Kaiser Permanente Anaheim and Irvine Medical Centers 2014-2016 Community Health Needs Assessment and Implementation Strategy Kaiser Permanente has been conducting Community Health Needs Assessments (CHNA) for more than 15 years. The Affordable Care Act requires hospitals to conduct a CHNA every three years, beginning in 2013. The CHNA and the resulting implementation strategies guide the investments, programs, partnerships, and assets each hospital will leverage to make measurable improvements in the communities served by Kaiser Permanente. Access to Mental Health Care Provide grants to community organizations to increase access to healthy food and promote physical activity in the community and schools Partner with organizations to provide direct care and guidance for overweight/ obese and diabetic patients at community clinics Support regional Healthy Eating Active Living (HEAL) Zone schools and community organizations to address access to and availability of fresh fruits and vegetables Provide grants and in-kind donations to community clinics and community-based organizations to support access to preventive, primary, and specialty care Fill gaps in community clinic capacity by maintaining community access programs (i.e. Surgery Day and Pathology Reading Program) Support Kaiser Permanente’s Southern California Safety Net Partnership in the provision of funds and in-kind donations to the Coalition of Orange County Community Clinics and other community clinics Provide funds to community organizations dedicated to improving mental health services and supporting families in trauma Provide funds to community-based organizations that provide individual and group cognitive-behavioral therapy Partner with community-based organization to address physician outreach and education Obesity & Overweight Prevention and Management Our Community Findings* Our Focus Our Strategies *Data based on the 2013 CHNA Findings Report.
