El número de personas que viven con VIH se ha incrementado de aproximadamente 8 millones en 1990 a casi 33 millones en la actualidad y sigue en aumento. Alrededor de un 67% de personas que viven con VIH se encuentran en África subsahariana.

The CD4+ T-cells of two of these individuals, designated EU2 and EU3, are highly resistant in vitro to the entry of primary macrophagetropic virus but are readily infectable with transformed T-cell line adapted viruses. We report here on the genetic basis of this resistance. We found that EU2 and EU3 have a homozygous defect in CKR-5, the gene encoding the recently described coreceptor for primary HIV-1 isolates. These individuals appear to have inherited a defective CKR-5 allele that contains an internal 32 base pair deletion. The encoded protein is severely truncated and cannot be detected at the cell surface. Surprisingly, this defect has no obvious phenotype in the affected individuals. Thus, a CKR-5 allele present in the human population appears to protect homozygous individuals from sexual transmission of HIV-1. Heterozygous individuals are quite common (approximately 20%) in some populations. These findings indicate the importance of CKR-5 in HIV-1 transmission and suggest that targeting the HIV-1-CKR-5 interaction may provide a means of preventing or slowing disease progression.

7. DIAGNÓSTICO:

Indicaciones para realizar un Test de VIH
• Requerimiento del paciente.• Pacientes que muestran riesgo de infección por VIH.• Condiciones que sugieran compromiso de la inmunidad incluyendo
candidiasis oral otras infecciones oportunistas.• Tuberculosis.• Candidiasis vaginal recurrente.• Mujeres embarazadas.• Hospitalizados en áreas de alta prevalencia.• Donantes de órganos o semen.
RECOMENDACIONES CLINICAS DE LOS EXAMENES
Serología para VIH• La serología mediante el método de Elisa es el test indicado para
detección de infección crónica. Tiene una especificidad y sensibilidad del 99 %.
• La prueba de Elisa debe ser confirmada con Western Blot• La prueba de Elisa tiene un porcentaje de falsos positivos del 2 %.• WB falsos: Son raros, incluyendo casos de auto anticuerpos y error
técnico.• Inmunofluorescencia: es considerada una de las pruebas confirmatorias
aceptadas en el país, requiere capacitación por ser una prueba operador dependiente.
Detección Viral
• La carga viral es útil para el diagnóstico, pero tomando en cuenta que cuando la viremia reportada es baja puede ser un falso positivo.
• Es útil para detectar infección en neonatos con una Sensibilidad >99% y una especificidad superior al 98%.

PEP = Postexposure prophylaxis.
* Class 1: Asymptomatic HIV infection or known low viral load (< 1500 RNA copies/mL). † Less severe exposure may involve a solid needle or superficial injuries. ‡ More severe exposure may involve a hollow-bore needle, deep puncture, visible blood on the device, or a needle used in patient's artery or vein.§Class 2: Symptomatic HIV infection, AIDS, acute seroconversion, or known high viral load. ¶PEP is optional and should be based on an individualized decision by the exposed person and the treating clinician. If PEP is offered and taken and the source is later determined to be HIV-negative, PEP should be stopped.

8. TRATAMIENTO:1. Se recomienda tratar a todos los pacientes con conteo de CD4 de ≤350 cells/mm3 sin
considerar el estadio clínico de la OMS.
2. Se recomienda en todos los pacientes con estadio clínico 1 y 2 (OMS) realizar conteo de CD4 para decidir cuando iniciar el tratamiento.
3. Se recomienda tratar a todos los pacientes con estadio clínico 3 y 4 (OMS) sin considerar el conteo de CD4.
