Kaiser Permanente Anaheim and Irvine Medical Centers 2014-2016 Community Health Needs Assessment and Implementation Strategy
Kaiser Permanente has been conducting Community Health Needs Assessments (CHNA) for more than 15 years. The Affordable Care Act requires hospitals to conduct a CHNA every three years, beginning in 2013. The CHNA and the resulting implementation strategies guide the investments, programs, partnerships, and assets each hospital will leverage to make measurable improvements in the communities served by Kaiser Permanente. For more information on the Anaheim and Irvine Medical Centers CHNA and strategies, go to: kp.org/CHNA.
