Registration FeesThe full-conference registration fee includes admission to all educational sessions, including the

injectables and practice management workshops. The registration package also provides access to

the Exhibit Hall, coffee breaks, lunches, the Welcome Reception, satellite breakfast symposia and other

industry-sponsored events.

The spouse/guest registration fee covers entrance to the Exhibit Hall, coffee breaks, lunches and the

Welcome Reception. This registration category is not eligible for CME credit or verification of attendance.

All meeting cancellation and refund requests must be made in writing. Please attach your signed, written letter of request and send to [email protected] by Friday, March 16, 2018.

A refund of all fees, less a $175 administrative fee, will be given for cancellations received by this date. A reduced administrative fee of $75 is offered for spouse/guest registrations.

No refunds are issued for “no shows” or for cancellations received after March 16. Forfeited fees may not be applied to other AAFPRS events.

Deposit PolicyA deposit equal to the first night’s room and tax is required to confirm your hotel reservation. This deposit will be applied towards your hotel bill upon check-in.

Cancellation PolicyReservations may be cancelled up to 72 hours prior to the date of arrival without penalty. Cancellation requests received less than 72 hours prior to the date of arrival will be charged one night’s stay. Please be sure to obtain a cancellation number at the time of cancellation and retain this number for your files.

Reservation DeadlineThe $249 rate is only available until March 16, 2018, or until the block is sold out, whichever comes first. Reservations received after March 16, or after the block has been filled, will be made based on hotel availability, and may be subject to significantly higher rates. We encourage you to book your rooms as quickly as possible to guarantee the best pricing and availability.

Air Travel

Ground TransportationUpon arrival in Chicago, there are several ground transportation options to get you to the Sheraton and/or the downtown area.

Airport ShuttleGO Airport Express provides quick, easy and economical transportation to and from O’Hare and Midway airports and the Sheraton Grand Chicago. Shuttles depart every 20 to 30 minutes from the airport. Please note, this is a shared ride service and the shuttle may make several stops before reaching the Sheraton. The fare is $36 one way or $63 roundtrip from O’Hare and $29 one way or $55 roundtrip from Midway. Prices are approximate and subject to change without notice. Fares are per person and do not include the $2.00 per person MPEA Airport Departure Tax.

TaxicabTaxis are plentiful and easy to hail downtown, at the airports and throughout Chicago. Fares are approximately $45 from O’Hare and $35 from Midway; however (excluding tip), rates vary based on travel time and are subject to change.

GETTING THERE

Chicago is home to two major airports, O’Hare International Airport (ORD) and Midway International Airport (MDW). Together they offer with thousands of daily inbound and outbound flights to more than 240 destinations, making Chicago easily accessible from anywhere around the globe. O’Hare is approximately 17 miles from downtown and is one of the busiest hub airports in the U.S. Midway, a premier point-to-point airport, is only 10 miles from downtown.
