1. Ons praktyk VRA NIE MEDIESE FONDS TARIEWE NIE.2. U moet self die kwotasie by u mediese fonds inhandig en die nodige magtiging kry.3. Dit is u verantwoordelikheid om by u mediese fonds uit te vind presies watter bedrag hulle teenoor elke

kode op die kwotasie gaan betaal.4. Enige bybetalings met betrekking tot die narkotiseur en hospitaal/kliniek het geensins iets met ons

rekening te make nie. Hulle funksioneer heeltemal op hul eie.5. ALLE GEMS en MOMENTUM HEALTH pasiënte word as PRIVAAT pasiënte hanteer by ons praktyk en

moet ALLE rekeninge self betaal en dan terugeis van die fonds.6. Alle konsultasies, opvolg konsultasies en lokale gevalle is dadelik betaalbaar en moet dan deur uself

teruggeëis word by u fonds.

1. Our practice DOES NOT CHARGE MEDICAL AID TARIFFS.2. You must provide your medical aid with the quotation and obtain the authorization yourself.3. It is your responsibility to find out from your medical aid which amount they will pay next to each code

on the quotation.4. Any co-payments regarding the anaesthesiologist and hospital/clinic has nothing to do with our

account. They function totally on their own. 5. All GEMS and MOMENTUM HEALTH patients are treated as PRIVATE patients at our practice and

must pay ALL our accounts and then claim back from the medical aid.6. All consultations, follow-up consultations and local procedures are payable immediately and then

claimed back from your medical aid.
