REMEMBER – YOUR ENROLLMENT DEADLINE IS NOVEMBER 25.

Log on to Your Benefits Resources™ (YBR) website at www.ybrcanada.hewitt.com/slb or the NAM

RSC website. You will need your Global Identification Number (GIN) (with no leading zeros) and your

specific YBR Personal ID Number (PIN).

Click “Enroll Now!” and follow the online instructions to confirm or make changes to your coverage.

Complete your wellness survey (www.healthycommunity.ca/MS/schlumberger/Default.aspx) to

receive $100 in extra flex dollars that you can use towards your flexible benefits or deposit into your

Health Care Spending Account (HCSA) or Wellness Account. If it’s your first time, you’ll need to create

a user ID and password to use every time you visit the site.

Verify your personal information.

Do you need to update your dependent information? What about coverage for family members? Make

sure you’ve checked your family’s coverage for extended health and dental coverage.

Remind yourself about the other options available to you. For full details on all of your options, check

out your Enrollment Guide (www.slb-benefits.ca/flex-guide/).

Remember, dental Option 3 has a two-year lock-in period. This means you will be unable to change your

coverage for two years if you choose, or have chosen, dental Option 3.

Do you have excess flex dollars? If so, you can allocate them, tax-free, to your Health Care Spending

Account (HCSA) or Wellness Account. Note that Wellness Account flex dollars are taxed.

Check your beneficiary designations and update them as needed.

Review your choices. Only you know what’s right for you and your family. Make sure your benefit

choices will give you what you need for the next year.

Click “Complete Enrollment” to lock in your choices. You have until November 25 to make changes.

Just remember – you must click “Complete Enrollment” for your new changes to take effect.

Print a copy of your Confirmation Statement for your records.

THAT’S IT! YOUR 2017 ENROLLMENT IS COMPLETE.

IF YOU DON’T ENROLL …

Your coverage will be dropped to Option 1, with the exception of dental Option 3 (which requires two-year

participation), and you will not have any dollars directed to your Health Care Spending Account.

ADDITIONAL FORMS MAY BE REQUIRED.

If you increase your own or your spouse’s optional life insurance coverage or are applying for the first time,

you must complete and submit an Evidence of Insurability Form, which you’ll find under “Action Needed” on

the YBR website (www.ybrcanada.hewitt.com/slb). If the application is approved, your benefits coverage

will be updated to reflect your choice.

If you need to change your beneficiaries, you must print out your Beneficiary Designation Form, sign it and

send it to the address on the form. Forms are available under “Action Needed” on the YBR website

(www.ybrcanada.hewitt.com/slb). Once your signed form is received and verified by the Schlumberger

Canada Benefits Centre, your beneficiary designation will be updated.

DID YOU FORGET YOUR PIN?

You can request a new one online on YBR (www.ybrcanada.hewitt.com/slb). Just click on “I Forgot My PIN”

on the home page. A new PIN will be emailed to you within 15 minutes of your request.

It’s your only chance! You have until November 25 to make changes to your benefits coverage. If you miss

the deadline, your benefits will default to the lowest level of coverage and remain in place until December

31, 2017, unless you experience a life event.
