• Diagnosis by–Excluding cause of reactive thrombocytosis–Excluding other CMPDs

• Female:Male = 2:1• Mean age at diagnosis 60 years

• 50% asymptom

• Vasomotor symptom: thromboxane,

microvascular thrombosis–Headache, lightheadedness, syncope

–Atypical chest pain, acral paresthesia

–Livedo reticularis, erythromelalgia

–Transient visual disturbances

Clinical manifestation

• Thrombosis: common complication–Arterial site: stroke, TIA, retinal artery occlusion,

coronary ischemia digital ischemia

–Venous site:DVT, PE, hepatic or portal vein

• Hemorrhage: risk–Extreme thrombocytosis

–Use ASA > 325 mg/day

–Use NSAIDs

–Myelofibrosis: 4% follow 9.2 years

–Acute myeloid leukemia
