• The County 4-H Portfolio should include thefollowing SECTIONS and be assembled in thisorder:

Cover- all portfolios must contain the approved 4-HPortfolio provided by the Extension Office.

Cover Page- 3X5 or 4X6 photo of the 4-Her withName, County and Age centered beneath photo.

Table of Contents- one page listing sections includedin portfolio.

Personal Profile- form containing information for themember.

Leadership Profile- comprehensive listing ofleadership experiences over the 4-H career. (formprovided)

Citizenship/Community Service Profile- comprehensiveexperiences over the 4-H career (form provided).

Project Profiles- a member must include a completedproject record form for each project they selected forthe current year. If the member has numerousprojects, tabs should be used to separate eachproject. Projects photos are allotted a single pageonly to be placed at the end of the specific project.

Additional 4-H Photos and SupplementalMaterials- no more than 3 pages total. Mayinclude photos with captions, news clippings,news clippings, letters, certificates, etc. NoLayering- use front side only. Do not use plasticsheet protectors. These 3 pages are in additionto the photos included with your project profiles.

• Part I- Introduce yourself- remember, people arereading your story that do not know you.Include your age, interests, and when and whyyou joined 4-H. You can always include anattention getting paragraph before yourintroduction. Capture the readers attention!

• Part II- Tell/show how you used the life skillsand knowledge from your 4-H experience in yourdaily life. Include major learning experiences,special interests and unusual situations that youencountered.
