Head injuries

A patient was admitted to the hospital after a road traffic accident.

After assessment of ABC, he was observed for head injury. Findings are:

Eye opening to pain, incomprehensible speech, flexion motor response,

Scalp contusion on right temporal region, right pupil is 5mm, left pupil is 3mm,

Left upper and lower limbs power is grad 3, blood pressure is 160/100mm hg, and pulse rate is 56/min.
