EXTRA–CURRICULAR CODE OF CONDUCT

We expect our athletes to be good citizens. Athletes are expected t be credible people both is the school setting and in public. If athletes choose not to carry themselves in this manner, they will not be a part of our Athletic Program at MHS.

Participation is a privilege not a right. Participation requires the acceptance of additional

conditions. In effect throughout the athlete’s entire high school

career.

In effect on and off the campus throughout the entire calendar year.

This includes weekends, summers and out of season. Online Signature indicates they are prepared to

Eligibility A student must be enrolled in and passing

twenty (25) hours of classroom work each week A student must have also passed twenty (25)

hours of classroom work the previous semester (does not apply to incoming freshmen)

Students are allowed to make up one class during summer school to become eligible for the first semester

Physical Education counts.

Absences Must be in attendance for 3 Hours to participate in

activities that day. No practice or play without this requirement Exceptions:  

A.) Death in family.

B.) School-sponsored field trip.

C.) Prior approval by principal or assistant principal.  Athletes will not be allowed to participate in activities

or athletics while serving a suspension.

INJURIES

Report ALL injuries We have a full time athletic trainer (Lauren) If the injury needs medical care and a medical

excuse is given to prevent practice or game participation, a written release from the doctor will be needed before athlete can resume participation.

TRANSPORTATION POLICY Athletes will ride to all athletic events on school

provided transportation. Exception may be made by the administration do to certain circumstances.

Coaches may require athletes to ride the bus back to the school.

If an athlete is allowed to ride home with their parents or legal guardian they must : Signed Bus Transportation Form Sign Athletic Contest Parent Sign-Out Sheet Other circumstances must be approved by Principal

ATHLETIC FEE POLICY

The fee is $50.00 for the first sport and $50.00 for the second. The maximum fee per family is $200.00. The fee must be paid before the first scheduled contest for the sport in which the athlete is participating.

REQUIRED FORMS CURRENT PHYSICAL - Physicals (including freshmen) must be

on file before an individual will be allowed to go out for a sport. Physicals are good for 13 months.

INSURANCE WAIVER - Athletes must show proof of insurance. Waiver form must be completed each year and must be on file prior to the athlete participating in sports. School insurance is available. (Yellow)

EMERGENCY INFORMATION AND PARENT CONSENT FORM - In the event that your son/daughter is injured. For emergency situations when you are not readily available.(RED)

IHSA STERIOD TEST POLICY - No student-athlete may participate in IHSA state series competition unless the student and student’s parent/guardian consent to random testing. (Blue/Yellow)

FORMS CONT. ATHLETIC PERMIT CARD – Cover items talked about and has

places for signature of parent and athlete stating you support and agree to follow Extra-Curricular Code of Conduct. (YELLOW)

CONCUSSION INFORMATION SHEET- Information packet that explains what a concussion is and symptoms of a concussion. It also includes signs to watch for and what you need to do if you think your son or daughter has a concussion. Also included in the packet is the return to play policy. Parents and athlete must sign and return the form. (Blue/Yellow)

PLAYER/PARENT/COACH This is what you should expect from every coach: 

- Philosophy of the coach- Positive Motivation- Constructive Criticism - Expectations- Locations and times of practices/contest- Discipline procedures- Fairness and communication

The following is what coaches should expect from parents: 

- Concerns expressed directly to the coach- Notification of any schedule conflicts in advance- Support of son/daughter in all related activities- Good Sportsmanship- Understanding that there are always two sides to every story- Let the coaches coach

The following are appropriate concerns to discuss with the coaches:- The treatment of your son/daughter- Ways to help your son/daughter improve- Concerns about your son’/daughters’ behavior- Concerns about academics- Guidance in off-season activities.

Issues not appropriate to discuss with the coaches: - Playing time of all team members- Team Strategy/Play Calling- Other student-athletes on the team

If a conference with a coach is needed.- 24 HOUR RULE- Call to set up an appointment to see the coach- If the coach cannot be reached, contact the AD’s office, who will

in turn set up a meeting- Son/Daughter must attend meeting - Respect must be given or meeting is over

If this meeting does not provide a satisfactory resolution - Call and set up an appointment with the Athletic Director

SCHOLARSHIPS Scholarship money is available and many MHS alums have received athletic

scholarships in the past. Facts About Athletic Scholarships

2% of High School athletes earn athletic scholarships. (less than 1% earn Division 1 Scholarships).

The average scholarship amount for a college athlete is $11,000. (Average Tuition + Room and Board per year in state = $18,155).

Register Online to Be eligible to Receive Scholarship Money www.eligibilitycenter.org NCAA CLEARINGHOUSE – Register at the end of Junior year because you must

have 6 semester grades before filing. ACT/SAT code 9999 to get results sent to the clearinghouse.

MOST IMPORTANT – YOU WILL ALWAYS BE MORE LIKELY TO GET SCHOLARSHIP MONEY FOR GOOD GRADES

BOOSTER CLUB Join and volunteer Athletic Hall of Fame Supplies/Equipment

Sound system, tents, spike machine, shooting machine, water boys, wrestling mat, track pull carts, tennis ball machine, sideline headphones, and over $6,000 in scholarships.

PHYSICALS Athletic Physicals will be offered at

Mascoutah High School on Thursday, August 6th from 6:30 – 8:00 pm

Physicals will cost $15 per athlete.
