SURVEY WAS TO IDENTIFY AND QUANTIFY ASBESTOS-CONTAINING MATERIALS

(ACM) AND LEAD BASED PAINT WITHIN AND ON THE EXTERIOR OF THE

STRUCTURES IN THE AREAS OF PLANNED RESETS OR REPAIRS. THE SURVEY

INCLUDED A VISUAL INSPECTION, SAMPLE COLLECTION, AND SUBSEQUENT

LABORATORY ANALYSIS OF SAMPLES OF MATERIALS SUSPECTED TO CONTAIN

ASBESTOS OR LEAD. SAMPLES WERE COLLECTED AT THIS FACILITY. ACM AND

LEAD PAINT WERE FOUND TO BE LOCATED WITHIN THE AREAS OF THE IDENTIFIED

G. ALL CONCRETE, MASONRY, TILE AND PLASTER SURFACES INDICATED TO BE CUT AND PATCHED OR ARE ON A

SURFACE IN WHICH A DOOR, WINDOW, OR PORTION OF THE SURFACE IS TO BE REMOVED, SHALL BE CUT

WITH MASONRY SAWS OR PREPARED FOR CLEAN JOINTS AND PATCHED FLUSH TO RECEIVE NEW MATCHING

GENERAL NOTES FOR EXISTING CONCRETE SLAB

REMOVAL AND REPLACEMENT

A. SAW CUT AND REMOVE EXISTING CONCRETE SLAB IN AREAS AS SHOWN. TO THE GREATEST EXTENT

POSSIBLE, MAKE EVERY EFFORT TO MINIMIZE THE AREA OF SLAB TO BE DEMOLISHED.

B. UPON COMPLETION OF PLUMBING/PIPING INSTALLATION, FILL VOIDS IN SUBGRADE WITH FLOWABLE FILL

MATERIAL. FLOWABLE FILL SHALL HAVE A MAXIMUM COMPRESSIVE STRENGTH OF 65 PSI (40 PSI MINIMUM) AT

28 DAYS. CEMENT SHALL BE ASTM C150 TYPE I/II WITH 50 LB/CY, FLY ASH 600 LB/CY (ASTM C 618, TYPE F),

FINE AGGREGATE 2,500 LB/CY (ASTM C33, PEA GRAVEL WITH SAND), WATER CONTENT 500-550 LB/CY (ASTM

C94), AIR ENTRAINMENT 4-7% (ASTM C260). EVAPORATION OF BLEED WATER SHALL NOT RESULT IN

SHRINKAGE OF MORE THAN 1/8 INCH PER FOOT OF FLOWABLE FILL DEPTH. ALLOW FLOWABLE FILL TO CURE

FOR A MINIMUM OF 12 HRS.

C. REPAIR/REPLACE EXISTING VAPOR RETARDER (IF ENCOUNTERED) WITH 6 MIL POLYETHYLENE VAPOR

RETARDER SHEET PER MANUFACTURER'S RECOMMENDATIONS.

D. IN AREAS OF SLAB TO BE PATCHED, CLEAN EXISTING CONCRETE SURFACES TO ACCEPT NEW CONCRETE.

PER MANUFACTURER'S RECOMMENDATIONS, APPLY EPOXY BONDING AGENT (ASTM C881, TYPE V) TO

SURFACES OF EXISTING SLAB WHICH ARE TO BE IN CONTACT WITH NEW CONCRETE.

E. CONCRETE SLAB SHALL MATCH THE THICKNESS OF THE EXISTING SLAB (4" MINIMUM THICKNESS) AND SHALL

SHALL HAVE A MINIMUM COMPRESSIVE STRENGTH OF 3,500 PSI AT 28 DAYS. PORTLAND CEMENT SHALL

CONFORM TO ASTM C150, WATER/CEMENT RATIO = 0.57, AGGREGATE SHALL BE PER ASTM C33, DEFORMED

STEEL REINFORCEMENT SHALL BE PER ASTM A615 GRADE 60. PROVIDE #4 AT 16" ON CENTER SPACING IN

EACH DIRECTION OF SLAB WITH A MINIMUM CLEAR COVER OF 3/4".

F. FINISH CONCRETE WITH STEEL TROWEL AND MATCH EXISTING CONCRETE FINISH SLAB HEIGHT TO THE

GREATEST EXTENT POSSIBLE.

G. CLEAN AND PREPARE NEW AND EXISTING CONCRETE FOR ACCEPTANCE OF EITHER "THICKSET" OR "THINSET"

TILE APPLICATION, TO BE DETERMINED BASED ON THE EXISTING CONDITIO AND THICKNESS. PROVIDE

LATEX-PORTLAND CEMENT MEDIUM MORTAR BED FOR ALL TILE - EXTRA HEAVY SERVICE RATED PER TCNA.

ALLOW CONCRETE TO CURE TO MEET MORTAR MANUFACTURER RECOMMENDATIONS PRIOR TO

INSTALLATION. PROVIDE WATER-CLEANABLE EPOXY GROUT. COMPLY WITH MATERIAL STANDARDS IN ANSI'S

"SPECIFICATIONS FOR THE INSTALLATION OF CERAMIC TILE" THAT APPLY TO MATERIALS AND METHODS

INDICATED.

LOCATION OF THE BRACING WIRES SHALL BE EXTENDED TO AND FASTENED TO

STRUT SHALL BE ADEQUATE TO RESIST THE VERTICAL COMPONENT INDUCED
