RT Report of the Forty-Second Annual Session of the CSHP House of Delegates

The forty-second annual session of the California Society of Health-System Pharmacists House of Delegates was held in the Concord A room of the Paris Hotel

in Las Vegas, Nevada, in conjunction with Seminar 2017, the Society’s annual meeting.

The CSHP House of Delegates (HOD) Chair Jeannette Hanni convened the meeting at 10:12 a.m. on Sunday, October 29, 2017.

Roll Call and QuorumFollowing the enrollment of delegates, Secretary Loriann De Martini reported that a quorum was present. Included are CSHP officers, directors, past presidents, and delegates representing CSHP-affiliate chapters, student delegates from each of the California schools of pharmacy, pharmacy technician members, and the California Pharmacists Association.

IntroductionsHOD Chair Hanni introduced CSHP CEO and Secretary of the House of Delegates Loriann De Martini, President Vicky Ferraresi, Chair of the Board of Directors Kethen So, President-elect Daniel Dong, Treasurer Pauline Chan, and Maria Serpa who served as the parliamentarian for the 2017 HOD.

Role and Function of the House of Delegates HOD Chair Hanni described the purposes and functions of the HOD and emphasized its responsibility for establishing policy. The number of delegates eligible to vote this year was 138.

Approval of Minutes from the 2016 CSHP House of DelegatesHOD Chair Hanni called for a motion to approve the minutes of the forty-first annual session of the CSHP HOD as published on the CSHP website. The minutes were approved by voice vote.

Report of the Blue Ribbon Committee on GovernanceDr. William Yee recognized members of the CSHP Blue Ribbon Committee on Gover-nance. Dr. Yee reported on the Blue-Ribbon Committee’s charge to review of CSHP’s administrative policies and provided a brief overview of the report.

Dr. Yee explained the difference between CSHP professional and administrative policy. This report was presented for the information of delegates and did not require action by the HOD.

CSHP Annual ReportHOD Chair Hanni reported that the CSHP Annual Report was presented at the CSHP business meeting held on Friday, October 27, in the Versailles Ballroom at the Paris Hotel in conjunction with CSHP Seminar 2017. The business meeting included reports from CSHP President Vicky Ferraresi, CSHP Treasurer Pauline Chan, and CSHP CEO Loriann De Martini. These reports will be available on the CSHP website as well as a special online issue of CJHP.

Report of the Committee on NominationsCSHP Committee on Nominations Chair Anne Tran-Pugh briefly explained the nomination process, then thanked Brian Kawahara for putting together the members for the 2017 Nomi-nation Committee. Dr. Tran-Pugh introduced and acknowledged the members of the 2016-2017 Committee on Nominations:

Dr. Tran-Pugh invited each of the nominees to stand and be recognized as she called their names and thanked them for their willingness to serve. Dr. Tran-Pugh moved for acceptance of the slate of candidates for the 2018 CSHP officers and board of directors. HOD Chair Hanni called for a vote and the slate was approved by voice vote.

HOD Chair Hanni thanked Dr. Tran-Pugh and the CSHP Committee on Nominations for their work in presenting a slate of candidates.

Action on Proposals and ResolutionsCSHP Board of Directors Chair Kethen So introduced 24 proposals and five resolutions on behalf of the CSHP Board for consideration by the HOD.

HOD Chair Hanni proposed and asked the delegates to accept a process for accepting proposals and resolutions in mass, rather than discussing each individually. She then asked delegates which proposals would need further discussion and be excepted out. This excepting out process was approved by voice vote.

A.1: Pharmacy Technician Training and Certification To replace CSHP policies 2012-09, 2013-10, and 2013-31 with ASHP Policy 1537 as CSHP Professional Policy 2017-01: Statement on the Roles of Pharmacy Technicians

Statement on the Roles of Pharmacy Technicians

The California Society of Health-System Pharmacists:

1. recognizes that pharmacy technicians have many different roles in a variety of health care settings, with varying levels of responsibility and complexity, from those suitable for techni-cians just entering the workforce to advanced or specialized roles that require additional education, training, experience, competence and compensation.

2. recognizes there is a need for specialized certification of pharmacy technicians who perform activities involving higher than normal degrees of complexity and risk. Any pharmacy technician in an area of pharmacy practice designated for specialty or advanced certification must have the appropriate certifications successfully completed prior to practicing in that specialty area.

3. recognizes that differing roles for pharmacy technicians will require advanced education and training beyond that of an entry-level technician. Education and training requirements must reflect the responsibilities that technicians will have in their daily activities. Ongoing competence assessment must be a component of advanced or specialized technician roles.

4. supports that pharmacy technicians, from entry-level to specialty-trained technicians, complete a pharmacy tech-nician training program accredited by ASHP and the Accreditation Council for Pharmacy Education (ACPE) through the Pharmacy Technician Accreditation Commis-sion (PTAC) as a requirement for earning PTCB certification or equivalent certification. A process should be developed to allow for grandfathering of qualified technicians.

5. supports that pharmacy technicians have either PTCB or equivalent certification as a prerequisite to licensure by the California Board of Pharmacy and maintain their certification through mandated continuing education and recertification.

3. encourages pharmacy practitioners to obtain the skills necessary to pursue administrative, managerial, and leadership roles.

4. encourages colleges of pharmacy and CSHP chapters to collaborate in fostering student leadership skills through development of co-curricular leadership opportunities, leadership conferences, and other leadership promotion programs.

5. support that residency programs should develop leader-ship skills through mentoring, training, and leadership opportunities.

6. fosters leadership skills for pharmacists to use on a daily basis in their roles as leaders in patient care.

2. acknowledges that the North American Pharmacist Licensure Examination (NAPLEX), combined with a California Practice Standard and Jurisprudence Examination (CPJE), is appro-priate to test for basic competencies essential for entry-level pharmacist practice if the examinations include, but are not limited to, testing for pharmacy principles and standards in:

C.1: Promotion of Off-Label Uses

To amend CSHP Professional Policy 2014-51 and adopt as CSHP Professional Policy 2017-08: Off-Label Use of Medication

The California Society of Health-System Pharmacists:

1. supports the practice of prescribing medications for off-label uses that are documented in the medical literature in a system that:

a. maintains patient access to pharmacist review of all medications.

b. protects the pharmacist’s right of refusal of an off-label use of a medication.

c. preserves the patient-pharmacist-prescriber relationship. d. provides adequate patient counseling and education,

particularly to patients taking medications for off-label use. e. recognizes the prescriber’s responsibility in assuring the

appropriate and safe use of all medications and f. encourages evidence-based decision making and

prescribing.

2. opposes efforts to restrict the off-label use of medication when the usage is medically appropriate, evidence-based and in the patient’s best interest

3. supports the authority of the Food and Drug Administra-tion (FDA) to regulate the promotion and dissemination of information about off-label uses of medications and medication-containing devices by manufacturers and their representatives.

4. supports that such off-label promotion and marketing be limited to the FDA-regulated dissemination of unbiased, truthful, and scientifically accurate information based on peer-reviewed literature not included in the New Drug Approval process.

C.2: Home IV Therapy

To amend ASHP Policy 1623 and adopt the following as CSHP Professional Policy 2017-09: Home Intravenous Therapy

The California Society of Health-System Pharmacists:

1. supports the continuation of a home intravenous therapy benefit under federal, state, and private health insurance plans.

2. supports expansion of the home infusion benefit under Medicare and Medi-Cal and other health plans at an appropriate level of reimbursement for pharmacists’ patient care services including patient and caregiver education, medications, supplies, and equipment to provide optimal therapy for patients.

3. encourages health-systems and providers to ensure that transitions of care into the homecare environment includes information and sufficient time for the home infusion pharmacy to provide the following in an optimal and timely manner:

a. coordination with other healthcare providers b. intravenous therapy c. medicationsd. ancillary supplies and equipment’se. education

C.3: Tobacco Products and Electronic Nicotine Devices

To amend CSHP Policy 2013-13 and adopt as CSHP Policy no 2017-10: Efforts to Discourage the Use of Tobacco Products

3. promotes the role of pharmacists in tobacco cessation counseling and smoking cessations medication therapy management.

5. recognizes that pharmacists, as health care providers, should be actively involved in reducing the negative effects that substance abuse has on society, health-systems, and the phar-macy profession.

6. recognizes that pharmacists play an active role in harm reduc-tion by furnishing opioid antagonist products.

During the discussion of Resolution A.2: Pharmacist Majority on the California State Board of Pharmacy, a two-minute break was taken to allow the parliamentarian and presidential officers to verify Article V of CSHP Bylaws regarding the allowance of a non-delegate the privilege of the floor. Upon clarification of the CSHP Bylaws, a recount indicated 106 delegates in atten-dance and verified a quorum. A motion was approved to allow a non-delegate member, Daniel Kudryashov, to address the CSHP House of Delegates regarding Resolution A.2.

Installation of PresidentOutgoing CSHP President Vicky Ferraresi installed Daniel Dong as CSHP’s new president and presented Dr. Dong with an inscribed CSHP president’s pin. He challenged every person in attendance to recruit one new member within the next year.

Recognition of Outgoing President & installation of the New Chair of the BoardNewly-installed President Daniel Dong called the immediate Past President Vicky Ferraresi to the podium. President Dong thanked Dr. Ferraresi for her leadership for the past year as president as well as her many years of service on the Board of Directors and to CSHP. Dr. Ferraresi was presented with a plaque on behalf of the membership and the Board of Directors to commemorate her year as president of CSHP. Dr. Ferraresi addressed the House and shared that serving as CSHP presi-dent was the greatest honor of her life. She thanked those who offered support during her tenure and vowed support to incoming President Dan Dong.

Recognition of Outgoing Chair of the Board of DirectorsImmediate Past President Vicky Ferraresi called outgoing Chair of the Board Kethen So to the podium and thanked him for leadership on the Board and contributions to the society. Dr. Ferraresi presented Dr. So, with an inscribed plaque to commem-orate his years as a Board member and as the chair of CSHP’s Board of Director.

Dr. So addressed the gathering and said the past three years on the board were challenging, but also the most rewarding and fun three years. Dr. So added that the Society’s membership has risen and is in a strong financial position. He thanked Loriann De Martini for the tremendous change she has brought to the Society within a year of her appointment as the CEO. Dr. So also thanked the CSHP staff for their hard work and contribution to bring CSHP back on track. He also thanked the CSHP member-ship for their trust and support of him over the last three years. Lastly, he also thanked the presidential officers and the board members past and present for their support and friendship.

Recognition of CSHP ParliamentarianHOD Chair Jeannette Hanni recognized Dr. Maria Serpa and thanked her for serving as parliamentarian during the proceed-ings of the 2017 House of Delegates.

AnnouncementsHOD Chair Jeannette Hanni reminded delegates that Seminar 2018 would be held in San Diego, California, on October 4-7, which is earlier than normal. Delegates were informed that the date for the 2017 Regional Delegates Conference will be earlier this year in August, will be decided by the CSHP Board of Direc-tors at its January meeting, and a ‘save the date’ notification will follow from the CSHP office.

Thanks to the tireless efforts of Dr. De Martini, Chairman of the Board Kethen So, the Board of Directors, and CSHP volunteers and staff, I can wholeheartedly assure you that CSHP is in good hands and headed in the right direction. You have heard that member-ship is increasing, and Treasurer Pauline Chan will report on our improving financial status. We have lots of plans for new services and programs for our members in 2018. Stay tuned as we roll them out. But now let’s take a look back at what CSHP has accom-plished during my presidential year.

We re-established the CSHP Sterile Compounding Committee. This group, ably led by Co-Chairs Maria Serpa and Doug O’Brien, is working to provide updated tools and resources for our members. They will report on their work to date at the Sterile Compounding Town Hall later today.

The Blue Ribbon Committee on Governance continued their work in 2017. This year they were charged with reviewing and reorganizing the CSHP Board Administrative Policy Catalog. Administrative policy guides the Society in providing activities and services for members. It is an essential component of CSHP operations. The committee has completed their charges and has done a fantastic job. Chairman Bill Yee will present their report at the House of Delegates on Sunday. Their work will no doubt improve how CSHP provides services for you, our valued members.

In August, the CSHP Board of Directors updated the strategic plan to identify five key initiatives to further our strategic priorities. They are:

• To promote the appropriate use of opioids via opioid stewardship

• To advance the professionalism of pharmacy technicians

• To provide leadership development for pharmacy professionals

• To promote implementation of pharmacist provider status

• To advance adherence to sterile compounding requirements

CSHP President’s Report

We will focus on these in the coming twelve to eighteen months. To that end, President-Elect Dan Dong has appointed the CSHP Opioid Stewardship Task Force. Over fifty members volunteered to serve. I look forward to their work in this critical area, providing guidance, strate-gies, and tools to assist you in managing this dire problem.

Legislatively, CSHP was the sponsor of SB716, authored by Senator Ed Hernandez, proposing to add a phar-macy technician to the California State Board of Pharmacy. We saw this as a step toward advancing the professionalism of pharmacy technicians and promoting the implementation of pharmacist provider status. Following an amend-ment to also add another public member and some members’ concern about the decreased pharmacist majority on the Board of Pharmacy, CSHP listened to our members. We held an affiliate chapter presidents’ phone call, had discussions with the affiliate chapters, and conducted a telephonic town hall. Subsequently, we asked Senator Hernandez to place a pause on the bill.

Next week, at the request of CSHP, Senator Hernandez is convening a meeting of stakeholders to discuss options

for the future of the bill. Stay tuned for developments. You are invited and strongly encouraged to participate in a discussion about SB 716 and other CSHP issues at the Seminar Town Hall, Open Forum, and at the end of this session.

All of our other CSHP Committees, Councils and Task Forces have been hard at work on the following:

• Monitoring legislation

• Operationalizing our strategic plan

• Identifying leaders

• Increasing membership

• Planning Seminar 2017 and 2018 and the CE that goes along with it

• Publishing our journal

• Keeping us updated on current health-care issues

• Serving our students, technicians, and new practitioners

• Recognizing achievements

As directed by last year’s House of Delegates, the CSHP Board of Direc-tors established a Code of Conduct Committee. Adding to The Event Code of Conduct created in 2016 is an incident

report form and a procedure for enforce-ment. We hope that these will never need to be used, but they will provide clear guidance and direction for action in case you need them.

To introduce candidates for CSHP office by more than an election bulletin, we are introducing a “Meet the Candidates” session at the end of the Open Forum. Candidates will be part of a panel and will answer questions on pharmacy practice and more. Also, be sure to check out the candidate videos on the CSHP website.

Not everything we accomplished in 2017 was innovative or glitzy and exciting. But it was important, strategic and sorely needed to restore your confidence.

It has been an extreme honor to be CSHP President. On Sunday I can turn the reigns over to my long-time friend and colleague Dan Dong knowing that the organization is in an improved position. Thank you for showing your faith and confidence in allowing me to have played a part in that. o

This marks my first year as Treasurer of the California Society of Health System Pharmacist (CSHP). This report covers the fiscal year ending June 30, 2017.

Financial Highlights1. CSHP assets at year end totaled $ 2,766,620.

2. Net membership dues at year end totaled $362,727 which is lower than last year. We believe this is due to changes in leadership and in the membership renewal cycle.

3. CSHP contingency reserve funds at year end totaled $250,778.

4. Seminar 2016 netted in higher than expected revenue, to a total of $1,227,139, and with a net profit of $907,212.

5. With the departure of the executive vice president and the director of operations in 2015, CSHP incurred unanticipated legal and consulting expenses. Some of these expenses were incurred in year ending June 30, 2017.

6. CSHP continues a series of proactive cost cutting measures, with reduced travel and not filling personnel vacancies.

Independent Auditor’s ReportA financial audit was conducted by Fechter & Company, a CPA firm located at 1870 Avondale Avenue Suite 4, in Sacramento, California. The auditor’s report ending June 30, 2017, was presented at the CSHP Committee on Finance Meeting on September 26, 2017, and presented to the full CSHP Board of Directors by conference call on September 27, 2017.

Statement of Financial Position for Fiscal Year Ending June 30, 2017The Statement of Financial Position as of June 30, 2017 showed that CSHP’s total assets were $2,766,620. Our cash and cash equivalents were $302,998. Our total liabilities including accounts payable, accrued expenses, long-term debt and deferred revenue were $2,515,842.

Table 1 provides a schedule of our assets as reported in the prior three fiscal years.

Statement of Activities for Fiscal Year Ending June 30, 2017The CSHP Statement of Activities provides our revenue and other support and expenses. CSHP reported $1,907,009 in total revenue and $1,962,869 in total expenses. The net asset for the year decreased to $250,778.

We received net revenue of $362,727 from total membership dues, $1,227,139 from Seminar 2016 registration and exhibits fees, $61,743 from Founda-tion contract revenue, $60,781 in other income, $163,405 in rental income, $31,211 in journal advertising, and $3 in investment income.

Table 2 provides a schedule of our revenue/income as reported in the previous three years.

Statement of Cash Flows for Fiscal Year Ending June 30, 2017The CSHP Statement of Cash Flow show operating activities was $149,020 and investing activities was $(210). Net cash provided by financing activities was $(28,789). Cash and cash equivalent at the beginning of the Year was $182,977 and at the end of the year was $302,998.

Reserve FundsReserve funds for year ending June 30, 2017 (also known as total net assets or equity), was $250,778. This compares to 2016 reserves of $317,894. Our reserves are equivalent to about 12.7% of current annual expenses.

Table 3 provides a schedule of our net assets compared to the prior three fiscal years.

Total revenue and other support was recorded at $1,907,009, with expenses for the year ending June 30, 2017, recorded at $1,962,869. This represents a decrease in net assets of $(55,860), also negatively impacting the reserves. Part of the decrease in net assets is due to building depreciation.

Current Ratios and Other StatisticsThe ten-year trending of current ratios, which is a ratio of current assets divided by current liabilities, shows a significant drop in the year ending June 30, 2009. This is due to the use of funds to purchase the office building, now recorded as prop-erty and equipment asset. Current ratios reached the lowest in 2015 and 2016, due to the legal expense as a result of the executive staff turnover. However, the current ratio has improved in 2017.

When viewing the ten-year trending of net income (Table 4), using earnings before interest, tax, depreciation and amortization (EBITDA), the current year shows a gain in EBITDA.

Cost of Services per MemberAs required per CSHP Administrative Policy, the cost of providing services per member in year ending June 30, 2017, was $323.80. The cost per member in year ending June 30, 2016, was $425.56. This represents a decrease/increase of 24% from 2016.

Controls and Fiscal AccountabilityOur certified, independent account, Craig Fechter, CPA, determined that the financial statements represent fairly, in all material respects, the financial position of CSHP as of June 30, 2017. The changes in its net assets and its cash flows for the year then ended in accordance with accounting principles generally accepted in the United States of America.

ConclusionThis year, our new CEO has been working tirelessly with the CSHP Board of Directors and staff to provide high quality services for our members and to strengthen our financial positions. There has been a gradual increase in member-ship, particularly in the “new practitioner” category. With a steady revenue, we are able to provide additional member services, including certificate programs.

I’d like to thank all the members of CSHP for allowing to me to serve as the trea-surer over the past year. The time spent with CSHP members has been very rewarding. I forward to serving you for the remainder of my term. o

Good morning. It has been a little over a year since I joined CSHP and its been a productive year. A year of rebuilding, growing, and embracing opportunities to bring

value to our members

I want to take a few minutes to share some highlights. First starting with membership:

• Since January 2017, our total membership has increased 61% (2672 to 4314).

• The greatest growth was in the category of new practitioner year 1 with a six to seven-fold increase (56 to 436) followed by the pharmacist category with a 165% increase (1084 vs 655).

• Our current membership numbers mirror our performance in fiscal year 2014, the time between July 2013 and June 2014.

• You may be wondering does our membership number equate to increased revenue? Absolutely, our current fiscal year started July 1, we are just about one-third of the way into the year, and we are at 65% of the projected goal for membership dues. One-third in and already two-thirds of the way to goal!

• Our performance is part of a positive membership growth trend over the last two years. In fiscal year 2015-16 (ending June 2016), we achieved 41% of the membership dues goal; however, in fiscal year 2016-17 (ending June 2017), we achieved 86% of our membership dues goals.

• Speaking of revenue goals, for our total revenue goals inclusive of membership dues, Seminar and sponsors, we are at 73% of goal (goal = $2,057,413, current = $1,509,421) and we are only one-third of the way into our fiscal year. At this pace, we should have one of our best years.

• And our student membership is growing with the addition of a new student chapter. A big welcome to West Coast School of Pharmacy.

CSHP CEO Report

Moving to our Seminar highlights:

• Our total attendance as of yesterday afternoon is 2,105, and exceeds our attendance at Disneyland last year. This may very well increase with onsite registration. Our attendance is 10% higher than we were in Las Vegas in 2012.

• Participation has increased in many areas:

o Poster sessions: 150 o Residency Showcase: 102o CV review: a whopping 149, and

a big thanks to the 50 volunteers who agreed to review and provide feedback

• A first for us is the pre-Seminar certificate programs covering advance practice pharmacist with an emphasis on chronic disease, pharmacoge-nomics, and transitions of care. Many thanks to our amazing partners who made this happen.

• We are currently on track to exceed our projected gross revenues by 12% (goal is $1,073,650, and we are currently at $1,204,084), and it may be one of our best Seminars, however, the final numbers won’t be in for a while.

What is on the horizon:

• We are launching a spring confer-ence (April 27-28, 2018) – the West Coast Preceptor Training Conference. Our goal is to create an environment that facilitates peer-to-peer learning while fostering and strengthening the preceptor community. The event will be held at the Asilomar in Monterey. Stay tuned for more information.

• Some may have heard about the recent establishment of the California Future Health Work Force Commission with the goal to identify a strategic plan to address the health work force needs for the next 30 years. The Commis-sion will be supported by a Technical Advisory Committee composed of about 30-plus appointed representa-tives across the healthcare continuum. I am pleased to say that last month I was appointed to the Technical Advisory Committee and will serve in the capacity of representing phar-macy professionals and provision of pharmaceutical care across all practice settings. This is an honor I don’t take lightly. To be the best representa-tive of all pharmacy professionals, I have personally reached out to other pharmacy associations and vested stakeholder to seek their input to inform my representation.

It has been a productive year. I personally want to thank the CSHP Board of Directors for their strong commitment and dedication. Your board has been meeting monthly instead of the usual quarterly. Without their dedication, support and guidance, much of what has been accomplished wouldn’t have been possible. Equal in their dedication, hard work and commitment, is the CSHP staff. I can honestly say without them and their tireless efforts most of what you have experienced would not have been possible.

Thank you for allowing me to serve as your CEO and I am hopeful that this time next year I will have more positive news of our growth, policy influence, and value to our members. o

Actions of the Committee1. The BRC completed its review of the Article of Incorporation, Bylaws, and House of

Delegates regulations in 2016. The work of this year’s committee was focused on the review and revision of the CSHP Administrative Policy manual.

2. The committee reviewed and recommended a complete reorganization of the Admin-istrative Policy manual. Such actions included:

e. Recommended CSHP Board review of all policies related to awards for streamlining and consistency.

f. Recommended a new system of a 5-year review based on topic rather than by year of last review.

g. Recommended a standard format for current and future policies to maintain consistency.

Conclusion• The BRC feels that charges 1 and 2

have been successfully completed.

• The recommendations above will lead to completion of charge 3, though the BRC recognizes that substantial work may be necessary by the BOD to review and modify the 86 policies recommended for amendment. The BOD may wish to consider whether this BRC or a new BRC should be organized to review the 86 up for review.

• The BRC recognizes that charge 4 does not provide objective outcomes and recommends an open discussion between the BOD and key stake-holders to verify whether the charge has been successfully completed. o
