Social Work

Wellness Circles offer individual care, group education, and peer support to patients experiencing a similar diagnosis. SouthEast Lancaster Health Services offers multilingual Wellness Circle classes for expectant parents, new parents and their babies, diabetes management, asthma management, and more!

Our program is flexible and individually tailored to your needs.

Wellness Circles

SouthEast Lancaster Health Services charges fees for the services we provide. There are numerous payment methods that assure services are affordable. We care for patients with no insurance, commercial insurance, Medical Assistance/Medicaid* or Medicare.

*Patients qualify for Medical Assistance and Marketplace Insurance if they meet income, resource, and other eligibility requirements. For assistance with enrollment in Marketplace Insurance, Medicaid, and CHIP, please call our Outreach and Enrollment department at 717-917-3756.

Sliding Fee ProgramSouthEast Lancaster Health Services provides services to all, regardless of one’s ability to pay. Our Sliding Fee Program offers medical and dental services based on household income and size. A patient may be eligible for this program if they do not have insurance or are underinsured.

To qualify for this program, patients meet with our Patient Benefit Coordinator and offer proof of identification and household income. Please call us at 717-299-6371 to schedule an appointment with a Patient Benefit Coordinator or if you have any questions.

How we connectThe Patient Connection: Patients can now connect with SouthEast Lancaster Health Services through our Patient Portal. These on-line tools allow patients to request an appointment, access information, and connect with our center at any time.
