WELCOME TO PRINCIPLES OF PEDIATRIC ANESTHESIA AND CRITICAL CARE!

It is a pleasure to welcome you to this year’s Principles of Pediatric Anesthesia and Critical Care conference in Boston, hosted by the Department of Anesthesiology, Critical Care and Pain Medicine at Boston Children’s Hospital and accredited by Harvard Medical School. We face an ever-changing world, and the practice of pediatric anesthesiology can be a risky endeavor. As the specialty continues to evolve, we are pleased to offer you an exciting, cutting-edge pediatric anesthesia conference focusing on innovation, common challenges, ‘hot’ topics, pediatric anesthesia research and interactive workshops.

This year we are very fortunate to have Evan Kharasch, MD, PhD, as our keynote speaker. Dr. Kharasch is Editor-in-Chief, Anesthesiology, and Professor of Anesthesiology, Vice-Chair for Innovation and Director of Research Entrepreneurship at Duke University School of Medicine. Dr. Kharasch will discuss Anesthesia Pharmacogenetics and its current role in anesthesia practice. Additional presentations by leaders in their field include Frances E. Jensen, MD, FACP, who will discuss the scientific basis for the strengths and vulnerabilities of the adolescent brain, Robert C. Tasker, MBBS, MD, who will review the recently updated guidelines for management of pediatric severe traumatic brain injury, and Monica E. Kleinman, MD, who will review updates on pediatric resuscitation.

The conference offers interactive optional breakfast and lunch breakout panels each day. These informal sessions are staffed by our faculty and invited speakers, and are designed to facilitate a two-way learning process between attendees and presenters. We will be featuring a complimentary Pediatric Airway workshop on Saturday afternoon. Optional workshops include a pre-conference full-day Point-of-Care Ultrasound Workshop, Pediatric Patient Blood Management Workshop, Ultrasound-Guided Vascular Access, Pediatric Regional Anesthesia, and Acupuncture. Registrants also have the option to pair two or three days of the conference with a pediatric MOCA® course, which will take place in the operating rooms and recovery area of Boston Children’s Hospital or Simulation Center.

We encourage participants to share their research and clinical experiences by submitting their work for a Poster Session, which is held during the complimentary wine reception on Friday evening. Our objective for the conference is to create an opportunity for Attendees and Faculty to interact with each other for the benefit of their pediatric patients, education and research. We are confident that you will enjoy your visit to Boston and that you will have a valuable experience.

WHO SHOULD ATTEND?Anesthesiologists, Intensivists, Certified Registered Nurse Anesthetists, Student Nurse Anesthetists, Residents, Fellows, Pediatric Nurses, and other health care workers that take care of pediatric patients in the perioperative setting.

COURSE DESCRIPTIONPediatric anesthesia and critical care continues to present challenges to anesthesiologists, nurse anesthetists, nurses and critical care physicians. This course provides a comprehensive and up-to-date review of the most important changes and advances relevant to the practice of pediatric anesthesia and critical care. The program includes a variety of evidence-based lectures with question and answer sessions, hands-on workshops in difficult airway management, point-of-care ultrasound, pediatric patient blood management, ultrasound-guided vascular access, regional anesthesia and acupuncture. Our mission is to provide information, strategies, and skills for improving care and outcomes for pediatric patients in the operating room, outfield and the intensive care unit.

LEARNING OBJECTIVESUpon completion of this activity, participants will be able to:• Describe the strengths and vulnerabilities of the adolescent brain.• Evaluate children with concussion.• Determine anesthetic management of the child with a vulnerable brain.• Identify new drugs for analgesia and sedation.• Determine the optimal anesthesia for the child with obstructive sleep apnea.• Discuss the role of genetics in determining drug kinetics and dynamics.• Develop virtual reality techniques to reduce perioperative anxiety.• Identify advances in pediatric neuroanesthesia.• Identify safe techniques for the anesthetic management of the pediatric patient with

congenital heart disease.• Understand the physiology and pharmacology of neonates and infants.• Discuss the new guidelines for management of the pediatric patient with severe traumatic

brain injury.• Classify updates in the management of pediatric resuscitation.• Determine treatment options to effectively manage pain and determine when and how to

use regional blocks in pediatric patients.• Understand the phenomenon of surgically-induced neuropathic pain.• Identify safe techniques to obtain peripheral and central vascular access with the use of

ultrasound in the pediatric patient.• Evaluate children with a difficult airway.• Identify important papers applicable to children published within the last year.

Gomez Morad, MDPoint-of-care ultrasound is an increasingly valuable tool in the perioperative management of pediatric patients. At the conclusion of this workshop, participants will be familiar with the basic concepts of the ultrasound technique, basic physics, and the use of appropriate transducers. Participants will gain knowledge in the use of ultrasound to evaluate ventricular function and volume status. Participants will also gain practical skills in using ultrasound to evaluate endotracheal intubation and the presence of a full stomach. In addition, participants will recognize how to evaluate for the presence of pneumothorax and pleural effusion.

At the conclusion of this course, the participants will be able to:• Describe the basic concept of the ultrasound technique, basic

physics, and the selection of appropriate transducers.• Learn how to use ultrasound to evaluate ventricular function

and filling.• Learn how ultrasound can be used to assess for a

pneumothorax, endotracheal intubation and a full stomach.

Appropriate pain control in the perioperative period is of the utmost importance in the pediatric population. Regional anesthesia is an important component of the current multimodal approach to perioperative pain management. ERAS (Enhanced Recovery After Surgery) guidelines emphasize the importance of adequate pain management and early mobilization of the patient and encourages opioid sparing techniques. Currently, the best opioid sparing technique available to pediatric anesthesiologists are neuraxial and peripheral nerve blocks.Although more research is necessary, we consider that regional anesthesia techniques offer many advantages in the perioperative period. Regional anesthesia decreases the amount of general anesthetic, provides pain relief, and decreases the utilization of opioids with their side effects, facilitating early mobilization and early oral intake, potentially helping to decrease hospital length of stay and costs.Many regional anesthesia techniques have been developed in the last decade thanks to the current use of ultrasound. Ultrasound has increased the success and safety of regional anesthesia techniques. This workshop is designed for anesthesia practitioners with basic knowledge of ultrasound who want to learn more about peripheral nerve blocks commonly used in pediatric patients and acquire knowledge of newly developed blocks and their application in the pediatric population.

Learning Objectives:• Acquire knowledge and understanding of the basic anatomic and

ultrasound principles for image acquisition and optimization while practicing on pediatric models.

• Review the important anatomic landmarks and sonoanatomy required to perform common, advanced and novel blocks in children (including paravertebral (PV), Pectoralis (PECs), Erector Spinae (ESP), Lumbar Plexus (LP), Fascia Iliaca (FI), and Quadratus Lumborum (QL).

• Recognize the indications, contraindications and possible complications of common and newly developed blocks in pediatric patients.

• Discuss the technical recommendations for the successful placement of nerve blocks (tips and tricks about patient position, transducer manipulation and movements, needle alignment).

• Compare different clinical models of pediatric regional anesthesia programs from experts in the top programs in the country.

Acupuncture can be integrated into pediatric perioperative care and pain practice. Attendees of this workshop will attend a brief didactic lecture.
