    • MPCA interest continued
    • RevUP revised to meet logistical and funding requirements
    • Nine applications received
    • Six centers selected

  • Initial one day face-to-face session with team members from all centers
  • Intense 2-day site visit at each participating center
  • Direct coaching by MPCA staff
  • Joint conference and individual calls for training, coaching and sharing
  • Interim report out at Clinicians meeting
  • Final one day learning/sharing session with team members from all centers
