How to use AHLTA

Questionnairesin 13 easy steps

Step 1: Select patient from the appointments tab.Step 2: Click on “Patient Questionnaires” on sidebar

Step 2: Patient Questionnaires tab opens and lists all prior questionnaires completed for this patient.Step 3: Click on “Interview” in icon-list on top-bar.

Step 3: Interview Questionnaire box opensStep 4: Expand “Clinic” option, then expand “Questionnaires” option. (Click on +)

Step 5: Scroll down to “Ped Pri Care Be” and expand this option (Click on +)Step 6: Choose the questionnaire you need to complete. In this case, the MCHAT

Step 6: The selected questionnaire (MCHAT) opens up in the 2nd box.Step 7: If this is correct, click “Select”

Step 7: The selected questionnaire now opens up on the main patient screen.Step 8: Complete the questionnaire.•Some providers directly ask parents the questions•Some providers have parents fill out questions on their computer while they examine the child.

Step 9: Click “Done”, then “Close” in upper icons.

Step 10: Do you want to save your changes? YES

You are returned to the “current encounter”. You now need to import the completed questionnaire into your note.

Step 11: Open “Actions” menu and click “Options”

Step 12: The Options Box opens up. Click the check-mark for “Questionnaires”

•Even if it is pre-clicked, you need to “un-click” and “re-click” to successfully import the Qs.

Step 13: Your questionnaire is now imported into your note.

•Now you need to score and interpret the results!
