Nahleh, MD, FACP FLASCO Board of Directors Member and Program Co-Vice Chairman Cancer Center Director and Department Chair Hematology-Oncology Cleveland Clinic Florida BIOGRAPHY: Dr. Nahleh is Chair of the Department of Hematology-Oncology and Director of the Cleveland Clinic Florida Maroone Cancer Center. Dr. Nahleh is a member of the American cancer Society (ASCO) Clinical Practice Guidelines Committee. She was previously Professor of

Medicine and Chief of the Division of Hematology-Oncology at the Texas Tech University Health Sciences Center and prior to that Associate Professor and Co-Leader of the Breast Cancer Multidisciplinary team at the Wayne State University-Barbara Ann Karmanos Cancer Institute.

Dr. Nahleh subspecializes in breast cancer and has special interests in cancer survivorship and disparities. She is a certified principal investigator (CPI) by the Academy of Clinical Research Professionals. Her research interests were funded by the National Cancer Institute (NCI), the Cancer Prevention Institute of Texas and other scientific agencies. She served as a principal investigator in the SWOG oncology cooperative group and has authored national protocols in breast cancer neoadjuvant therapy and male breast cancer. She served on the NCI Neoadjuvant and Metastatic Breast Committees. Dr. Nahleh has received numerous awards, including the Barbara Ann Karmanos Hero of Breast Cancer Award for Scientific Distinction, the TTUHSC Dean’s Excellence in Research Award as well as the Distinguished Chancellor’s Council Award for Research. She has authored and co-authored numerous articles, book chapters and books in the field of cancer and breast cancer research, and has presented at national and international cancer meetings. She serves on the editorial board of numerous medical journals. She has a double American board certification in hematology and oncology.

Great Strides Together Program Chairman Cheryl Habaue, MSN, RN, CCRP Nursing/Navigator Track Chairman, Florida Society of Clinical Oncology Clinical Research Nurse BIOGRAPHY: Cheryl Habaue received her BSN from Mount Mercy College in Cedar Rapids, Iowa and MSN from Chamberlain College of Nursing. She has been in research for the past 7 years working on studies related to all cancers including but not limited to Lung, Breast, GI, GYN, GBM, Leukemia, Multiple Myeloma, Lymphoma, Head and Neck, as well as Sarcomas and

Melanomas. Cheryl currently works on phase I-IV clinical trials, focusing on trial management, process improvement and project management.

Great Strides Together Program Vice Chairman Jessica McMillan, DNP, FNP, APRN-BC Breast Cancer Treatment and Prevention Program Mayo Clinic Florida BIOGRAPHY: Dr. McMillan has been a member of the Mayo Clinic staff since 1998, in multiple roles. Currently, she serves as an Advance Practice Provider in the Department of

Hematology/Oncology since 2010. She has also worked as a Clinical Nurse Educator and Charge Nurse/Team Leader in the surgical Intensive Care Unit. Dr. McMillan obtained her family Nurse Practitioner Certification from the University of North Florida in 2009, and went on to obtain her Doctorate of Nursing in Family Practice with a Sub-specialty in Palliative Medicine in 2015. She serves as a preceptor for physician assistant and nurse practitioner students at Mayo Clinic Florida School of Medicine. She also works as an Adjuvant Instructor for the University of North Florida where she inspires and mentors future nurses. She is very active with the Florida Society of Clinical Oncology and the Northeast Florida Advance Nurse Practitioner Association. Within the Division of Hematology/Oncology, she specializes in Gastrointestinal Malignancies but serves as a resource in all areas of solid tumors for her fellow colleagues. Her passion is patient care and education. She feels very fortunate to work with cancer patients where she able to utilize her skills and education to forge relationships for patients who face uncertain futures.

Moderator Mayra Lima, MSN, APRN, FNP-C FLASCO Great Strides Together Planning Member Breast Cancer Treatment and Prevention Program Sylvester Comprehensive Cancer Center University of Miami

BIOGRAPHY: Mayra Lima has been a Registered Nurse since 1985 (34 years). She has worked in many different roles such as ER, Ophthalmology, Surgery, Disease Case Management, and Director of Nursing. She accomplished this all the while continuing her education and completing her Bachelor's degree at FIU, followed by the Masters in Nursing Science and Family Nurse Practitioner with a Certificate in Gerontology by December of 2000. As a nurse practitioner, Ms. Lima has dedicated her work to oncology and solid tumors in GI and Research at University of Miami, Sylvester. Ms. Lima decided to dedicate herself to helping those with breast cancer once she became a survivor of breast cancer herself. She has worked with breast cancer patients for the past 13 +years. Recently she relocated from the Memorial Breast Cancer Center to the Sylvester Breast Cancer Center in Plantation. In her personal life, she loves to travel, arts & crafts, dancing, music and all within the context of her family. Ms. Lima’s survival of breast cancer propelled her to do half marathons. Since 2007 she has completed 16. Ms. Lima also has a love of knowledge; she is a recent alumna of the Cancer Genetics Risk Assessment Program.

Krishna Komanduri, MD Medical Director, Adult Stem Cell Transplant Program Professor of Medicine, Microbiology and Immunology Sylvester Comprehensive Cancer Center University of Miami BIOGRAPHY: Dr. Komanduri holds the Kalish Family Chair in Stem Cell Transplantation and is Professor of Medicine, Microbiology & Immunology and is the

Director of the Adult Stem Cell Transplant Program and Associate Director for Clinical Innovation at the Sylvester Comprehensive Cancer Center at the University of Miami Miller School of Medicine. He has been appointed inaugural Chair of the newly created Division of Transplantation and Cellular Therapy. Dr. Komanduri received his undergraduate education at MIT (1987), his MD at the University of Minnesota Medical School (1991) and trained at UCLA (in Internal Medicine) and UCSF (in Hematology/Oncology). Prior to moving to Miami in 2008, he was a faculty member at UCSF and at the University of Texas M.D. Anderson Cancer Center. His laboratory research is focused on studies of cancer immunology and has been widely published and supported by the NIH and cancer-related foundations. Dr. Komanduri served as the 2017-18 President of the American Society for Transplantation and Cellular Therapy (formerly the ASBMT). He serves as Co-Chair of the CIBMTR Working Committee on Infections and Immune Reconstitution and as a member of the Board of Directors of the National Marrow Donor Program (BeTheMatch) and as a member of the MIT NEWDIGS “think and do” tank on financing of novel curative therapies in oncology. He has also served as Chair of the Immunology and Host Defense Scientific Committee for the American Society of Hematology and as a clinical expert for the Institute for Clinical and Economic Review Policy Roundtable on CAR-T Therapies. Dr. Komanduri co-chaired the 2018 ASBMT consensus conference on grading and reporting of toxicities for CAR-T cell therapies and continues to serve as co-chair of the ASBMT Committee on Cellular Therapies. He has been the recipient of awards including election to the American Society for Clinical Investigation (in 2009).

Moderator Nancy Pitruzzello, DNP, APNP, CPAN Mayo Clinic Florida BIOGRAPHY: Dr. Nancy E. Pitruzzello APRN has been a member of the St. Luke’s- Mayo Clinic staff since 1981. Currently, she serves as an Advanced Practice Provider in the Department of Perioperative Medicine. She has been in this role since 2010. She has also served in numerous

Nursing roles such as staff nurse on various units and a Team leader in the perioperative department. Prior to nursing, she functioned as laboratory secretary, File clerk and then office insurance biller for Medicare and Medicaid.

Dr. Pitruzzello obtained her Family Nurse Practitioner Certification from the University of North Florida in 2009. She continued her education at the University of South Alabama and received her doctorate in Advance Practice in 2015. She serves as a preceptor for nurse practitioner students at Mayo Clinic Florida School of Medicine. Additionally, for the last nineteen years she has worked as an adjunct instructor for the University of North Florida where she motivates and mentors future nurses. She is the Treasurer and a Board Member for the North East Florida Advanced Nurse Practitioner Association and works closely with the Association of Perioperative Nurses.

Dr. Pitruzzello is passionate about education. She has mentored numerous students from a variety of National Nursing Educational Institutions. She frequently will be found giving in-services and presentations to local and state nursing organizations. One of her crowing moments was achieving the honor of Mentor of the year for the Florida Nurses Association. Dr. Pitruzzello is serious about making sure the next generations of nurses are ready for the coming challenges.

Pashtoon M. Kasi, MBBS, MD, MS Assistant Professor of Oncology University of Iowa BIOGRAPHY: Dr. Kasi is an Assistant Professor at the college of medicine and oncology at the University of Iowa with strong interest in gastrointestinal cancers, circulating tumor DNA testing (ctDNA – Liquid Biopsies), pharmacogenomics and individualized medicine.

Dr. Kasi did his internal medicine training at University of Pittsburgh, followed by hematology/oncology fellowship at Mayo Clinic, Rochester before joining as faculty at Mayo Clinic in Florida. In parallel, Dr. Kasi received a post-doctoral Master’s in Clinical and Translational Science from Mayo School of Graduate Medical Education. This has allowed him to continue ongoing research and trials while taking care of patients with GI cancers.

Ashton Ritter, PA-C Mayo Clinic, Florida

BIOGRAPHY: Miss Ritter joined Mayo Clinic upon graduation from Wake Forest School of Medicine in 2016. She has focused her clinical scope by working with the dynamic GI Oncology group – treating patients with all gastrointestinal malignancies. She continues to be involved in Advanced Practice Provider Hematology Oncology

education and serves as a preceptor to many Nurse Practitioners, Physician Assistants, and medical students.

BIOGRAPHY: Dr. Ana Rosa Espinosa is the Administrative Director of the Oncology Clinical Services at Memorial Cancer Institute. In her role she oversees the practice of all

nursing and ARNP’s. She is an exceptional Nursing Director with extensive education in nursing as wells as business development skills. She has been in the oncology field for most of her long nursing career, and is Oncology Certify. She has a Doctor of Nursing Practice from the University of Miami, a Master’s in Business from Nova Southeastern University and a Bachelors in Nursing from Barry University. She is a speaker, has published several nursing articles and has an interest in nursing research. She is promoter of higher education and supporter of nursing research and evidence-based practice. Prior to Joining Memorial Cancer Institute, she was the Nursing Director for the Outpatient Clinics at Sylvester Comprehensive Cancer Center. She is a member of the Florida Nursing Association and the Oncology Nursing Society. She is an active member in her community and is involved in many cancer events.

Gelenis Domingo, MD Memorial Healthcare System

BIOGRAPHY: Dr. Gelenis Domingo is a hematologist-oncologist at Memorial Cancer Institute. She treats patients with a variety of cancers, including hematological malignancies (such as lymphoma and leukemia), blood benign hematology diseases, solid organ tumors (including breast, lung, prostate, and colon cancer) and also cares for

patients with anemia in pregnancy. Dr. Domingo was drawn to the oncology field by the unique relationships that often develop between patients and their doctors, and the ongoing discoveries and opportunity in cancer treatment. She believes every patient has a story and makes it her priority to get to know her patients so she can offer the best guidance and support as they fight the disease. Dr. Domingo earned her medical degree at the University of South Florida College of Medicine in Tampa, Florida. She completed an internal medicine residency at Jackson Memorial Hospital and a hematology/oncology fellowship at the University of Illinois at Chicago.

Paola Izquierdo, FBC-APRN, BSN, RN Memorial Healthcare System

BIOGRAPHY: Paola Izquierdo is the APRN of Thoracic Tumors at Memorial Cancer System in Pembroke Pines, FL. She is a physician in Colombia and received her BSN/FNP Magna Cum Laude degree from Florida International University, FL. She has been working

in oncology for the past 5 years as a RN and now as an APRN. Paola has published an abstract for ASCO 2018 and also participated as a speaker on Best of ASCO® 2018.

BIOGRAPHY: Terry Gruchow, PA-C, joined Moffitt Cancer Center as an Advanced Practice Professional with Department of Internal and Hospital Medicine in 2006. A graduate of the University of Wisconsin and Lock Haven University (Pa), his clinical

responsibilities include the Moffitt Direct Referral Center (the Center’s Urgent Care clinic) and the Inpatient Wards. His other responsibilities include Administrator of the Moffitt PA Student Mentoring Program and the Moffitt Student Shadowing Program. He is Adjunct Faculty at Barry University and South University. A member of The American Academy of Physician Assistants, Association of Physician Assistants in Oncology and Society of Hospital Medicine, he is a Distinguished Fellow at the Florida Academy of Physician Assistants. Terry is also on the Moffitt APP Council, has served as the PA Liaison to the FLASCO Board of Directors, and is a member of the South University PA Program Advisory Board.

Asmita Mishra, MD Moffitt Cancer Center

BIOGRAPHY: Dr. Mishra’s clinical and research interests include targeted post-transplantation molecular therapies for myeloid disorders and tailored transplantation therapies for diseases of the elderly, such as myelodysplastic syndromes. She is board certified in Medical Oncology, Hematology, and Internal Medicine. Dr. Mishra completed

her fellowship at Moffitt Cancer Center in Hematology Oncology with a focus on transplantation for hematologic malignancies. Dr. Mishra spent her residency years at the University of Louisville and received her medical degree from St. George's University, Grenada, West Indies.

Marian Dam, DNP, APRN Moffitt Cancer Center

BIOGRAPHY: Marian Dam is an advanced practice registered nurse (APRN) with the Department of Blood and Marrow Transplant and Cellular Immunotherapy since 2016. Marian earned her Doctorate of Nursing Practice at

the University of South Florida. She completed additional clinical hours and course work in endocrinology, specifically management of Type 1 diabetes. Marian’s clinical interests focus on cancers and associated therapies’ impact on the endocrine system. She is also an adjunct faculty at the University of South Florida’s College of Nurse Practitioner Program and serves as a clinical mentor to nurse practitioner students. In addition, her passion is to continue to volunteer among the underserved health and homeless population with not for profit organizations.

Ana Botero, MD Memorial Healthcare System

BIOGRAPHY: Dr. Ana Botero is a board-certified doctor of radiation oncology. She completed her radiation oncology residency at Mallinckrodt Institute of Radiology at Washington University in St. Louis, where she served as chief resident in her final year. After her graduation, Dr. Botero joined the radiation oncology faculty and served as Director of the Radiation Oncology Department at Veterans Hospital of St. Louis. Dr.

Botero's experience in radiation oncology includes IMRT , Rapid Arc, IGRT, HDR brachytherapy treatment, intracranial stereotactic and SBRT utilizing CyberKnife and Gammaknife techniques Dr Botero’s experience also include Y90-Liver microsphere infusion, thyroid therapy and overall Radiopharmaceutics.

Kimberly McDuffie, APRN, FNP-BC Baptist Health South Florida

BIOGRAPHY: Mrs. Kimberly McDuffie is a seasoned Advanced Practice Register Nurse with the hematology oncology department at Miami Cancer Institute. In her role she assigns advanced practice providers to appropriate consults and addresses triage concerns regarding patients, ED, lab, imaging and hospital to hospital transfers. She has been in oncology for 50% of her nursing career. Prior to hematology oncology, radiation

oncology was her passion. She has a Masters degree in nursing education from Barry University, Masters Degree as advanced practice nurse practitioner; she has a Bachelors degree in nursing from Barry University. She is motivational in her approach allowing patients to cling to her for support during an extremely difficult chapter in their lives. She was selected by her peers for Nurse practitioner on the year at MCI in 2017 and nominated for a high ranking nursing award “Daisy Award”. Prior to joining MC I, she was employed for the Sylvester Cancer Center department in radiation oncology for almost a decade. She is a member of the American Nursing association, Florida Nursing association and Oncology Nursing Society.

BIOGRAPHY: Martin Dietrich, MD, PhD, is medical oncologist and geneticist. At the AdventHealth Cancer Institute, he is pioneering the precision medicine program and is involved in immunotherapy and targeted therapy trials. His specialties are malignancies of the chest. He also has a research interest in the biological properties

of cutaneous malignancies. A PhD in molecular genetics, personalized medicine and genetic testing has been a center piece of his career.

She received her Associate Nursing degree from St. Petersburg College, Bachelors in Nursing from Florida State University and a Master’s of Science in Nursing specializing in Oncology from the University of South Florida. BIOGRAPHY: Dr. Beth Faiman received a bachelor of science in nursing and registered nurse certification at Ursuline College in Pepper Pike, Ohio, completed in 1996. She received a master of science in nursing and certification as an adult nurse practitioner at Kent State University in Kent, Ohio in 2002 and a PhD in Clinical

Research and Nursing from Case Western Reserve University in 2014. Dr. Faiman holds an advanced oncology nurse certification through ONCC. She is currently employed as an Adult Nurse Practitioner in the department of Hematologic Oncology and Blood Disorders at the Cleveland Clinic in Cleveland, Ohio. She is adjunct faculty at Ursuline College, Case Western Reserve University and Kent State University. She received the 2012 Excellence in Medical Oncology and 2013 Commendation for Patient and Nursing Education Awards sponsored by The Oncology Nursing Society. Dr. Faiman is an active author, presenter, and educator on the topic of multiple myeloma, amyloidosis, plasma cell dyscrasias, general cancer diagnosis and treatment, as well as management of skeletal and other cancer complications. She is an appointed delegate on the International Myeloma Foundation Nurse Leadership Board. She is currently Editor-in-Chief of The Oncology Nurse APN/PA and on the editorial board of ASH Clinical News among others. She has edited several books and authored many chapters relating to diagnosis and management of multiple myeloma, blood disorders and has written numerous articles relating to the diagnosis and treatment of myeloma, palliation, and cancer symptom management.

A graduate of the University of Wisconsin and Lock Haven University (Pa), his clinical responsibilities include the Moffitt Direct Referral Center (the Center’s Urgent Care clinic) and the Inpatient Wards. Jessica McMillan, DNP, FNP, APRN-BC Breast Cancer Treatment and Prevention Program Mayo Clinic Florida BIOGRAPHY: Dr. McMillan has been a member of the Mayo Clinic staff since 1998, in multiple roles. Currently, she serves as an Advance Practice Provider in the Department of

Hematology/Oncology since 2010. She has also worked as a Clinical Nurse Educator and Charge Nurse/Team Leader in the surgical Intensive Care Unit. Dr. McMillan obtained her family Nurse Practitioner Certification from the University of North Florida in 2009, and went on to obtain her Doctorate of Nursing in Family Practice with a Sub-specialty in Palliative Medicine in 2015. She serves as a preceptor for physician assistant and nurse practitioner students at Mayo Clinic Florida School of Medicine. She also works as an Adjuvant Instructor for the University of North Florida where she inspires and mentors future nurses. She is very active with the Florida Society of Clinical Oncology and the Northeast Florida Advance Nurse Practitioner Association. Within the Division of Hematology/Oncology, she specializes in Gastrointestinal Malignancies but serves as a resource in all areas of solid tumors for her fellow colleagues. Her passion is patient care and education. She feels very fortunate to work with cancer patients where she able to utilize her skills and education to forge relationships for patients who face uncertain futures.

She received her BSN at West Chester University in West Chester, Pennsylvania and her MSN at La Salle University in Philadelphia, Pennsylvania. Lindsay has been an Oncology Nurse for 19 years with 11 of those years in the role of Nurse Practitioner. She has worked in a variety

of settings including inpatient and outpatient as well as community based and academic based. Also, she has worked in a variety of roles including Oncology Staff Nurse, Infusion Nurse, ARNP in a community practice, Pharmaceutical Sales Representative and Clinical Operations Manager of the Lab Draw and Infusion areas at Moffitt Cancer Center. Accreditation and Credit Statements In support of improving patient care, the Medical Educator Consortium is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
