Zwaan Farm EVH Outbreak

On March 10th, Peguis (15 yr TBx gelding) and Reyna (14 yr Arab/QH mare) are taken to a Saskatoon riding stable. In the previous month, only Reyna had left the farm – twice for John’s riding lessons at the stable. No other horses had come on to Zwaan Farm since October.

Reyna is tacked up in the first stall and tied to the bars so she cannot contact the corner feederReyna is tacked up in the first stall and tied to the bars so she cannot contact the corner feeder.  John has a 45 minute riding lesson in the arena. There are 4‐5 other horses in the arena during that time; all apparently normal. Reyna has no direct contact with any horse, and is only touched by Trisha, John and the riding instructor. After the lesson, Trisha leads Reyna  directly to the trailer and untacks her at the trailer.

Brian brings Peguis into the Polo wing, which is empty of horses. Peguis is placed in cross‐ties and Brian works on his hooves. Peguis has no contact with any other horses and no one but Brian touches Peguis. When done, Brian brings Peguis directly to the horse trailer.

The horses are returned to Zwaan Farm and turned out and the Zwaans go on vacation for a week.

Dr Katherine Ball takes care of the farm for the Zwaans. On March 15th, Dr Ball informs Trisha that an EHV‐1 outbreak has occurred at the stable. Trisha assumes that the risk to Reyna and Peguis is negligible, but instructs Dr Ball to watch the horses closely.

I h i f M h 17th D B ll T i h i Fl id ll h h P i i h i i f liIn the evening of March 17th,  Dr Ball contacts Trisha in Florida to tell her that Peguis is showing signs of colic. Trisha instructs her to enlist the aid of Dr Chris Clark, who lives nearby. Dr Ball and Dr Clark perform a routine examination of Peguis and conclude that it is a “gas colic”. Peguis is given flunixin for pain and kept up in a paddock for observation.

On March 18th  at 7 PM, while enroute back to Saskatoon, Dr Ball contacts Trisha to tell her that while Peguis is not showing overt signs of abdominal pain, he is laying down more than normal and appears depressed. Trisha instructs her to give him an additional dose of flunixin.

When they arrive home at 11 PM, Trisha and Brian examine Peguis. He appears depressed, has lost weight, is reluctant to eat and a skin pinch test indicates dehydration. Trisha passes a nasogastric tube and administers 1 gallon of mineral oil and 1 gallon of water with electrolytes. 

March 19th: at 6 AM mineral oil is present on Peguis’ legs and tail but he stillMarch 19 : at 6 AM, mineral oil is present on Peguis  legs and tail, but he still appears “off” and clinically dehydrated. Trisha passes NG tube and gives 1 gal water with electrolytes and additional flunixin. In the PM, Peguis is less depressed and his appetite improved. Other parameters within normal limits. All other horses are individually fed grain and all appear normal, including Reyna. All go out to pasture to eat hay, except Peguis remains in the paddock for observation.

March 20th: at 6 AM, Peguis appears less depressed, but Trisha collects blood for testing. He is fed and turned out in pasture with rest of horses. All other horses come for hay and appear normal At noon Brian observes the horses coming into front pasture and all appearappear normal. At noon, Brian observes the horses coming into front pasture and all appear normal. At work, Trisha receives the results of Peguis’ CBC and serum chemistries; they are not diagnostic for any specific problem.At 3 PM, Reyna is seen down in pasture by Brian and John, in right recumbency and paddling her legs. Trisha is contacted and arrives at 3:30 PM: Reyna is unable to rise, shows bizarre nystagmus (eye movement), tachypnea (labored breathing), but her heart rate is only 56‐60, and mucous membranes are pink and moist. Tail tone appears reduced, but anal tone is normal and she leaks urine. Reyna is moved on a sheet of plywood by tractor to the paddock and placed in deep straw. 

At 5 PM, Dr Francesca Sampieri arrives with the ambulatory students and helps Trisha manage Reyna for the next 36 hrs (IV fluids, DMSO, flunixin, dexamethasone, sedatives, rolling over every 2 hrs). Blood is submitted for CBC/chemistries, but the results are not diagnostic for any specific problem. Blood and a nasal swab are also collected to test for EHV. Reyna continues to show signs of brain, brainstem and cerebellar inflammation/damage.

With only minimal improvement in neurological signs after 36 hrs of treatment, Reyna is euthanized at 2 AM, March 22nd.

March 22nd: Reyna is taken to Prairie Diagnostic Services for a post‐mortem at 9 AM. All other horses at the farm appear normal. A complete post‐mortem is performed by Dr Ahmad Al‐Dissi. Reyna’s brain and spinal cord are grossly normal 

t f i ibl d th b O h lf fexcept for some visible edema on the cerebrum. One half of her brain is sent to the CFIA for rabies testing.

The only other gross lesions were the muscles along the spinal column for approximately ½ meter in the withers area. Bilaterally, the longissimus dorsi muscles, underneath the scapulae (shoulder blades), were dark red and friable. The skin and all the muscles superficial to these muscles in all directionsand all the muscles superficial to these muscles in all directions were normal in appearance. There remains no explanation for these findings.

March 23rd: Reyna’s rabies test results are tinegative.

On March 27th, Porsche (14 yr old Arabian mare) is depressed and off feed. Her temperature is 40.1 C. Trisha informs WCVM clinicians that she believes that the horses on Zwaan Farm are affected by EHV‐1.

Samples from Reyna’s brain are submitted for immunohistochemistry testing on March 28th. Because it is a 2 day test, a preliminary ‘rapid incubation’ is done for EHV‐1. The preliminary results are negative. 

From March 28th on, Trisha and Brian examine all horses on the farm twice daily. Porsche remains febrile and off feed and is treated symptomatically with flunixin The only other

remains febrile and off‐feed and is treated symptomatically with flunixin. The only other horse to have a fever (≥38.5 C) is Cain (6 yr old Arabian gelding).  Cruise (19 yr old Arabian gelding) shows slight blepharospasm (squinting) and tearing of the left eye on the evening of March 29th. It is worse by March 30th, so Trisha applies fluorescein stain to rule out a corneal ulcer or laceration. There is no evidence of corneal damage, so Cruise is given flunixin and atropine is applied topically to the eye as Trisha suspects uveitis (inflammation within the eye).

March 31st: Porsche’s temperature is normal and she is bright and alert and eating, but shows ataxia when she tries to walk.

Cruise’s left eye shows only minimal improvement.

Trisha goes in to the WCVM to seek further assistance.

Trisha contacts fellow clinical pharmacologist, Dr Lara Maxwell at Oklahoma State University, who had been funded by the Grayson‐Jockey Club to investigate the efficacy of the human antiviral drug, valacyclovir, against neuropathogenic EHV‐1.  From the preliminary results of her research,  Dr Maxwell advises horses may be best 

d b i h h h d b d hprotected  by treating when they had been exposed to the virus but before they demonstrated fever or any other clinical signs. However, the recommended 2 week treatment will cost  approximately $2000/horse. 

Trisha decides to treat Mocha (11 yr old Arab/Standardbred), an international 100 mile endurance horse and her hope for the Canadian Equestrian Team for the 2010 World Equestrian Games.

At this time, Mocha is the only horse on the farm who has h li i l i f i f inot shown any clinical signs of EHV‐1 infection.

The dose is 27 mg/kg three times a day for 3 days, then 18 mg/kg twice a day for an additional 12 days. Trisha asks the WCVM pharmacist to get enough valacyclovir to treat Mocha.

Dr Steve Manning and Dr Kirsten Poirier follow Trisha back out to Zwaan Farm at 2 PM to examine the horses. Dr Lynne Sandemyer agrees to examine Cruise’s eye if he is brought to the WCVM.

The 3 veterinarians examine all the horses on the farm.

Porsche’s ataxia is worsening and she is graded 3/5 (1 is barely perceptible, 5 is recumbent).

Chino (15 yr old Arab⁄Saddlebred) and Anicawas (8 yr old Arabian) are also showing signs of ataxia. Chino is graded 1.5‐2/5. Anicawas is difficult to catch and handle, so he is only observed. Mondo, Cain and Mocha are assessed as normal.

Cruise is brought to the WCVM and examined by Dr Sandemyer inside the Zwaan’s trailer. The stress of the trailer ride increases Cruise’s ataxia significantly. Dr Sandemyer confirms the uveitis in the left eye.

By the evening of March 31st  and through the night, Porsche’s ataxia increases  (3.5/5). She comes close to falling several times, but appears to be able to urinate.

Mocha begins 3 time a day valacyclovir therapy.

All of the neurologically affected horses are treated with flunixin (1.1 mg/kg) and dexamethasone (0.2 mg/kg) daily. In addition, Porsche and Cruise are treated with 1 g/kg of DMSO once daily for 3 days, diluted in 5 liters of fluids (cost approximately $100/day). Trisha is able to maintain jugular catheters  and carry out the therapy in the barn from March 31‐April 2. 

12 hours after the first dose of dexamethasone, Cruise’s left eye appears normal.

April 1st: during the evening examination, Mondo (11 yr old Arab/QH gelding, owned  by Bert Sutherland) shows ataxia (Grade 1.5/5). With the permission of his owner, he is started on dexamethasone/flunixin therapy.

• April 2: final necropsy report on Reyna is filed

• Immunohistochemistry tests are negative for EHV‐1, rabies, and Neospora organisms.

• Despite the negative tests for EHV 1 the• Despite the negative tests for EHV‐1, the pathologist gives the etiology as suspected EHV‐1 based on the histopathological lesions.

April 3rd: during the morning examinations, Cain is noted to have blepharospasm and tearing of the left eye. As with Cruise, an ocular examination and fluorescein staining do not show corneal damage. Uveitis is suspected and Cain is treated with flunixin and topical atropine, but Trisha has run out of dexamethasone. 

Neurologically, the other horses appear stable.

By the evening of April 3rd, Cain’s left eye has worsened. The globe feels softer than normal on palpation. Cain is treated with 0.2 mg/kg dexamethasone along with flunixin and topical atropine.

The horses showing neurological signs have stabilized or improved slightly.

• April 4th: 12 hrs after receiving dexamethasone, Cain’s left eye appears normal. The other horses appear stable or improved. p

• The results of the pan‐Herpes PCR test become available– Reyna and Porsche are positive on both blood and nasal swabs. 

– Peguis, Cain, Chino are positive on nasal swabs.Peguis, Cain, Chino are positive on nasal swabs.– Cruise and Mocha are negative on nasal swabs.– Anikwacas was not swabbed.– Mondo’s results are not available due to a lab problem.

From April 4th to present: all affected horses continue to improve and their doses of dexamethasone are slowly being decreased.

Mocha continues to receive oral valacyclovir therapy.Blood samples are being collected  for Dr Maxwell to 

h bl dmeasure her blood concentrations of the drug.Through the assistance of Dr Cynthia Cole, a veterinary clinical pharmacologist with IDEXX Pharmaceuticals, additional samples from the phorses are being sent to Kentucky for testing with a PCR that is specific for EHV‐1.

• With very limited exposure, Peguis and Reyna contracted EHV‐1.– The first manifestation was colic and depression in Peguis.

– Reyna’s CNS signs were not consistent with myReyna s CNS signs were not consistent with my previous experience with neurological EHV‐1.

• 8/9 horses at Zwaan Farm have shown clinical signs of EHV‐1 infection. – Only 2 have had demonstrable fevers and uveitis was the presenting sign in 2.

• The 9th has been treated with valacyclovir. IThe 9 has been treated with valacyclovir. I cannot prove that the treatment has protected her, but her value is such that I needed to mitigate the risk of her developing clinical disease.

Despite 32 years as a horse owner and 20 years as a veterinarian, I did not recognize that our horses were infected with EHV‐1 until 17 days after the exposure.The affect of this virus on our family and farm has been devastating.
