FLU CLINIC

Whitewood will be holding a flu vac-

cination clinic on Thursday, November

21, 2018 from 1-3. We will need a copy

of an insurance card or a Medicaid card

for billing purposes. For children with

Medicaid, Medicaid pays for the flu

shot so the family doesn’t pay anything.

If the family has insurance, the insur-

ance is billed $45.00. If the family has a

co-pay or deductible, the family may

receive a bill for the flu shot from SD

Department of Health in Pierre. If the

family has no insurance, there could be

a $20.00 fee. you have any questions,

please call our office at 269-2264.

*Each child was sent home with a pink

consent form that will need to be re-

turned to school to receive the vaccine.
