New Information about the child/young person

In this area of the record you will record a summary of the new information that has emerged about the child/young person in each life area and consider the needs that the information raises. For example, in the area of health you would record any new health conditions diagnosed, any periods of hospitalisations, any concerns that require follow up, the general health of the child/young person since the last Care and Placement Plan or Review.

The summary of new information will partly come from the child/young person’s Essential Information Record and, if it has been completed, the child/young person’s Assessment and Action Record. It is important to check and update the child/young persons Essential Information Record before beginning the Review. It is also important to attach the Assessment and Action Record “Summary of work to be undertaken” to the Review document.

Case Progress Reports There are some children and young people in out of home care where the Department of Human Services contracts the implementation of the Statutory Case Plan to either the Placement Agency or to another agency or program – e.g. Intensive Case Management Service (ICMS). DHS requires Case

Last name/First nameProgress Reports on a regular basis for these children and young people. In addition to the Review of the Care and Placement Plan which must be completed for all children and young people in out of home care, the case planning chairperson may also require a Case Progress Report which includes:

A casework overview specifying the contact between the PACW and the child and family, and the child and his/her family. A case plan progress and evaluation report detailing the work that has occurred towards the case planning goals and including

recommendations regarding the ongoing appropriateness of the case planning goals. A risk assessment report detailing the ongoing risk and safety issues that would exist for the child/young person if they were to return home.

The decision to contract the implementation of the case plan for a child/young person is a case planning decision. In discussion with the CSO, the DHS case planning chairperson will specify:

The frequency with which Case Progress Reports are required The areas that need to be included in the Case Progress Report.

Transition Planning is required for children and young people where a decision has been made that the child/young person will leave the placement to return home, go to another placement or move to live independently. The Care and Placement Plan and Review Record and process can be used to develop Transition Planning.

Transition Planning for Young People Leaving Care to Live IndependentlyTransition Planning must describe how the young person will be assisted to develop the life skills to be able to live independently. It will include the support services the young person will need when they leave the placement, for as the young person sets out on their own they will need a range of people to support them.

Transition Planning in this context needs to start early and be regularly reviewed and up-dated. Young people need time to develop strong relationships with those who will be supporting and assisting them when they move to live independently. They need the opportunity to develop and strengthen these relationships from within their out of home care placement. They need time to become confident and familiar with the information they will need (how to sign a lease, how to get an electrician etc.) and the issues they will have to resolve (housing, income etc.).

Key information regarding children and young people in Out of Home careThe Review of the Care and Placement Plan Record asks you to record some key pieces of information about the child/young person. The purpose of asking you to record this information is: To inform your review of this particular child/young person’s needs and the way they are being met. To be used by your manager as a snapshot of how your agency is meeting some of the more significant needs of this child/young person. To gain a picture of the population of children and young people in out of home care both within your agency, in your region and across the state of
