Living the MissionNational Church Residences is an organization on a mission, one that becomes even more important with each passing year. We have completed the first year of a five-year strategic plan refining and realigning our work to support the growing needs of our senior population. The planning process included the development of our mission statement:

Originating from a Christian commitment of service, our mission is to provide high quality care, services and residential communities for all seniors.

We also created a vision to chart our pathway for growth: To advance better living for all seniors, enabling them to remain home for life.

Living this mission and fulfilling this vision require thousands of team members to

make many decisions each day. We encourage each other to guide each decision according to our core values: mission, compassion, professionalism and leadership.

We work continually to improve our skills and our systems. But achieving high quality is also dependent upon growing our capacity for compassion. We tell all new employees that we want them to treat others the way that they want to be treated. We ask them to listen and to be actively engaged with our residents and clients. We place a high priority on treating residents, clients, their families and our team members with dignity and respect.

We ask ourselves to live the mission. This is not always easy, but it is the best way to advance better living for all seniors, enabling them to remain home for life.

Philanthropy Highlights

In 2014, National Church Residences acquired Telfair Arms, a senior affordable housing community in Savannah, Georgia. Originally built in 1886 as the Telfair Hospital for Women, the building needed comprehensive renovations in order to best serve our residents and the surrounding community.

These renovations allow National Church Residences to expand our current Home for Life Program to the residents of Telfair Arms and individuals in the surrounding community. Home For Life is significant to our mission strategy of assisting seniors to remain healthy and happy while aging in place, wherever they call home.

The Harry and Jeanette Weinberg Foundation made a $185,000 grant to provide adequate space for the Home for Life Program at Telfair Arms (a medical suite), the creation of professional office space for the Service Coordinator and to help renovate the resident common areas. A holistic approach is used with services across the health care spectrum, resulting in the best possible care for each individual. This project will move forward National Church Residences’ commitment to transforming the way that seniors live and thrive in Savannah, Georgia.

JUNE 2016

The Randolph A. Jester Foundation initiated a $15,000 fund to underwrite a final wish program for National Church Residence’s Hospice patients. The Randolph A. Jester Final Wish Program will permit the hospice team to fulfill a patient’s wish that exceeds the financial wherewithal of Hospice’s normal fundraising activities. Wishes fulfilled can be as simple as a family dinner at a favorite restaurant, or underwriting a visit by a family member.

Our Hospice Program provides compassionate assistance to those who are at the end of life’s journey. We believe that hospice care is about living and that each of us has the right to a passing that is dignified and free of pain.

Many of the hospice patients we serve have led modest lives and even simple needs can be beyond their personal resources. While philanthropy has been a source of support to hospice patients for everything from groceries to phone services that keep them connected with their loved ones, general fundraising activities have traditionally been inadequate to satisfy a hospice patient’s “one last wish” or life-long dream.

Mary Garcia was diagnosed with cancer and given six months to live. That diagnosis was a year and a half prior to her receiving the Volunteer of the Year Award. Mary attributed her extended life to her volunteer service.

“Even when she is not volunteering, Mary is

volunteering,” shared Maria Maldonado, Service Coordinator at Frio Crossing. “She is always looking out for the needs of other residents.” Mary regularly cooked food to share with residents, kept an eye on those that had become sick and ran errands when needed. Her gifts of time and service made her not only an extraordinary volunteer, but an exemplary human being.

A celebration was held to recognize Mary for being the National Church Residences Volunteer of the Year, one month before she passed away. Her friends, family, fellow residents and staff gathered and shared the impact she had on their lives. Her recognition event became a living funeral and her demonstration of service up until her last day of life is a true reflection of a life well-lived.

Grants Solve Transportation Needs

The Sisters Health Foundation

SunTrust Bank Volunteers

Transportation is critical to a senior’s desire to age in place. In support of enhancing mobility options for seniors and persons with disabilities, the Federal Transit Authority provides funding to non-profits through a competitive application process known as the 5310 Specialized Transportation Program.

Our Community and Housing Based Transportation Program in Belmont County, Ohio was awarded a 5310 grant amounting to $84,000 for two handicapped accessible vans to transport residents and community clients to non-emergency medical appointments, shopping and social events. Center for Senior Health, our Franklin County, Ohio Adult Day program received two 5310 awards, totaling over $200,000. One grant underwrote the acquisition of three handicap accessible vehicles to transport adult day clients to and from the centers and the second grant-subsidized a portion of the Center for Senior Health’s fleet maintenance costs.

The Sisters Health Foundation awarded $20,000 to support a new transportation project located at Putnam Howe Village in Belpre, Ohio. The project identified that the lack of public transit options and/or disposable income necessary to have access to a working vehicle has been an issue for low-income residents. The project provided 711 trips covering more than 19,000 miles. The residents at both Putnam Howe and Restoration Plaza of Barlow now have the opportunity to receive regularly scheduled trips to grocery shopping, pharmacy, senior center, and community events.

SunTrust Bank is one of the nation’s largest and strongest financial holding companies and is headquartered in Atlanta, Georgia.  Their purpose of Lighting the Way to Financial Well-Being, was seen in action by their executive leadership.

Lynette Bell, SVP of Community Development for SunTrust Bank, joined the Southeastern Advisory Board in 2016 and quickly went to work making an impact in National Church Residences communities.

On September 21, 2016, the CEO of SunTrust Bank, William Rogers, and his ten direct reports volunteered at the Commons at Imperial Hotel in Atlanta, Georgia.  The Commons at Imperial Hotel is a 90 unit permanent supportive housing (PSH) community for the disabled and homeless located at the historic Imperial Hotel.

The volunteer event started with a tour of the building and each executive was partnered with a resident. The pairs completed a

resume worksheet and went through mock interview questions.  As the event kicked off, Rogers shared with the group that he and his team hoped to learn from the residents as much as they planned to teach.  The event was a tremendous success in terms of relationship building and an important component of the PSH vocational training program.

The SunTrust Foundation Board of Directors recently awarded The Commons at Imperial Hotel a $100,000 grant to establish our Employment Services Program. SunTrust will continue to be involved as volunteers during the implementation of this program. The combination of volunteer and financial support strengthens our communities and helps to deepen the impact of our mission in the lives of our residents.

Mill Run Tranquility Garden

ORIX Foundation Expands Health ServicesNational Church Residences received a $50,000 grant from the ORIX Foundation for the expansion of the Healthy at Home Program which allows for the provision of primary and episodic health care to uninsured or under-insured residents at two of our Columbus, Ohio permanent supportive housing communities, Commons at Livingston, serving formerly homeless and/or disabled veterans exclusively, and Commons at Chantry, serving low-income, at-risk

families with children and single adults — many of whom have struggled with long-term homelessness, trauma, disability, and crippling poverty. In addition to primary health care services, Health and Wellness Education and Prevention Services (including TB testing, flu shots, diabetes testing/education, smoking cessation, etc.) was offered to all of the 250+ residents at these locations.

annual report 2016-17 u 11

The completion of Phase I of the Tranquility Garden at our Hilliard, Ohio Mill Run Assisted Living and Memory Care community was made possible through financial and in-kind donations from Home Depot, The Randolph A Jester Foundation, Scotts Miracle-Gro, Roehrenbeck Electric, Merrill Lynch, Environmental Management, Inc., MKSK, the Trombetti and O'Grady families, and many individual donations, both large and small.

Donors, residents, family and friends of Mill Run gathered on May 11th to celebrate the dedication of the garden’s initial phase. This first phase, named Maggie’s Garden, in honor of a former Mill Run resident whose passion for gardening provided the inspiration for the project, includes a water feature, a perimeter loop walk, a seasonal display area, security fencing and signature plantings.

Mill Run residents, families and staff continue to fund raise for this ambitious garden project. Phase Two construction is well under way and garden planners remain enthusiastic about the transformative possibilities that the garden’s evidence based design offers to the senior residents of Mill Run.

Mission Day 2017

12 u annual report 2016-17

Mission Day is a grassroots fundraising effort that is hosted by National Church Residences communities across the country. This year, sixty-six properties hosted events, each with the purpose of increasing awareness for our mission, elevating our brand, strengthening our community relationships, and raising funds for the programs and services provided at the hosting communities.

Atlanta, southeastern Michigan and central Ohio were Mission Day focus markets in 2017. These communities hosted events on the same date to enhance collaboration and visibility. Nine events were hosted in Michigan, seven in Atlanta and seven in central Ohio. Monarch Private Capital was the presenting sponsor for Atlanta

Mission Day and JenCare was the Silver sponsor. JenCare committed to donating a van from their fleet to the residents of Trinity Towers and Lakewood Christian Manor. In attendance were city council members, state senators, school board officials, county chairman, the First Lady of Atlanta and three mayoral candidates.

The central office staff participated in the Mission Day celebration by volunteering at the local events, hosting a recycling event, and a Mission Day breakfast. Senior leadership jumped on the Mission Day tour bus visiting with residents and offering gratitude and appreciation to local staff. In total, more than $50,000 was raised.
