การตรวจภาพถ่ายรังสี

• ต้องส่งตรวจอย่างน้อย 2 มุมมอง

• ต้องการดูตำแหน่งใด ควรให้ตำแหน่งนั้นอยู่ตรงกลางแผ่นฟิล์ม

• บาดเจ็บที่นิ้ว ส่งตรวจภาพถ่ายนิ้ว ไม่ใช่มือ!!!

ข้อควรระวัง

ถอดแหวน, กำไลข้อมือ, นาฬิกาข้อมือทุกครั้งเมื่อเกิดการบาดเจ็บ

Distal End Radius Fracture

Fracture ที่มักจะรักษาแบบไม่ผ่าตัด

• Simple extra-articular fracture

• Epiphyseal plate injury in children (Salter-Harris type I, II)

• Low-demand patient เช่น ผู้ป่วยสูงอายุ ผู้ป่วยแขนอัมพาต

• ที่เป็น closed fracture

Guideline of Management

• นัด F/U ทุกสัปดาห์ (อย่างน้อยใน 3 สัปดาห์แรก)

• แนะนำ active ROME ของนิ้วมือ ข้อศอก และไหล่ ตั้งแต่วันแรก

• ระยะเวลาการใส่ slab / cast 6-8 สัปดาห์

Common Pitfalls• ไม่มี 3 point fixation

• พัน EB / เฝือกแน่นเกินไป

• นัดผู้ป่วยระยะเวลาห่างเกินไป

• ไม่ได้แนะนำ active ROME ของนิ้วมือ ข้อศอก และไหล่ ทำให้เกิดอาการข้อติดแข็ง

• ระยะเวลาการใส่ slab / cast นานเกินไป (โดยทั่วไปไม่เกิน 8 สัปดาห์)

เหตุผลที่ไม่แนะนำให้ใส ่V-shape / U-shape Finger Splint

• มองไม่เห็นนิ้ว

• มีการกดทับทั้งด้าน dorsal และด้าน volar

Radial collateral ligament injury


• ส่ง x-ray นิ้วที่บาดเจ็บเพื่อดูว่ามี fracture หรือไม่

• ในรายที่เป็น ligament sprain ทำ buddy splint กับนิ้วข้างเคียง ด้านที่มี ligament injury

• Splint นาน 4-6 สัปดาห์ โดยให้ผู้ป่วยทำ active ROME ของนิ้วมือระหว่างใส่ splint

• ในรายที่ไม่สามารถ reduction ได้มักจะมีการฉีกขาดของ ligament , มี soft tissue เข้าไปขวางใน PIP joint หรือมี fracture ร่วมด้วย ควรปรึกษาแพทย์ orthopaedics

Take Home Message

• การรักษาการบาดเจ็บที่มือ เป็นการรักษา function

• การ immobilization นาน ทำให้มี stiffness

• Extra-articular fracture ส่วนใหญ่รักษาแบบไม่ผ่าตัดได้แต่ต้องนัดติดตามอาการอย่างใกล้ชิด
