It is time to make your 2015 DaVita benefit elections!
This year we watched as the country adjusted to new healthcare reform laws and the idea of health care exchanges. Even after a year there are still a lot of questions and uncertainty about how healthcare will look in the future. This is why DaVita is pleased to say we will be staying the course, offering teammates the same insurance carriers and coverage levels for 2015.
As you begin preparing to make your 2015 elections, be sure to read this Enrollment Guide and learn about the few things that are changing, including updated 2015 premiums and new technology.
This year’s highlights include continuation of:• The same three Cigna plans, with no change to deductibles or out-of-pocket maximums.• The same Regional plans, including Kaiser, BCBS and Group Health.• DaVita’s Health Savings Account (HSA) funding of $600, $900 or $1,200 for HDHP participants.• DaVita’s Health Reimbursement Account (HRA) funding, up to $800 for Cigna PPO with HRA participants.• Free preventive generic medications through mail-order for Cigna PPO with HRA and HDHP participants.• NEW! DecisionDirect, a simplified decision tool that helps guide you toward the medical plans which best
meet your needs.

2014 Vitality Points If you haven’t earned them, there’s still time. Teammates and spouses/domestic partners have until Dec. 31, 2014, to earn Vitality Points and receive the base medical premiums, listed above, in 2015.
