 18, who are already receiving mental health support and who have severe mental health needs. Its services are provided in the community it aims to prevent the need for admission as an inpatient.
