INDICATIONS FOR USE – iConnect Access

• The iConnect Access application provides Internet access to multi-modality softcopy medical images, reports, and other patient-related information to conduct diagnostic review, planning and reporting through the interactive display and manipulation of medical data.

• The iConnect Access application can be configured to provide either lossless or lossy compressed images for display. The medical professional user must determine the appropriate level of image data compression that is suitable for their purpose.

• Display monitors used for reading medical images for diagnostic purposes must comply with applicable regulatory approvals and with quality control requirements for their use and maintenance.

• Lossy compressed mammographic images and digitized film screen images must not be reviewed for primary image interpretations. Mammographic images may only be interpreted using an FDA approved monitor that offers at least 5 MP resolution and meets other technical specifications reviewed and accepted by FDA.

• iPads and other mobile devices can present diagnostic quality images, but the iConnect Access application on such mobile devices is not intended for diagnostic use.

INDICATIONS FOR USE – iConnect Access 3D

• iConnect Access 3D is intended to provide reading physicians, referring physicians, and other appropriate healthcare professional tools to aid in interpreting medical images, including:

• Displaying DICOM compliant medical image volumes, such as CT, MRI, and PET.

• Reformatting images, including creation of MPRs, MIPS, MinIPs, color/monochrome 3D volume rendered images

• Manipulating displayed images via control of slice thickness, slice interval, obliquity, perspective, rotation, window/level, crop, zoom, color/monochrome transformations, segmentation, sculpting, straightening the display of curved structures, and creating images perpendicular to a curvilinear path.

• Creating individually captured DICOM images that can be displayed and stored in a PACS.

• Measuring coronary calcium, which is intended for non-invasive identification and quantification of calcified atherosclerotic plaques in the coronary arteries using tomographic medical image data and clinically accepted calcium scoring algorithms.

Chapter 1 About the Application

The iConnect Access application is a portal technology platform that aggregates diagnostic data from multiple archives, then seamlessly delivers patient information to physicians without duplicating data or proprietary integration. Built on robust interoperability specifications, iConnect Access delivers standards-based, on-demand integration with any DICOM or XDS compliant repository irrespective of the vendor.

The zero-footprint universal viewer and image sharing tools provide clinicians and CIOs with a solution that overcomes the challenges associated with distributing images and reports to their referral base. Challenges typically include expensive hardware replacements, complex deployment, costly IT support, platform compatibility, and high bandwidth requirements.

In addition, the iConnect Access application provides the scalability vital to enterprise networks and multi-site systems. HTTPS facilitates secure transmission of images over the Internet while seamlessly integrating with any portal or EMR system. The application also provides audit logging which supports HIPAA compliance requirements for privacy and security of patient information.

iConnect Access is also available in a mobile device format for Apple iPhone devices; the Apple iPod touch, and the Apple iPad, all with extended accessibility for reports and images.

Chapter 2 New Features and Enhancements

This release of iConnect Access introduces the following new features and enhancements.

OCT Reference Lines

Added support for DICOM OCT Ophthalmology exam cross reference lines. In the Study Viewer, users can display the scout line and all cross sections on the reference OCT image.

Updated New MRN Feature in the Exam Importer and Web Uploader

The New MRN field is now available in both the Exam Importer and Web Uploader for all registered users. Guest users do not have access to the New MRN field. The New MRN field was previously available in the Exam Importer to both registered and guest users. The Web Uploader did not previously include the New MRN field.

Also, an global setting is now available for System Administrators to have the application prompt users if a New MRN is not entered when performing an upload. Users have the option to enter a New MRN or continue without a value.

Reconciliation Update and Destination Selection

The Reconcile/Archive Study dialog has been enhanced with a new Update feature that allows users to save the updated DICOM information stored in the holding pen without archiving it to a data source. The study state of the updated exam can be reconciled without changing the destination, and when re-routed to another destination the status changes to show that its uploaded.

The permission for the Update feature will be enabled for archivists in addition to the archive and route functionality that exists today. A new privileged type of user called “Updater” will inherit the receiver privileges on that destination and also have the ability to save/ update, but not archive.

The Reconcile/Archive Study dialog also includes a destination selector that by default contains the sent to destination, but allows the user to change destination to another based on the domain and role segmentation for the user.

Destination Role Based User Management

The Destination management page has been enhanced to allow a new user type other than receiver or archivist that has updater privileges. Users with updater privileges can update patient demographics, order notes, and/or select a destination to reconcile exams without archiving. This updater option is included at the role level to ensure any local or active directory user who is a part of the role has access to that destination's inbound data.

Destination Based MRN Prefix

The Destination Management page has been enhanced to enable MRN prefixing and allow a configurable syntax to be used to identify the destination that studies are sent to/from. Data is then automatically prefixed by the image sharing tools and stored to the holding pen.

NOTE: Once data is stored to the holding pen, changing destinations will not prefix the MRN, even if MRN prefix is enabled.

Invite to Upload

iConnect Access allows external guest users to access the main login page and use any email address to upload exams when Image Sharing is enabled. Registered users with the privilege to invite to upload can now invite other users to upload by sending an email with a link to upload studies.

The Invite to Upload Study feature is available from the Inbounds tab or Study Viewer. Users can enter any email address and the recipients name, select a destination, and write a free text comment. Once the email is submitted, a PIN will appear, that is configurable by how many digits in length and number of days until expiration. This PIN is then communicated through some other form of communication like phone or separate email.

Disable Guest User Login

A new global configuration setting has been added that allows system administrators to prevent guest email logins on the upload devices. When guest user login is disabled, only registered users (LDAP/ Local) are able to login to the upload devices.

NOTE: This configuration does not impact the invite to upload guests as they are confirmed guests with an access PIN.

Chapter 3 Fixed Issues

The following table lists the issues that have been fixed in this release.

ICA-11108 Audit Logging Duplicate login entries were occasionally being logged in the Audit log.

ICA-12150 XDS The Patient History Encounter column did not display values for the priors listed.

ICA-13250 Mobile On mobile devices, if more consultation notes are listed then can fit on the screen at one time, users were unable to access the notes that were not on the screen. Users can now scroll to access all the notes.

ICA-13691 Viewer On mobile devices, link icons were displayed on the thumbnail views. Link icons should only be displayed on the images.

ICA-13748 Mobile On mobile devices, when an emailed study was opened, the Study Viewer would not rotate orientation when the device was rotated.

ICA-13760 Image Sharing For new domains, the Exam Importer and PACS Gateway installer links were not displayed on the homepage if they were not generated for the SuperAdmin.

ICA-14477 Security DICOM tags could be used to include malicious Javascript in data sent to Enterprise Archive.

ICA-14588 Security A URL/parameter pair was vulnerable to reflected cross-site scripting which could be used to submit malicious JavaScript to iConnect Access via the URL.

ICA-14660 Viewer MPEG2 and MPEG4 video files were not playing in browsers Internet Explorer 11 and Edge.

ICA-15060 Study List Matching studies were not listed according to the Role Access Filters when filtering with multiple institutions.

ICA-15199 Study List The Study List Group By drop-down could become partially hidden. Also, studies with date “unknown” were not all grouped together.

ICA-15242 Image Sharing, Uploader Tool

Uploading studies using the Exam Importer would fail when a network fault occurred.

ICA-15262 Test Automation, Viewer

The browser could become unresponsive when linking three viewports on a study due to images not being fully cached yet.

ICA-15273 Test Automation, Viewer

The browser could become unresponsive after selecting Link All on a study for the first time due to images not being fully cached yet.

ICA-15292 Download Setting the Package Maximum Size to larger than 2048 MB in the Service Tool would cause study transfers to fail.

ICA-15302 Exam Importer When studies were uploaded from the Exam Importer, email notifications were not received for deleted studies.

ICA-15324 HTML5 Uploader When using the HTML5 Web Uploader in Internet Explorer 11, a popup message displayed “Did not find any supported files to upload, please select another folder.” if the user clicked “Drag and Drop Your Files Here For New Upload” more than once.

ICA-15325 PACS Gateway An error message may display after logging out from the PACS Gateway.

ICA-15337 Image Sharing Studies could contain unused private tags when uploaded to the Holding Pen.

ICA-15351 Reports PDF files with special characters in the file name would not load in the Study Viewer

ICA-15358 HTML5 Uploader When using the HTML5 Web Uploader, attachment uploads canceled by the user during upload could still continue to upload.

ICA-15372 HTML5 Uploader When using the HTML5 Web Uploader, if an email address or phone number is entered in an incorrect format, an error message is displayed when the user clicks Login. Users had to click Login again to dismiss the error message, and then again to continue to login.

ICA-15403 Image Sharing When searching studies from the Inbounds/Outbounds tabs, search strings entered in the First Name field produced search results that contained the string in either the First Name or Last Name fields.

ICA-15471 Mobile When rotating an iPAD to landscape orientation, the splash screen on the login page did not resize to fit on the screen correctly.

ICA-15472 Mobile The connection quality rating on the login page was not visible on the iPad mini.

ICA-15474 Mobile When viewing studies on an iPAD, images could be dragged from the thumbnails giving the impression users could drag and drop from the thumbnails.

ICA-15479 Image Sharing The Email ID field in the Exam Importer allowed users to enter an invalid email address format.

ICA-15489 Image Sharing The HTML5 Uploader configuration settings in the Service Tool did not include field labels to display the unit types.

ICA-15499 Mobile When viewing studies on an iPad in full screen mode, the link icon would display on the thumbnails.

ICA-15500 Mobile When viewing studies on an iPAD, users were unable to perform zoom and pan functions when the Scrolling tool was selected.

ICA-15501 Mobile When viewing studies on an iPAD, a viewport can become misaligned when the Safari browser is minimized and reopened

ICA-15502 Mobile When rotating an iPAD to landscape orientation, emailed studies would not display properly correctly.

ICA-15516 Viewer When viewing reports in the Chrome browser, a left click on the mouse would cause the mouse to get stuck to the scrollbar until a right click was performed.

ICA-15536 Email Study When deleting a study from the Outbounds tab, users could click OK multiple times which resulted in an error

ICA-15577 Email Study, Viewer

Emailed studies with a large number of priors could fail to load.

ICA-15610 Uploader Tool Encryption services were not synchronized to the WebUploader configuration.

ICA-15611 Security iConnect Access was vulnerable to Cross-Site Request Forgery (CSRF) attacks.

ICA-15645 Exam Importer A Call Home error would occur when logging into the Exam Importer after a system is restart and the Exam Importer auto starts in minimized mode.

ICA-15657 Viewer Secondary captures were not displaying in the Study Viewer.

ICA-15689 Viewer Studies with DX modality could display as washed out.

ICA-15700 Localization For localized applications, the warning messages on the self enrollment sign up page were not translated.

ICA-15718 Security iConnect Access was vulnerable to Inclusive Phishing Through URL Redirection.

ICA-15736 Viewer Modality LUT handling was not configured to match the DICOM standard for PR modalities.

ICA-15742 Viewer Using Stacking Mode when viewing studies with Integrated URL resulted in rendering errors.

ICA-15745 Conference Lists Users were unable to delete Conference Folder Managers when using Internet Explorer browser with High level security.

ICA-15760 Email Study, PACS Gateway

PINs generated for PACS Gateway and Email Study were stored in the database in plain text. PINs are now encrypted.

ICA-15774 Security iConnect Access had a security vulnerability with Query Parameters in SSL Requests.

ICA-15779 Patient History, URL Integration

When MPI and PID Domain are configured, some studies displayed an error when opened from the History panel, but opened normally from the Study list.

ICA-15784 Patient History When MPI and PID Domain are configured, the History panel would display the error message “Cannot identify patient by using configured patient services”.

ICA-15805 Email Study Email Study fields were hidden when emailing an ePDF study in Internet Explorer.

ICA-15818 Exam Importer Users were unable to upload JPEG files using the Exam Importer.

ICA-15820 Patient History, URL Integration

While launching iConnect Access through a patient-level URL integration, some studies displayed an error when opened from the History panel, but opened normally from the Study list.

ICA-15840 XDS When using Internet Explorer 11, XDS videos could take a long time to load.

ICA-15844 All, Security When updating the Protocols and Server FQDN in the Security tab of the Service tool, some configuration files did not apply the updates.

ICA-15855 User Management

iConnect Access did not enforce adequate access controls to prevent users from changing others passwords.

ICA-15856 Security The iConnect Access session cookie did not have a Secure Flag attribute to prevent discloser over an unencrypted connection.

ICA-15859 Viewer If a PR referenced several multiframe images, frames could display in an incorrect sequence.

ICA-15901 Conference Lists In the Conference Folders tab, Active Directory users could not add subfolders to top level folders, or studies to conference folders.

ICA-15965 Viewer When using the Edge or Firefox browsers, the Study Viewer could flicker when scrolling through images.

ICA-15972 Viewer When using Windows 10, PR images were not created for studies with a large number of images.

ICA-15994 Web Uploader PDF file names could be changed when uploaded from the Web Uploader.

ICA-16048 Image Sharing POP PINs were displaying as invalid logins in the Windows Event Viewer.

ICA-16085 Image Sharing Delete notifications with the parameter “twophasedelete = false“ were being sent to the holding pen. This resulted in studies being completely removed from the holding pen instead of being placed into the deleted folder.

ICA-16125 Exam Importer, Image Sharing, PACS Gateway

Uploaded studies were duplicated if reconciled while they were still in the process of uploading.

ICA-16134 Viewer When using XDS for PDQ, SAD as PIX, an exception occurred when no patient was returned from PDQ while loading a study.

ICA-16195 XDS The DocumentSourceType list was not passed to the server for XDS document retrieval.

ICA-16225 User Management

The Create/Edit Roles pages did not include the Inbounds and Outbounds tabs as options for Default Start Page.

ICA-16312 Image Sharing Some patient data could not be retrieved when reconciling a study for the first time.

ICA-16317 PACS Gateway The POP Priority Queue was set to LIFO instead of FIFO.

ICA-16334 Image Sharing When Enterprise Archive received images with invalid character sets, it issued an association abort which resulted in those studies remaining in Upload Scheduled.

ICA-16341 PACS Gateway After the first reboot since installing the PACS Gateway, an error would occur when opening the PACS Gateway.

Chapter 4 Known Issues

Case-Insensitive Accession Number

Grant Access is case-insensitive. As a result, if Accession Numbers are not handled consistently regarding letter case and have similar accession numbers, studies may not be visible in the user’s Inbound Study List.

Image Sharing

The PACS Gateway, Exam Importer, and Web Uploader may not be able to upload studies that contain multi-byte character sets like Unicode.

Issue Component Description

ICA-7737 Web Uploader The Institution field is missing from the Web Uploader. As a result, users cannot determine configured institutions.

ICA-13144 Image Sharing The user is unable to upload studies through the Java Uploader. This issue is caused by old MC3 JAR files on the client side Java cache after upgrading.

Workaround: After upgrading, clear the Java cache.

ICA-14089 Viewer In the Study Viewer, some review tools do not behave correctly when using an iPAD with iOS 10 or later, When releasing your touch from the screen during an operation such as pan, zoom, or window/level, the image resets back to the initial state and does not apply the change.

ICA-15516 Viewer When viewing reports of type DICOM SR or HL7, the mouse cursor may get locked on the scroll bar and/or its up/down arrows when clicked. To unlock the mouse cursor from the scrollbar, right-click or move the mouse scroll wheel.

ICA-16764 Image Sharing, PACS Gateway

When PACS Gateway sends very large DICOM data to Enterprise Archive (EA), EA will occasionally produce a C-store failure. By default, iConnect Access expects that C-Store status to be returned from EA in five seconds. However, with very large DICOM data, EA could take longer to process, resulting in a timeout for iConnect Access to get the status returned.

To increase the timeout parameter so iConnect Access can accept very large DICOM data, update the file WebAccess\Config\ MC3StorageSCUConfiguration.xml to change the “associationTimeout” value to “600000”, which is equal to 600 seconds or 10 minutes.

ICA-16778 Image Sharing, LDAP

LDAP users are not getting study upload notifications when a study is uploaded using the HTML5 Web Uploader.

Workaround:

Scenario # 1 — “User Management Mode“ is set to both “Ldap Directory Service“ and “Local Database“.

1. Open file “C:\WebAccess\WebUploader\Config\ServiceFactoryConfiguration.xml”

2. Find text “Python.User” and update the value for Class attribute to “Python.Core.User.Services.MixedUserProcess“:

Database Requirements

New installations will use SQL 2012 or 2014.

Upgrades will be supported on SQL 2008 R2, 2012, and 2014.

NOTE: SQL Server Express is installed by default. Supported versions 2008 R2/2012/2014 have a 10 GB database size limit. To keep the IRWSDB under the maximum size limit, a configurable audit purging job should be run regularly.

Regular job backups are recommended for disaster recovery as well for the HIPAA audit logs. If the audit log retention policy requires centralized querying (more than 3–6 months of audit trails) then a standard SQL license is required for the hosting SQL instance.

iConnect Access Virtual Machine Requirements

The following table describes virtual machine settings and minimum required resources. Depending on workloads and complexity of configuration, additional resources may be required if the system is underperforming.

NOTE: iConnect Access has been validated on a VM environment using VMware vSphere 5.x. and Hyper-V in Windows Server 2008 R2 and 2012 R2. Because iConnect Access is used in patient care, it is important to avoid over-committing the resources allocated to the application.
