PreParing to leave the hosPital
health and social service Centre: a partner to help you prepare for a safe return home
Are you hospitalized and worried about being able to go back home? Most of the time, it’s possible to be discharged home! For now, just take the time to get better.
During your stay, someone from your Health and Social Service Centre’s CLSC will contact you or meet with you and your family, if necessary, to assess your needs and plan for your return home. This person will work together with professionals from the hospital. When your doctor signs your discharge papers, services to ease your return home will have been arranged.

First choice: living at homehere’s what needs to be done before you are discharged:
1. A professional from the CLSC will contact you or meet with you and your family, if necessary.
2. The CLSC professional, together with professionals from thehospital, will assess your needs. You and your family can thendiscuss your options with them.
3. During your stay in hospital, the CLSC professional will makeplans for the services you’ll need at home.
4. While you’re still in hospital, don’t make any quick decisions!
Don’t give up your lease or sell your house right away. Mostof the time, it’s possible to go back home with help from yourfamily and the services you and the CLSC have agreed on.
5. When the doctor signs your discharge papers, the person fromthe CLSC will have made arrangements for the services you’llneed at home or in the place that is best suited to your needs.
