Check with admissions and records to see if you are an active student. Sign up for at least 1-hour of 697 the semester you graduate. Don’t forget to submit your application for graduation and diploma http://www.as.wvu.edu/graduate/Application.htm. This is usually due in the middle of dead week at the latest – perhaps earlier. You should check and confirm that you have all forms in for graduation by the posted date (basically application for graduation, $30 graduation fee, and the request for shuttle sheet). This is also a deadline for taking care of any incompletes or NRs. You should download the shuttle sheet request form (at http://www.as.wvu.edu/graduate/ssreqest.htm ideally a yellow sheet) and obtains signatures. Hope has copies. This sheet specifies the date, time, & title of your defense and is signed by all committee members & department chair (or associate chair). Kim Dixon will put the Blue Shuttle sheet together. This sheet provides a list of any outstanding course incompletes – for example in research hours. Make sure you run necessary paper work on these. The committee signs that when student successfully completes the defense. Post notice at least 1 week prior to defense. Kim Dixon is a key information source regarding graduation requirements. When in doubt, call Kim 293 2505. You should obtain information on the electronic thesis formats from the graduate forms site and check your thesis to ensure formatting is correct. You should also download the ETD signature approval form at http://www.libraries.wvu.edu/theses/forms/ETDform.pdf Finally, upon completion (after final defense and ETD are complete) we have a "Plan of Study" completion form, to be done by your advisor and submitted to Kim (formerly Carol) -- this is to be signed by the Grad Program Chair and is the departmental signoff on the degree. This also involves checking to see that your courses taken match your plan of study.
