SECTION I. REQUEST FOR PROPOSAL SUMMARY
A. Summary Statement
The City of Bowie, Maryland (Bowie) seeks to offer free public wireless Internet access at select City
facilities. Bowie views wireless “hotspot” services as a way to provide a beneficial service to the citizens
and visitors of the City’s facilities.
The City is soliciting proposals from wireless network integrators to provide a turnkey public WiFi
solution at the following City locations:
• City Hall, 15901 Excalibur Road, Bowie, Maryland 20716
• Senior Center, 14900 Health Center Drive, Bowie, Maryland 20716
• Kenhill Center, 2614 Kenhill Drive, Bowie, Maryland 20715
• Gymnasium, 4100 Northview Drive, Bowie, MD 20716
The awarded scope of services will include equipment supply, configuration, installation, testing, and
ongoing maintenance and support.
The City is seeking responses to this RFP from qualified network integrators, hereafter referred to as
“Respondents,” with experience designing, installing, testing, and supporting wireless LAN solutions.

The City of Bowie, Maryland will only receive proposals in accordance with Section V of this RFP.
C. Pre-Proposal Conference, Questions, Inspection and Inquiries
A mandatory pre-proposal meeting for Respondents will be held on October 25, 2013, beginning at 10:00
AM in City Hall, located at 15901 Excalibur Road, Bowie, Maryland 20716. Respondents will only be
able to inspect the sites during the mandatory pre-proposal meeting. Site surveys will not be
accommodated at any other time prior to the proposal submission deadline.
Before submitting proposals, respondents should carefully examine the Proposed Contract Documents,
acquaint themselves with all governing laws, ordinances, and regulations, and otherwise thoroughly
familiarize themselves with all matters that may affect the performance of the work. The act of submitting
a proposal shall be considered as meaning that the Respondent has so familiarized himself/herself and,
therefore, no concession will be granted by the City because of any claim of misunderstanding or lack of
information. Respondents are expected to read the Scope of Services with special care, and to observe all
specified requirements. Discrepancies, ambiguities, errors, or omissions noted by Respondents should be

reported promptly to the City's Issuing Office for correction or interpretation before the date of the opening
of proposals.
The pre-proposal conference will include a guided survey of the City facilities encompassed by the Scope
of this RFP. During this survey, it is expected that Contractors evaluate sites for the purpose of identifying
any attributes of the physical sites that could impact proposal pricing, pre-requisite site preparations to be
performed by the City, or site conditions that may otherwise cause difficulties with the performance of the
specified scope of work, to include but not limited to:
• ;
• Electrical power requirements of the equipment;
• Requirements for deactivation and removal of Physical space requirements for equipme. nt;
• Antenna and access point mounting locations;
• LAN cabling pathways existing WiFi hardware; and
• Climate control.
Questions and inquiries, both verbal and written, will be accepted from Contractors and must be submitted
to the Issuing Office by November 1, 2013

D. Closing Date
Two (2) original copies of the proposal must be submitted. Proposals must be received in the Information
Technology Director’s Office, Bowie City Hall, 15901 Excalibur Road, Bowie, Maryland 20716, in
envelopes marked “Design and Installation of Public WiFi Proposal”, or by e-mail
([email protected] and [email protected] ) with a subject line containing “Design and
Installation of Public WiFi Proposal,” by 4:00 PM November 8, 2013 in order to be considered.
Contractors mailing proposals should allow sufficient time for delivery to the Issuing Office. Proposals or
unsolicited amendments to proposals arriving after the closing time and date will not be considered. The
City reserves the right to reject any and all proposals or portion(s) thereof.
E. Duration of Proposals
Proposals shall be valid for 90 days following the closing date of this RFP. This period may be extended
by written mutual agreement between the Respondents and the Issuing Office.
F. Qualifications and Experience of Respondents
The Proposal must provide specific information regarding the firm’s experience and capabilities, focusing
on past or present experience with projects similar to the scope of this project, and also include an
overview of the firm’s general business history.
Respondents shall provide a list of at least three customers for which projects of similar size and scope
have been performed within the last three years. A brief description of the project completed with the
company/agency name, physical address, and contact name, telephone number, and e-mail shall be
provided for each reference. The responsibilities and qualifications of any personnel involved in projects

referenced who are also proposed to support the scope of work defined by this RFP shall also be clearly
identified in the Proposal – it is the intent of the City to verify that the specific personnel proposed for this
project are qualified for and experienced with similar projects.
The Proposal must include sufficient information to show how the Respondent will ensure successful
completion of the project, including any subcontracted portions of work.
The Proposal shall identify the technical qualifications of the Respondent and the key staff proposed to
provide WLAN system implementation in accordance with this RFP. Capabilities of the proposed key
staff shall be illustrated with respect to the technologies and skill sets required to complete the scope of
work defined by this RFP.
No changes to the selected Respondent’s proposed project team shall be allowed without prior written
approval from the City.
If a portion of the work would be subcontracted, the Respondent shall clearly identify the responsibilities
of each proposed subcontractor, and the manner in which performance of each party will be assured. The
Provider will be responsible for the entire contract performance, whether or not subcontractors are used.
The Proposal shall discuss proposed quality control measures, project management and review, and
expected City staff involvement during the installation, implementation, and ongoing operational phases of
the project.

The City may make such investigation as it deems necessary to determine the ability of each Respondent to
furnish the product, and the Respondent shall furnish to the City all such information and data for this
purpose that the City may request. The City reserves the right to reject any proposal if the evidence
submitted or investigation of such Respondent fails to demonstrate to the City’s satisfaction that the
Respondent is properly qualified to carry out the obligations of their offer.
G. Required attachments
Each Proposal shall be accompanied by all required attachments, as outlined in Section V of this Request
for Proposal.
H. Acceptance or Rejection of Proposals
The City will accept or reject Proposals within ninety (90) days of the date set for receiving Proposals. The
City reserves the right to reject or accept any or all Proposals or portion thereof where such rejection
would, in the City’s sole discretion, be in the best interests of the City.
I. Notice of Award, Issue of Purchase Order
The successful Respondent will be issued a purchase order binding it to the terms of this Proposal as set
forth in the legal notice inviting proposals, Section II, the Proposal documents, and any addenda thereto,
within seven (7) days of notice of award.

J. Specifications
Respondents must examine the specifications carefully. In case doubt shall arise as to the meaning or
intent of anything shown in the specifications, inquiry shall be made of the City as outlined in Section I
items C and D. The submission of a Proposal shall indicate that the Respondent thoroughly understands
the terms of the Proposal and the specifications.

SECTION II. GENERAL INFORMATION FOR RESPONDENTS
A. Purpose
The overall purpose of this RFP is to provide information to Contractors interested in submitting Proposals
to meet the requirements for equipment and services described herein.
B. Addenda to the RFP
If it becomes necessary to revise any part of this RFP, addenda will be provided to all vendors who
received the initial RFP. Acknowledgment of the receipt of all amendments, addenda and changes issued
shall be required from all vendors receiving the RFP.
C. Cancellation of the RFP
The City may cancel or modify this RFP, in whole or in part, at any time.
D. Proposal Acceptance
The City reserves the right to accept or reject any and all Proposals, in whole or in part, received as a result
of this RFP, or to negotiate with all responsible Respondents, in any manner necessary to serve the best
interests of the City.

E. Incurred Expenses
The City will not be responsible for any costs incurred by any Contractor in preparing and submitting a
Proposal.
F. Multiple Proposals
A Respondent may submit more than one Proposal. Additional Proposals may be prepared following the
same format as the primary Proposal, but containing only that information which differs in any way with
the primary Proposal. Each Proposal must be bound or emailed separately and prepared in accordance
with Section V of this RFP.
G. Public Information Act Notice
Respondents should identify those portions of their Proposals which they deem to be confidential,
proprietary information or trade secrets and provide any justification for why such materials, upon request,
should not be disclosed by the City under the Maryland Public Information Act, State Government Article
10, Sections 10 - 611 ET SEQ of the Annotated Code of Maryland.
H. Contractor Responsibilities
The City will enter into an agreement with the selected Contractor or Contractors only. The selected
Contractor or Contractors shall be responsible for all products required by this RFP.

I. General Contractual Conditions
The purchase order or contract resulting from this RFP shall, at a minimum, incorporate by reference as if
fully set forth therein, all Proposal documents, including but not limited to the agreement, the Legal Notice,
Request for Proposal, Contractor Responsibility Form, Equal Opportunity Employer form, Statement
Under Oath to Accompany Proposal, Specifications, Evaluation and Selection Procedure, General
Information for Respondents, Proposals, Information Required in Proposals, and any and all attachments to
said documents.
J. Disputes
In cases of disputes as to whether or not an item quoted or delivered meets specifications, the decision of
the City shall be final.
K. Failure to Deliver
In the event the selected Contractor fails to deliver the products covered in accordance with the delivery
terms stipulated in the purchase order issued, then the City will have the right to purchase on the open
market the products covered in the Proposal and shall have as damages, any additional costs incurred by
the City as a result thereof.
L. Indemnification
The successful Contractor will be required to indemnify, defend and hold the City harmless against any and
all liability to any person or persons or property for or by any reason of any condition which is the
responsibility of the selected Contractor in connection with the delivery of equipment and software, and

against any and all liability to any person or persons by any act or omission or negligence of the Contractor
or any of its agents, servants, or employees.
M. Conflict of Interest
The successful Contractor will be required to certify that he understands the provisions of the Bowie City
Charter and Code dealing with conflicts of interest in the prohibition, solicitation or acceptance of gifts.
N. Doing Business in Maryland
The successful Contractor shall warrant and represent that it: (1) is either (a) incorporated in Maryland or
(b) registered or qualified by the Maryland State Department of Assessments and Taxation (SDAT) as
required by the Maryland Annotated Code, Corps. & Assocs. Article, to do business in Maryland; and (2)
is in good standing with SDAT.
O. Insurance
The selected Contractor shall maintain such commercial general liability and broad form property damage
insurance and Workers' Compensation Insurance as will protect the City of Bowie from any and all
Workers' Compensation claims and from any other claims for loss or damages or for general injury or
damage to property which may arise from Contractor's operations under the contract, whether such
operations be by itself or by any subcontractor or anyone directly or indirectly employed by either of them.
Certificates of Insurance shall be on an occurrences basis. The Contractor shall name the City as additional
insured to the required insurance policy and will furnish a Certificate of Insurance or other acceptable
evidence of insurance coverage to the City upon signing the contract.

a. Comprehensive Liability Insurance: Limits of Coverage (Applies ONLY to Contractors performing
services in and/or for the City; NOT for supply only Contracts ) Limits of coverage for commercial general
liability and broad form property damages coverage are to be no less than One Million Dollars
($1,000,000.00) occurrence/One Million Dollars ($1,000,000.00) aggregate personal injury and death and
Two Hundred Fifty Thousand Dollars ($250,000.00) property damage/Five Hundred Thousand Dollars
($500,000.00) aggregate, where insurance aggregates apply. Property damage insurance shall specifically
include explosion, collapse, and underground damage (X, C, U).
b. Automobile Liability Insurance. Motor vehicle insurance meeting the requirements of Maryland law
and covering every vehicle and driver involved in providing the services, in the following amounts:
(1) Bodily injury liability with limits of $500,000.00 each person and $1,000,000.00 each accident;
(2) Property damage liability with a limit of $100,000 each accident.
c. Workers' Compensation: Failure of the Contractor to maintain Worker's Compensation coverage for
the duration of the contract will result in the City deducting from each payment made under this contract,
to the Contractor, a pre-determined percentage to defray coverage costs of the City. Contractor shall
comply with the requirements and benefits established by the State of Maryland for the provision of
Workers' Compensation.
P. Testing and Inspection
The City has the right to inspect and test all services and materials called for by the contract, to the extent
practicable at all times and places during the term of the contract. The City shall perform inspections and
tests in a manner that will not unduly delay the work. If any of the services and/or materials does not
conform to contract specifications, the City may require the Contractor to perform the service again or
provide a replacement product in conformity with contract specifications, at no increase in contract
amount.

SECTION III. SCOPE OF WORK
A. Summary
The selected Contractor shall procure, install, configure, test, document, and provide ongoing maintenance
and support of wireless LAN (WLAN) systems at the following city facilities for the sole purpose of
providing public WiFi Internet access:
• City Hall
• Senior Center
• Kenhill Center
• Gymnasium
The WLAN equipment shall provide public access to the Internet. The configuration shall be completely
separate from the City’s internal network at each location – the scope consists of four standalone WLAN
networks, each with their own dedicated Internet connections. The City will provide rack space for
equipment, electrical power and separate connection space for the selected Contractor to provide Internet
service at each location. All other services, labor, materials, and equipment required to meet the specified
functional objectives of this RFP shall be the responsibility of the selected Contractor.
Limited WLAN equipment used to support public access exists within certain City facilities, which shall be
replaced by the systems provided under this RFP. The selected Contractor shall be responsible for
identifying and removing these existing systems, and providing these systems in their current working
order to the City.

Any additional responsibilities expected of the City of Bowie must be clearly identified within the
Respondent’s Proposal.
B. Post-Award Site Survey and Implementation Plan
Upon award of contract, the awarded contractor shall perform a thorough survey of all sites. All sites shall
be evaluated for the purpose of identifying any attributes of the physical site preparation that may result in
network service interruptions or cause difficulties with the installation, including but not limited to:
• Physical space requirements for equipment;
• Antenna and access point mounting locations;
• LAN cabling pathways;
• Electrical power requirements of the equipment; and
• Climate control.
Any necessary site preparations shall be identified and reported to the City.
Additionally, the selected Contractor shall prepare a detailed implementation plan for review and approval
by the City, identifying the key steps and associated timeline for the installation and testing of the WLAN
systems.
C. Technical Solution
The Proposal shall include an explanation of the technical solution proposed to meet the requirements of
the RFP. The explanation shall address all technical requirements of the RFP, providing a list of the
hardware and software components, including manufacturer and model information. For each equipment

component, manufacturer information shall be provided to illustrate compliance with the technical
equipment specifications defined in this Section.
Descriptions of applicable hardware and software configurations shall be provided, in addition to network
diagrams, to illustrate the architecture of the proposed solution.
The technical portion of the Proposal shall provide network schematics and technical documentation
outlining the proposed wireless hotspot architecture.
The technical proposal submitted shall, at a minimum, address the following technical issues: (All
Proposals are to list each of the following issues, and then the response):
1. List the quantity, make and model of equipment proposed, and discuss what equipment would be
located at each City facility.
2. Provide the technical specifications for the proposed equipment, including physical dimensions
and power requirements.
3. Discuss any network configurations proposed.
4. Discuss how the Respondent plans to test and prove the specifications listed in the technical
proposal upon system implementation.
5. Demonstrate that the proposed system is scalable and to what degree (and at what cost) additional
WiFi access points could be added in the future.

6. Describe network management system capabilities, configurations, and how these will be used to
provide reliable service and support.
D. Timeline
The Proposal shall include a timeline defining key milestones, resources, and estimated time requirements.
The awarded contractor shall be expected to adhere to the proposed timeline, which shall be stipulated as
terms within an awarded contract. The timeline shall indicate project completion within three months from
contract award. The actual project timeline will be developed by Bowie, and may occur in a phased
implementation.
E. Price Quotation
A price quotation shall be provided using the attached Proposal Pricing Submission sheet, encompassing
any costs to be borne by the City of Bowie. The price quotation should distinguish between any non-
recurring and recurring costs, if applicable.
The price quotation shall be itemized on a per site basis.
The City shall reserve the right to purchase or reject any itemized portion of the Proposal at quoted prices,
and shall reserve the right to modify quantities of itemized equipment and services after the award of a
contract. Itemized prices for equipment and services shall be valid for twelve months.

F. Description of Work Required within City Facilities
The following section describes the existing City facilities and required scope of work at each location
where wireless LAN equipment is requested.
City Hall
City Hall is located at 15901 Excalibur Road and is the center of the City’s data network. City Hall is a
newly constructed 80,000 square-foot, three-story building that houses many of the City’s departments.
The contractor shall be required to provide a turnkey wireless solution for the entire building, which
includes, but is not limited to:
• Performing a detailed WiFi design and determining mounting locations for wireless access points;
• Purchasing of all network electronics necessary to operate a public WiFi network, including
power over Ethernet switches and central wireless LAN management system;
• Installing new network cables from the Intermediate Distribution Frame (IDF) on each floor of
City Hall to the mounting location of every access point;
• Mounting of wireless access points at each location;
• Installing and configuring of all network electronics;
• Testing of the WiFi system; and
• Documenting the as-built public WiFi system.
The contractor shall perform a wireless LAN site survey within City Hall to identify the locations where
the WiFi access points are to be installed. For the purpose of this Proposal, the contractor is to assume that
the selected contractor shall supply and install fifteen (15) access points within City Hall. The public WiFi
system shall be completely independent of the City’s network at City Hall.

Senior Center
The Senior Center is located at 14900 Health Center Drive. The Senior Center is a 29,000 square foot,
single story building. The Senior Center is a multi-purpose building providing a wide range of services
and activities for seniors.
The contractor shall be required to provide a turnkey wireless solution for the entire building, which
includes, but is not limited to:
• Performing a detailed WiFi design and determining mounting locations for wireless access points;
• Purchasing of all network electronics necessary to operate a public WiFi network, including
power over Ethernet switches and wireless LAN management system;
• Installing new network cables from the Main Distribution Frame (MDF) to the mounting location
of every access point;
• Mounting of wireless access points at each location;
• Installing and configuring of all network electronics;
• Testing of the WiFi system; and
• Documenting the as-built public WiFi system.
The contractor shall perform a wireless LAN site survey within Senior Center to identify the locations
where the WiFi access points are to be installed. For the purpose of this Proposal, the contractor is to
assume that the selected contractor shall supply and install six (6) access points within the Senior Center.
The public WiFi system shall be completely independent of the City’s network at the Senior Center.

Kenhill Center
The Kenhill Center is located at 2614 Kenhill Drive. The Kenhill Center is a 39,000 square foot, single
story building. The Kenhill Center is a multi-purpose building providing a wide range of services and
activities. Comcast and the Prince George’s County I-Net have hubs in this building. It is also the City’s
main network distribution center and contains backup servers and a backup City EOC.
The contractor shall be required to provide a turnkey wireless solution for the entire building, which
includes, but is not limited to:
• Performing a detailed WiFi design and determining mounting locations for wireless access points;
• Purchasing of all network electronics necessary to operate a public WiFi network, including
power over Ethernet switches and wireless LAN management system;
• Installing new network cables from the Main Distribution Frame (MDF) to the mounting location
of every access point;
• Mounting of wireless access points at each location;
• Installing and configuring of all network electronics;
• Testing of the WiFi system; and
• Documenting the as-built public WiFi system.
The contractor shall perform a wireless LAN site survey within the Kenhill Center to identify the locations
where the WiFi access points are to be installed. For the purpose of this Proposal, the contractor is to
assume that the selected contractor shall supply and install seven (7) access points within the Kenhill
Center. The public WiFi system shall be completely independent of the City’s network at the Kenhill
Center.

The Gymnasium is located at 4100 Health Center Drive. The Gymnasium is a single story building
providing space for community residents to participate in athletic programs. This facility has enough space
for two professional basketball courts or eight volleyball courts. Meeting rooms and locker rooms are
available. The contractor shall be required to provide a turnkey wireless solution for the entire building,
which includes, but is not limited to:
• Performing a detailed WiFi design and determining mounting locations for wireless access points;
• Purchasing of all network electronics necessary to operate a public WiFi network, including
power over Ethernet switches and wireless LAN management system;
• Installing new network cables from the Main Distribution Frame (MDF) to the mounting location
of every access point;
• Mounting of wireless access points at each location;
• Installing and configuring of all network electronics;
• Testing of the WiFi system; and
• Documenting the as-built public WiFi system.
The contractor shall perform a wireless LAN site survey within Gymnasium to identify the locations where
the WiFi access points are to be installed. For the purpose of this Proposal, the contractor is to assume that
the selected contractor shall supply and install three (3) access points within the Gymnasium. The public
WiFi system shall be completely independent of the City’s network

G. System Technical Requirements
This section outlines the functional technical requirements of the WiFi system. The wireless access points
shall support the IEEE 802.11n protocol and support at least two spatial streams. The wireless system shall
be deployed in a configuration to support legacy 802.11a/b/g clients. The wireless access points shall have
a RJ-45 Gigabit Ethernet port that supports IEEE 802.3af power over Ethernet.
The Ethernet switches supporting the public WiFi system shall be equipped with Gigabit Ethernet ports
and shall support IEEE 802.3af power over Ethernet. The switches shall be equipped with multimode fiber
optic uplink ports to connect between the MDFs and the IDF at City Hall, in which an aggregation switch
shall be required.
The WiFi systems at each site shall be controlled from a central management device and gateway (WLAN
management system). The WLAN management system shall control and monitor the wireless access
points. The system shall be capable of automatically adjusting the RF levels and channel selection of the
wireless access points to minimize interference between access points, and to compensate if an access point
is not functioning. The central management system shall also be capable of alerting the selected
Contractor’s monitoring staff via e-mail and/or other electronic means when there is an outage on the
public WiFi system. The central management system shall be able to track the use of the public WiFi
system, and shall be able to block access to public WiFi at specific times of day, or as seen necessary by
the City.
The wireless system shall prompt users to accept the City’s terms and conditions prior to using the WiFi
network.

All work shall be performed according to applicable codes and standards, and shall reflect a high level of
workmanship. All cabling and connections installed by the Contractor shall be labeled clearly, and best-
practices for cable management shall be followed.
H. System Acceptance Testing
The City of Bowie’s staff, or its authorized representatives, reserves the right to inspect the proposed WiFi
system upon completion of implementation to ensure that the network meets the technical specifications
stated in this Proposal.
Testing shall include, but not be limited to, successful login as a public user and obtaining access to the
Internet, and WiFi coverage testing. The Contractor shall check area-wide coverage, documenting absolute
RF field strength and signal-to-noise ratio (SNR) throughout each facility using a City-approved test
procedure and apparatus. The network must provide 100% coverage within all facilities, providing a client
receive signal strength of -85 dBm at 2.4GHz (802.11b/g) and -83 dBm at 5 GHz (802.11a) for a client
device having an antenna gain of no more than 1 dBi.
I. Training and Documentation
The selected Contractor shall provide one day of on-site training on the wireless system as part of the
integration services. The training shall provide an overview of the administrative functions and
configuration of the system. The selected Contractor shall provide manuals and other materials for future
reference.

The Contractor shall supply final as-built drawings and materials, consisting of detailed, interface-level
schematics, a matrix containing cable and patch panels/ports identification, and device configurations.
J. Ongoing Maintenance and Support Requirements
The Contractor shall provide ongoing maintenance and technical support for the installed WLAN systems.
This shall include ongoing, real-time status monitoring of the WLAN equipment, as well as remote or
onsite technical support, as required. System monitoring shall encompass hardware failures detected by the
WLAN management system, including for all managed wireless access points, as well as basic operational
status of the Internet connection (i.e. via periodic ping tests to, or heartbeat signals from, the WLAN
management system).
The Contractor shall immediately alert City staff in the event of a detected outage, and shall begin
diagnostics within two hours. The Contractor shall provide manufacturer-authorized maintenance
contracts providing access to all related software and firmware updates, and advance equipment
replacement on an 8x5, next business day basis.
Proposals shall describe the Respondent’s remote monitoring capabilities and systems, and shall specify the
Respondents proposed approach to technical support, defining specific timeframes for escalation to varying
tiers of technical support expertise.
Proposals must specify pricing for ongoing support on a per site basis for a period of one year.

SECTION IV. EVALUATION AND SELECTION PROCEDURE
A. Qualifying Proposals
Each proposal will be reviewed for compliance with the mandatory feature requirements in Section III
specifications and with all other necessary requirements of this procurement. Failure to comply with any
mandatory requirements will normally disqualify a Respondent's proposal. The City retains the right to
waive a mandatory requirement when it is in its best interest to do so.
Any alternate solution proposal submitted must be complete and will be subject to a full and independent
evaluation. In addition, the Respondent must assume full responsibility for addressing all necessary
technical and operational issues required to meet the objectives of the RFP.
B. Selection
A contract may be awarded to provide a turnkey Wireless hot spot solution, in accordance with this RFP,
based upon an evaluation of Proposals according the following criteria:
Responsiveness to Proposal requirements
Qualifications of the integrator
Completeness of the technical portion of the Proposal
Price for all components to meet the project requirements
The City of Bowie reserves the right to reject any and all proposals and to accept the proposal that the City,
in its' sole and absolute discretion, considers most advantageous. All proposals will become the property of
the City of Bowie.

C. Award
The contract award will be made to the most qualified Respondent who, in the City's sole opinion, has
presented the proposal most advantageous to the City.

SECTION V. INFORMATION REQUIRED IN PROPOSALS
A. Transmittal Letter
A transmittal letter prepared on the Respondent's business stationery should accompany the proposal. The
purpose of this letter is to transmit the proposal; therefore, it should be brief. An individual who is
authorized to bind the firm to all statements, including services and prices, contained in the proposal, must
sign the letter.
B. Technical Proposal
The Proposal will consist of and must contain the items specified in Section III. The proposal should
contain all pricing information for all products proposed including transportation cost per location. Best
and final prices should be offered.
C. Anti-Bribery Affidavit
The City requires that Respondents submit an affidavit stating whether the entity or any of its officers,
directors, or partners, or any of its employees directly involved in obtaining contracts with the City have
been convicted of bribery, attempted bribery, or conspiracy to bribe under the laws of any state or the
federal government. The Respondent's affidavit must be completed by all respondents to the RFP and
returned with their respective responses. The form that the Respondent is to complete is included in the
appendices of this RFP.

D. Non-Collusion Certificate
Notarized affidavit (non-collusion oath) executed by the Respondent, or if Respondent is a corporation,
executed by a duly authorized representative of the corporation.
E. Respondent Responsibility Form
The Respondent must submit a completed Respondent Responsibility Form with the proposal.
F. Equal Opportunity Employer Form
The Respondent must submit a completed Equal Opportunity Form with the proposal.
G. Submission Deadline
The Respondent must submit the required number of copies of his proposal by the closing time and date
specified in Section I of this RFP.
