  • Enrollment Checklist 2013-2014

    Documents required for enrollment

    Completed Official Birth Certificate or Passport

    Student must be born on or before September 1, 2008. Certificate should have official seal from state/country issued.

    Physical Exam Physical must have been within the previous 12 months prior to enrollment.

    Florida Immunization Form Completed by physician DH 680.

    Custody Documentation If divorced, official court documents regarding custody must be provided to school.

    Proof of Residency See other side.

    Official transcript from previous school Mandatory for High School students.

    Withdraw form or last report card from previous school For Elementary and Middle school students.

    *Parent/ Guardian enrolling student must produce photo ID

  • Enrollment Requirements 2013-2014

    Proof of Residency

    Standard Parents live w/someone else Child lives w/someone else Provide ONE of the following: Current Collier County Homestead

    Exemption Card (considered familys primary residence).

    Current Collier County Property Tax Notice (may require additional verification).

    Home Purchase Contract in Collier County, specified closing date, with a copy of the deed to be provided within 30 days of closing date.

    Copy of a Collier County Manifestation of Domicile filed by the parent.

    A current rental or lease agreement.

    AND TWO of the following: Current Florida driver license or

    Florida Identification Card. Automobile insurance (last two

    statements). Current electric billing statement,

    water bill, cable bill or landline phone bill (last two statements).

    *New residents must provide confirmation of set up of services for 2 utilities with name and address (2 months bills must be submitted to the school within 60 days).

    Provide ALL of the following: Notarized letter from

    person they are living with stating that the parent and child live there and;

    Notarized letter from parent(s) that they, in fact, live with such person and;

    (3) proofs of address from person who wrote the letter to establish that they live in the school zone and;

    (3) proofs of address from the parent to verify that they live at the address.

    Provide the following: A notarized Power of Attorney with two witness signatures that must include the following information to be submitted to the District by the school for review prior to enrollment:

    Relationship of guardian to child.

    Guardian will be responsible for the childs educational needs.

    Length of temporary guardianship.

    The District will notify the school within 24 hours regarding status of enrollment. If the District approves enrollment of the child by the guardian,

    The guardian must sign District Statement of Acceptance of Responsibility Form.

    Parent/guardian must provide evidence of pursuit of legal custody through the courts to the school within 90 days (school will note on TERMS S313 screen).

    The District will follow-up with a letter to parent / guardian and school, as needed.
