Intervention and Student Assistance Team Process PEP/Tier I Intervention Plan Team members consist of: o Classroom teacher o Parent Any student that is having difficulty, either academic or behavioral, must have a PEP/Tier I that addresses the area or areas in which he/she is struggling. Tier I –The classroom teacher completes the Intervention Form. An intervention plan must be implemented and progress monitored for 4 - 6 weeks. Two parent contacts need to be made and documented. o 1 at beginning of Tier I o 1 after 2 - 4 weeks to discuss results. After 4 - 6 weeks, a decision should be made: o Exit Tier I o Modify interventions & continue Tier I o Begin Tier II Decision is documented on the intervention form. (Include intervention results.) PEP/Tier II Intervention Plan Classroom Teacher o Grade Level Teachers o Other School Personnel as needed (ESL, Reading, etc.) o Parent Classroom teacher presents information about student to grade level PLC. Teachers brainstorm ideas and new interventions. Tier II letter sent to parent with request for Hearing and Vision Screening. Classroom teacher continues intervention form for Tier II. The Tier II intervention plan is started and progress monitored for 4 - 6 weeks. PLC reconvenes to discuss results of interventions. Decision is made: o Exit Tier II and move back to Tier I o Modify interventions & continue Tier II o Begin Tier III

Hearing and Vision Screening must be completed before moving to Tier III. If H & V is over 1 year old, it needs to be completed again at Tier II.

PEP/Tier III Intervention Plan Classroom teacher emails all paperwork and documentation to the Student Assistance

Team Chairperson If all paperwork is complete, the chairperson schedules a Tier III meeting. Chairperson will send a Tier III letter of notification to parent with Social Developmental

History form. Tier III meeting invitation sent to parent. (Student Assistance Team Process Starts)

Student Assistance Team Process

1 st SAT Meeting Team members consist of:

o Administratoro Counseloro Classroom Teachero Specialist o

SAT meetings will be held on __________________. Administrator leads the meetings. Chairperson takes notes on the computer. Classroom teacher presents all information from Tier I and Tier II. Team reviews assessments, interventions, and progress monitoring and brainstorms for

ideas and revisions to intervention plan. Decision is made by the team:

o New interventions to be implemented for 2 - 4 weeks.o Modify current interventions and implement for 2 - 4 weeks.o Referral to EC

Follow-up SAT meeting is scheduled by the chairperson (4 weeks)

Follow-up SAT Meeting After 4 weeks, the classroom teacher presents results from intervention. Decision is made by the team:

o exit Tier III and move back to Tier IIo continue on Tier III o EC referral

Chairperson submits paperwork to EC.
