Safety talks Topics:

1. Sleeping, urinating and eating outside designated premises are strictly prohibited. Anybody caught in the act of violation shall incur administrative fines and sanctions mandated by HSE department.2. Working at heights; standard hierarchy of controls, proper scaffolding dimensions and components, proper PPE Difference of Fall restraint and fall arrest systems, safety culture and mentality when working in a scaffolding3. Working under rigging lifting radius of the Crane; control measures while lifting overhead is under taken, safe rigging of various materials i.e. Blocks, steel, etc. 4. Near Miss reporting; safety awards and incentives5. Evacuation procedure and emergency response6. Roll Call and Introduction of staffs and their respective roles and responsibilities

Site Safety Observation

1. Housekeeping has been arranged by the PM and CM to be given under the foremans responsibility within his designated area. However, it is unfortunate that up to this point the foremen at site fail to deliver the standard of cleanliness required to facilitate safe working. Meeting was arranged and one of the purposes of which was discussing and arranging the system on which they will operate as far as proper housekeeping in the site is concerned. (see pictures attached)

Overflowing Sewage Tank

Our sewage tank is full again. It is overflowing which may cause a lot of detrimental factors concerning health and environment. Furthermore, it may reach the mobile cranes hydraulic components. We need to hire back up contractor to enable on-time collection of the waste water during time when the primary contractor cannot service.

Sub-standard Ladders

As of the moment, I have recalled all defective ladders we have in the site. Some of these ladder were actually just delivered few months ago. In this regard, it is quite evident that the ladders we are purchasing are sub-standard and non-compliant to quality requirement.

Most of the major injuries occurring in a construction site are working heights-related injuries i.e. fracture, mucsle contusions, damage to spinal cord, paralization and worse death. If this doesnt get sorted out, worse consequences may be faced by KELE as a whole.

At the moment since we have disposed a number of ladders, we are now scarce of this said equipment especially now that majority of works need height extensions.

Faisal, please place an MR, if there isnt, for staight ladders of various heights. 10 pieces each.Alsp raise a number of mobile and platform step ladders also in various lengths as shown in the picture.

Incident Report

We had an incident this afternoon 24/03/2015 at around 2:40PM when one KELE steel fixer, Muhammad Ednan ID number 20271 was lifted via FIRST AID man cage from level 2 down to ground level to be treated at the first aid facility. Accounts of the event are as follows.

1. I, HSE representative was notified that one guy fainted at level 2 and needed urgent attention. I proceeded to level 2 immediately.1. Whilst on the way, the site nurse was immediately notified and He responded swiftly.1. The worker laid on the floor and was feeling drowsy but still fully conscious as per the assessment of the nurse. He was not having heat stress according to him as reflected by his pulse rate. When other workers have been asked, they said he was having seizures and possibly came to the site already ill but did not notify his direct supervisors. 1. Site nurse assessment: Possible seizure; Syncope1. After full initial assessment has been made, he was evacuated and lifted via first aid cage in less than 3 minutes and was treated at the first aid cabin.1. Currently the site nurse is observing his recovery progress. (Since the time of evacuation up to this time 3:00PM)
