ProgramAccountable Care Organizations (ACOs) are groups of health care providers that work as a team to coordinate care for a group of patients, with the goals of providing high-quality, patient-centered care and reducing costs. The conference will focus on the role of care coordinators, case managers and navigators in the ACO through all phases of care delivery including after hospitalization. At the close of this activity, participants will be able to:
