Ross Payant is a 38 bed long term care facility located in Assiniboia. 12 of our beds are on a secured special needs unit.

Aim, purpose, scope, boundaries, objectives and goals.

Reduce the number of falls in our facility by 20% by march 2012 . Implement the FHHR falls prevention policy for all staff and residents.

Changes Tested

Appropriate footwear

Check the special needs unit and discover that residents (out of 12) where wearing the appropriate footwear

Nursing staff reading the FHHR policy

Initial findings- 42% staff read policy.

Took actions by using calendars and talking to staff in person to read the policy, these actions led up to the 92% of all staff knowing/reading the FHHR policy.

We piloted a q-30min check of the residents on special needs wing. Duration was June 24th-July 24th, we discovered this was the highest month of falls. Staff did not like the added documentation. We will re-look at this project.

Lessons Learned

It is difficult to work as a team when all members work rotating shift work.

The more that staff are involved and educated, the more progress we make.

We need to have a look at the results of what we have already done to determine what we do next.

We appreciate being a part of this group.

We are going to continue to follow the FHHR fall prevention policy.
