Dear day camp families! It is with great excitement that we welcome you to Summer Day Camp at the West Valley Family YMCA! Thank you so much for choosing the West Valley Family YMCA for your Summer Day Camp program. We have planned so many wonderful activities for your child that they are sure to want to come back year after year! Please help us welcome your child to the Best Summer Ever, so far! It is our goal to provide your child with a safe, positive and enriching day camp experience within the child care program. The YMCA is committed to providing a family-friendly environment. You are welcome to visit the program at any time to observe your child’s participation. Parents and other family members are encouraged to attend YMCA special events, volunteer, support the YMCA annual fundraising Campaign that raises monies for financial assistance and program expansion or join the a child care center Parent Advisory Committee, whose role is to evaluate the program and make recommendations for improvements and enhancements to the program. The highlight reel for our Summer Day Camp Program is available now at the Welcome Center and at all of our child care and after school program sites. The highlight reels provide you with a complete snapshot of our camp programs. The detailed weekly calendars will be available in late April. The weekly calendar is a great source for all the happenings of the week. Another great resource for you is the summer day camp program guide, which is also available at the Welcome Center and all of our child care sites. Our summer day camp program guide provides tons of logistical information, such as camp locations, dates, times, cost, FAQ’s, and additional highlights. As long as you have read both the parent handbook and the summer program guide, you will have all the information you need for day camp. This Handbook is your guide to the YMCA and our Licensed Summer Day Camp Program. It outlines our Summer Day Program operations, policies, procedures, safety and risk management, program activities, and communication. If you have a question that is not covered in this handbook or need clarity, please contact the child care program office, at 818 668 2611, or feel free to speak directly with your Site Director. About the West Valley Family YMCA Board of Managers The West Valley Family YMCA is governed by a Board of Managers, who provides oversight to local branch operations, protocols, and procedures. Through regular meetings, as well as an extensive committee structure, the Board monitors on-going operations, ensures the proper funding for the organization, and provides leadership to the long range planning process for the YMCA in the Los Angeles County area. The Volunteer / Staff Partnership The community-based, member-oriented ownership of the YMCA provides for a unique relationship between the staff hired to implement programs and those members participating. In many cases, the staff for a particular program has been involved as a member prior to their taking on a leadership position. Is the West Valley Family YMCA Child Care / Day Camp Program a Good Fit for My Child? Overview The West Valley YMCA’s Licensed School-Age Child Care Programs (and day camp programs) are group child care programs, with an average daily ratio of 1 qualified child care teacher for every 14 school-age children. Our license is issued by the California Department of Social Services Community Care Licensing Department, which approves, issues, maintains and regulates licensed care facilities (child care, family child care homes, senior care facilities, etc.). Under Title 22, we are not permitted to allow a child care teacher to supervise more than 14 children. At some sites, due to the way in which the program is set up, children may be cared for on a daily basis in smaller groups of 1 adult for every 12 or 13 children, however, this is not typical. What is group care? The West Valley YMCA follows the Personal Care Group (PCG) system established by the YMCA of Metropolitan Los Angeles. The PCG system is designed to keep your child safe and supervised at all times through continuous 15-minute headcounts, internal sign-in and out forms, absence verifications, travel logs and documentation. Each child in our program is assigned to a Personal Care Group, which, for the most part, they will remain in during the duration of the day, camp session or school-year. In their PCG, they will typically be assigned to one child care teacher who will provide the majority of their care for the day and ensure that their personal rights (License form 613A) are never violated. Typically, your child will engage in the day’s activities, meal and snack time, group time and team time with the child care teacher assigned as their personal care giver within their personal care groups. At the YMCA, we do not provide one-to-one adult-to-child services. We also do not provide care is groups smaller than 10-14 children --- typically your child’s PCG will consists of 12-14 children total. One-to-One Services Families with children who may require one-to-one services (such as families with children who have a one-to-one aide while the child is at school during the school day) must set-up an appointment with the Senior Program Director of Child Care to determine the level of the child’s needs and better understand the strategies and techniques needed to help the child be successful

in the Y program. The Y will do our best to meet the needs of the child but families need to understand that our program may not be the right fit for their child. The Y will provide resources to the families as appropriate. Take a Tour Before You Enroll First time enrollees / families who are interested in the YMCA Child Care Programs are STRONGLY encouraged to visit the center and observe the program PRIOR to registering. At the center, site staff will provide a tour, answer questions, and share about the program philosophy, general schedule of activities, staffing structure and qualifications, and the group-centered nature of the program, including teacher to child ratios. Please make an appointment by contacting the Child Care Center directly. We unfortunately cannot accept walk-ins for tours. Children MUST accompany parent(s) to the tour. We want to meet you and your child. The 48 Hour Rules for First Time Enrollees The West Valley YMCA has a “two (2) business days of 48 hours” policy for first-time enrollees. If your child is a first-time enrollee in our programs, the child may not start until we have received a complete registration packet, program fees have been paid and after two (2) business days of 48 hours has passed. This will give us time to review your registration packet. First time enrollees, please enroll in advance so that your registration is approved prior to the start of a program, session or school year. Please be sure to also schedule an appointment so that you can tour the facility and introduce your child to the center staff and director. Friday enrollments will be approved for a Tuesday start date and Monday enrollments will be approved for a Wednesday start date, and so on and so forth. Returning children and families may start right away and may also start a program on the same day that they re-register. This includes families who continuously attend only our day camp programs. Trail Period There is a THREE BUSINESS DAY trial period for all first time enrolled children once they start our program (before and / or after school program and / or day camp program). The trail period is for both you as the parent and for the YMCA to ensure that our program is able to make reasonable accommodations to meet the child’s needs and interests. The Y staff will work in

partnership with families to address any issues or concerns and help the child be successful in the program. If the Y is unable to meet the child’s needs or the child becomes a danger to himself or others, arrangements will need to be made for other care. After the three business day trail period, if the parent or the Y determines that our program is not the correct fit for your child and their individual needs, the Y will terminate child care services and parent will receive a full-refund of all fees paid (this type of refund is titled a “child care trail period refund”. If parent is choosing to terminate child care services, the parent must request the child care trail period refund on the child’s first, second or third day of care or thereafter, unless the child attends a fourth day of care, in which case, the child care trail period refund is expired and no longer applicable by request of the parent. On your child’s fourth day of attendance, all requests for a child care trail period refund will not be approved. The YMCA reserves the right to terminate child care services at any time if it is determined that the program is not a correct fit for the child. This reserved right by the YMCA is in effect within both the three business day trail period and any day afterwards. Child care trail period refunds are only guaranteed during the child’s first three days of attendance. If approved and if a child care trail period refund is issued, the entire family is disqualified from requesting a second child care trail period refund indefinitely, including if they re-enroll into our programs. Program Values and Goals With the right building blocks for healthy development, children gain the skills they need to be active, thriving and contributing members of society. To deliver on our commitment to nurture the potential of every child, the Y is more intentionally fostering Achievement, Relationships and Belonging through our Day Camp program. Our focus is the infusion of evidence-based activities proven to build dimensions of well-being, including: • Discovering skills to help children realize their

accomplishments, passions, talents and potential.

• Building friendships to demonstrate the role positive relationships play in the well-being of a child.

• Being a place to belong where kids feel safe, welcome and can express their individuality.

By incorporating social-emotional, cognitive and physical development activities into Day Camp, the Y is more holistically equipping children to realize their potential in school and life. Within the child care program, our Summer Day Camp experience will focus on six goals for each child. Each child will:

1. Gain confidence in individual abilities 2. Learn how to work together as a team 3. Develop leadership skills 4. Learn independence and self-reliance away

from home 5. Acquire an appreciation for nature 6. Have a chance to understand one’s self and

one’s values a little better Additional our child care programs goals are as follow:

1. To support and strengthen the family 2. Provide a safe, fun and nurturing

environment that teaches and builds positive character development

3. Support and encourage academic growth 4. Create meaningful opportunities for family

and community involvement 5. Improve health and fitness of school age

children and their families 6. Foster creativity, independent decision

making and personal growth Program Start and End Dates Our Summer Day Camp program is offered for 10 weeks. 10 weeks will be offered for LAUSD students and 9 weeks will be offered for LVUSD students. Parents are responsible to sign their child up for the correct week. Our LVUSD (Las Virgenes School District) program runs out of our Bay Laurel Elementary School YMCA. This program at Bay Laurel Elementary School is closed Week 1. LAUSD: Monday, June 8 – Friday, August 14, 2015 (Monday – Friday; 5 days) LVUSD: Monday, June 15 – Friday, August 14, 2015 (Monday – Friday; 5 days) Good Times Camp (formerly known as Teen Camp) We have revamped our Teen Camp program and it is now called Good Times Camp. Good Times Camp is designed for children entering grades 6-

9 in the fall. This program takes place at our Vanalden, Woodlake and Pomelo Sites. This program does not take place at our Bay Laurel Site. Teen Camp was for children entering grades 5-8. Please note that Good Times is not for children entering grades 5-8, but, rather for children entering grades 6-9. Children entering 5th grade should sign-up for our K-5 day camp. Bay Laurel: Bay Laurel Elementary School / LVUSD Summer Day Camp Program will only accept children entering grades K-5. This is the only site that will not accept children entering grades 6th – 9th. Financial Assistance The West Valley Family YMCA membership and programs are open to everyone who desires to participate. Financial assistance, to the extent possible, is made available through the generosity of our donors. Financial assistance is based on need. Applications for consideration are at the front desk and should be submitted as soon as possible, as awards are granted to qualifying families on a first come, first serve basis. We are only able to process complete applications, with all the required documentation. Incomplete applications will not be processed. We are required to keep and file all documents used in determining your eligibility for financial assistance, so please make copies prior to completing your application. The YMCA will not adjust fees if you enroll into the program before you are approved for financial assistance. If you intend to apply for financial assistance, you may not enroll until your financial aid application has been approved. The YMCA will not adjust fees. Not all applicants will receive financial assistance. Funds are limited. Please be sure to give us your e-mail when you apply for financial assistance – we will be able to send award notifications to you electronically. Multi-child Discount You will receive 10% off for the 2nd, 3rd and 4th child you sign-up for the same week of day camp (even if the program is different). The sibling discount is 10% of the cost of the lowest costing camp program in the same week and can be applied to no more than 3 additional children. All children must be on the same account and this discount cannot be applied to children on different accounts or in different households. Families who abuse the multi-child discount policy will lose this benefit.

One Discount Limit The Y offers multiple discounts, including staff discount (for employees of any Y), financial assistance (for members who qualify) and a multi-child discount (10% off the 2nd, 2rd and 4th child sign-up in the same week). Members and staff may only apply ONE discount and cannot combine discounts. It is highly recommended that members and staff request that the largest discount be applied. We will not combine discounts, even if you qualify for more than one type of discount. Program Fees: I understand that I can pay for the day camp program in one or more of the following ways 1. Upfront and in advance, using cash or credit

card, if space is available (walk-in). 2. Week-by-Week, by, on, or after the due date,

using cash or credit card, if space is available (walk-in).

3. By or on the due date, using cash or credit card, if you reserved a camp space (walk-in or online).

4. Through an automatic funds transfer (checking or savings account) or a recurring credit card or debit charge, on the due date, by reserving a camp space and completing a specialized day camp Payment Authorization form (walk-in)

Program Fees and Other Notes • When signing your child up for day camp, you

are required to pay a non-discountable and non-refundable $50 registration fee for each child that you are enrolling in the form of cash or credit. After paying the registration fee, you may register your child for camp weeks using one of the following options: (1) pay for the camp weeks upfront and in advance using cash or credit (2) reserve the camp weeks and walk-in payment to the branch by the due dates (one week in advance of each week) using cash or credit (3) reserve the camp weeks and set-up an auto-draft deduction from your checking / savings or credit card account to deduct for the week on each due date (one week in advance of each week) or (4) pay week-by-week without a reservation (space is not guaranteed). If the payment is not collected (due to insufficient funds or my failure to walk-in a payment), your child’s enrollment will be cancelled for the specified week and your reservation will be forfeited. You will be

subject to a $25 service charge for all returned items due to insufficient funds. You can re-enroll my child if space is available.

• TRANSFERS: Families may transfer from one week of camp to another week of camp by request at the welcome center. There is no charge to transfer funds from one week of camp to another. Funds can be transferred from one child to another at the request of the parent. Transfers are subject to space availability at the camp you are transferring to. We unfortunately cannot process partial transfers. Once the week of camp has begun, we will not transfer any funds for that week or any past weeks. Transfers can be done at our welcome center and must be requested, in-person, before the start of the camp week you are transferring out of.

• CREDITS: The YMCA will not issue credits for missed days of camp or my personal scheduling conflicts. The YMCA will not issue credits for missed days of camp due to the wait period for first time participants.

• REFUNDS: Parents may request a refund for day camp using the refund request form. There is a $25 refund fee for all approved refunds that will be deducted from the total refund amount approved. The $25 refund fee is per week and per camper. If you are requesting a refund for several weeks and it is approved, you will be assessed a $25 refund fee for each week and for each child. The refund request form is exactly what it states, a request. Completion of the refund request form does not guarantee a refund. The payment due date deadline is also the deadline to request a refund for each week. If you are requesting a refund for a week of camp and it is after the payment due date for that week, that refund will not approved under any circumstances. For example, the final payment for Week 1 is due June 1. June 1 is also the deadline to request a refund for Week 1, if you paid in advance.

• Summer Day Camp closes each day at a specific time. Late pick-ups will be subject to a charge of $1 per minute, beginning 60 seconds (1 minute) after the program closes. Unless noted, all YMCA child care and day camp program centers close at 6:00 pm. Fees must be paid to the YMCA before the child can return the next day.

• The YMCA does not prorate, issue credits or refunds for missed days of camp or late registration. The YMCA is not responsible for

personal schedule conflicts. The YMCA does not prorate for wait periods for first time participants.

• The YMCA will not accept deposits, partial payments, or split payments for Summer Day Camp. The YMCA will allow you to register after the payment due date by walk-in if space is available.

Other Guidelines • The YMCA reserves the right to combine or

cancel programs due to low enrollments. • The YMCA reserves the right to limit the age

or grade range for any program. Other Financial Policies • If your child is involuntarily removed from the

program (suspended or expelled) due to behavior problems, the YMCA will not issue a refund, credit or transfer of any kind for any fees paid.

• If your child is suspended from the program, you have 30-minutes to pick my child up from the YMCA program site, beginning once you are notified (or your designees) by the YMCA. For every 30 minutes, beginning at the 31st minute, and so forth, that parent is late in picking up their child, an additional day of suspension will be added to the child’s suspension, up to 3 additional days. Parents will not receive a refund, credit or transfer of any kind for any fees paid.

• For day camp, payment may be made in cash, credit or auto-draft from checking / savings account or charge to a debit or credit card. We unfortunately cannot accept checks or any kind.

• We unfortunately cannot accept and program fees over the phone. All payments must be made in-person at the branch. Child care sites cannot accept cash or check payments on site. You may complete a payment authorization form if you would like to have our credit card on file charged remotely for the program fees.

• There are no registration deadlines for Summer Day Camp. There are no penalty fees for late registration for Summer Day Camp. Registration is open until the camp is full.

• We apologize, but there is no online registration for Summer Day Camp. All registration must be done at the YMCA. A registration packet must be turned into the YMCA at the time of registration. If you are

currently enrolled in before and / or after school care through one of West Valley’s child care centers, you do not need to complete additional Summer Day Camp paperwork. If you reserve a space for day camp and do not set-up an auto-draft from your account, you can log onto our website and pay your balances online. Balances must be paid by the due date or your registration will be cancelled and your reservation forfeited. You can reenroll if space is available.

Payment Due-Date Schedule All payments and fees are due at the time of registration. Partial payments and deposits will not be accepted.

Important Notes: • Due to the short turnaround, we cannot

accept checks as a form of payment for Summer Day Camp. We will accept cash or credit only.

Late Pick-Up Fees / Late Drop Off: • A late pick-up fee of $1.00 per minute will be

charged beginning at 6:01 p.m. for all children picked up from the child care site late. All child care centers close at 6:00 p.m. The dollar-per-minute-fee is per child. In the event that a bus returns after 6:00 pm, parents will have a maximum of 20 minutes to sign-out their child, after which, a late pick-up fee of $1.00 per minute will be assessed. Parents / authorized adults are expected to be on-site when the bus returns late for sign-out.

• Late fees must be paid by cash or credit card at the West Valley YMCA main branch on the same day to ensure continued enrollment in the child care program. No payments will be taken at the child care locations. After making

your late fee payment, you will be given a receipt, which you must show to the child care staff the next day of program. All late pick-up charges must be paid before the child can return to program. Payments cannot be made to child care staff. Please contact the Child Care office if any child care staff accepts a cash late pick-up payment.

• Three late pick-ups within a rolling 30 days may result in termination of your child’s enrollment in the child care program. If you are terminated from the child care program due to a late pick-up termination, the YMCA will not issue any credits, refunds or transfers of any kind for any fees paid.

• If your child’s group has left for a trip and it is after designated departure time, you will be responsible for your child’s care and / or transportation for the day.

Payments on Site Cash program payments will not be accepted by any child care staff. This includes weekly program fees, late pick-up fees, etc. All payments must be made at the Front Desk of the West Valley Family YMCA. Under no circumstances does the YMCA allow cash program fees / payments to be made to a child care staff or at a child care site. Please inform the YMCA if a child care staff accepts or request a cash payment on site. Program Cancellations The YMCA reserves the right to cancel any program. If the YMCA cancels a program, all money will be refunded, unless transferred to another program as requested by the participant. We reserve the right to close the program due to low enrollment. We reserve the right to combine a program or site due to low enrollment. This policy relates only to the complete cancellation of a YMCA program and does not relate to our policies in regards to emergency closures. The YMCA reserves the right to cancel a planned field trip or activity if there are circumstances beyond our control, such as inclement weather, bus / transportation issues or issues with our planned venue or destination, etc. If the YMCA cannot provide an activity or field trip, as advertised, the YMCA will make accommodations to provide a substitute field trip or activity (whichever is most feasible). The YMCA reserves the right to change the schedule without notice. Although we try very hard not to change the

schedule without sufficient notice, it has and may happen. Parents should have their children at the child care site by 10:00 am each day. If you bring your child after 10:00 am, you risk missing the bus, if it is a planned field trip day or if a field trip was added to the schedule. The YMCA will not issue credits or refunds for late arrivals or for missing the bus. Children should be at the child care center by 9:00 am. The YMCA anticipates full enrollment for Summer Day Camp. Summer Day camp combines both field trips and / or special visitors, weekly.

Program Registration A registration packet is required for all program participants. Children will not be admitted into program without a registration packet. Registration packets need to be turned in at the time that you register. T-shirts Summer Camp shirts will be available on the first day of Camp. Your first shirt is included in your registration fee. Additional shirts may be purchased for $10.00 each, if available. For safety and identification purposes, T-shirts must be worn on Field Trip Days and are optional on all other days. If your child comes to the site on a field trip day without a T-shirt, you will be notified. You can either bring your child their Camp T-shirt or the YMCA can provide them with a new T-shirt which will be charged to your account. You must clear the T-shirt balance before the child can return to Camp the next day. If you do not want the YMCA to provide your child with a T-shirt you must either pick-up your child from Camp for the day or bring your child a Camp T-shirt. The YMCA does not have extra shirts to loan. Administering Medication If your child needs to be given medication while at the site, please follow these guidelines. Fill out the YMCA Medication Authorization Form and return it to your Director the Monday of your first day at program. All medication must be brought to the YMCA in its original packaging with dosage information. All medication requires a doctor’s note. We are unable to administer over the counter medicine (Tylenol, Cough Syrup, etc.) unless accompanied with a doctor’s prescription. Please check-in / out medication with your Site Director. It is important that you

give them enough medication for the entire week. Children may not bring over-the-counter medication or self-medicate themselves. Please contact Christopher Jefferson (Senior Program Director) if you need a Medication Authorization form or if your child has an allergy that requires an EpiPen. [email protected] Communication Stay informed by reading calendars, newsletters and other information distributed by the YMCA staff. Keep the YMCA informed about changes in your child’s life that may affect his / her behavior. Let us keep you informed of your child’s progress on both good and bad days. Feel free to make an appointment with your child’s Teacher, Site Director or both to discuss issues. Give us suggestions on effective means of working with your child. Help us praise your child for a job well done. Don’t feel alone, we are here to help! Parents are expected to behave respectfully when communicating with staff. Any inappropriate behavior, abuse or harassment will result in the suspension or cancellation of your child’s enrollment. If you believe something is unsatisfactory, please speak with your Site Director or a Program Director immediately. Please note that refunds will not be issued if your membership or your child’s enrollment is suspended or terminated. YMCA Staff All YMCA child care staff are certified in CPR and First Aid, are 18 years of age or older, and have completed Child Abuse Prevention and Positive Discipline Training. Most of our child care staff are graduates of the YMCA Child Care Training Program, which is a 20 hour training program on policies, procedures, safety, risk management, etc. and attend monthly cluster meetings and trainings for professional development. We ask that you work with our staff to resolve any issues. We believe in quality care and appreciate any questions, concerns, feedback and compliments. 99% of our “camp counselors” are child care teachers and have completed the required coursework to qualify as a school-age child care teacher. Our staff members are mandated to report all suspected child abuse. All Site Directors attend an additional training that includes topics such as: Administrative responsibilities Team Building Communication Staff Performance Management

All Site Directors are California Title 22 Center Director qualified and have completed all the college coursework required for their position. Ratios & Ages Our ratio for Summer Day Camp is 1 adult for every 14 children. Additionally, each child care center has two fully-qualified Site Directors on hand during hours of operation. Drop-Off / Pick-up Please arrive on time if the site has a field trip planned. Parents will be responsible for the care of their children if they miss the bus. The YMCA will not issue credits or refunds of any kind, so be sure to check the schedule, as it may change without notice. Parents are welcome to call the child care site as early as 7:00 am to ensure that the schedule for the day has not changed. Parents will be charged a late fee of $1 per minute if they are late after 6:00 pm. The late fee starts at 6:01pm. In addition, if your child’s group has left for a trip and it is after designated departure time, you will be responsible for your child’s care and / or transportation for the day. Parents who have not notified the Child care site that they will be late, can expect the following sequence of events to occur. These steps are necessary to ensure the safety of the child as well as YMCA staff members. • 6:00 pm: Child Care center closes.

• 6:15 pm: Staff member in charge begins calling parent work number(s) to check for problems or miscommunications. If contact is not made, alternative contacts listed on the registration form will be called.

• 6:45 pm: Staff member in charge contacts local authorities to determine if any problem related to the parent has been reported. The Program Director (or closest Director available) is contacted and appraised of the situation.

• 7:15 pm: If the child has not been picked up by this time and we have not received communication from the parent or an authorized adult, he or she will be turned over to the Sheriff’s Department (or local Police) and DCFS will be contacted.

Sign In / Out Your child will only be released to authorized individuals who appear on your registration paperwork. All individuals picking up children must be at least 18 years of age. Children may not sign themselves out of program and may not be signed out by a sibling under the age of 18. For safety purposes, we will require photo identification from all individuals prior to releasing the children (including from parents). Please notify your family and friends! We I.D. everyone! Parents, family, guest, etc. should always bring I.D. when picking up a child from the child care center. Always bring a photo I.D. Always sign-in and out on forms provided at checkpoints using your entire signature, including first and last name. Children may only be signed out by individuals who are 18 or older and who have been authorized by the parent / guardian. Authorization is noted on the registration packet. Anyone without proper authorization will be stopped from taking a child. If someone other than those persons authorized on the registration form will be picking up your child, you must notify the Site Director in writing. YMCA staff personnel are not permitted to sign out children. Please note that all legal guardians / parents have the right to pick-up their own child. If there is a domestic or custody dispute, the YMCA must be provided with legal documentation that restricts a guardian or parent from signing out a child. Without such documentation, the YMCA will honor all parents / guardians rights to pick-up their own child.

You should check the Sign-in / Sign-out area for schedule changes, which may have occurred. Weekly calendars are intended to provide children and parents with a detailed daily plan. Calendars will designate early departure and late return times when needed. In addition, calendars include information on special items that must be brought to the site for art projects and children performances. Calendars and field trips are subject to change without notice. Please make sure you have the most up-to date calendars, as they are subject to change without notice and may change over time. Please note: Summer Day camp combines both field trips and / or special visitors, weekly. Appropriate Dress Please send your child to program in closed-toe, closed heel tennis shoes each day. Children will return home dirty from their outdoor adventure. If your child is not dressed appropriately, you will be required to pick up your child or bring adequate clothing. Apply sunscreen to your child each morning to help prevent sunburns. For the safety of your child and our staff, the YMCA will NOT apply sunscreen to any child; however, we do have a very limited amount of sunscreen on site for your child to apply to him / herself (this is not a guarantee that the YMCA will also have sunscreen readily available – parents are responsible to send their child with sunscreen each day). Children must be trained on how to apply their own sunscreen. If you do not want sunscreen applied to your child, please request a waiver from your Site Director on your first day of program or indicate such in your registration packet. The YMCA will not be responsible for clothing and personal items that get damaged, lost, stolen or dirty at program. Children will typically return home dirty after playing and enjoying program activities. Please send children in clothing that is appropriate for the daily planned activities. More on Sunscreen Please apply sunscreen to your child in the morning and have your child pack a bottle of sunscreen for application throughout the day. We highly encourage children to: • Wear hats while outdoors; • Bring a water sipper and keep it full

throughout the day;

• Bring sunscreen and apply it to exposed parts hourly; and

• Wear a rash guard on swimming day

YMCA Staff are not allowed to apply sunscreen on your child. Please instruct your child that they will be responsible for reapplying their sunscreen throughout the day. While it is hard to avoid being out in the sun between 10:00 a.m. and 2:00 p.m., we do try to schedule groups to stay out of the sun during these hours as much as possible. We always alternate between indoor and outdoor activities. Please Send Daily Sack lunch, drink, snacks, sunscreen, hats, sunglasses and sweatshirts for cool mornings. All items must neatly fit in an inexpensive backpack. LABEL EVERYTHING. The YMCA is not responsible for broken, damaged, lost or stolen items. Personal items are brought at your own risk. In regards to Share Days, please do not send valuables. The YMCA Site does not have a microwave. Please do not send food that needs to be warmed or food that is intended to be eaten warm. The YMCA does not allow children to share food. The YMCA also requires that children bring healthy lunches and snacks with limited sweets. If the Child care site has a refrigerator, the children may utilize it for their lunches and drinks. The YMCA and our staff members are not responsible for lost backpacks, lunch pails / bags, or any other personal items brought to the child care center. Teachers and site staff are not responsible for holding money for children. Children are not permitted to bring valuables, electronics, cell phones, drugs, alcohol, medication, animals or weapons of any form to the YMCA. The YMCA stands for healthy living, so please do not send sugary drinks, soda pop, cookies, cakes, candy, goodies and other unhealthy food to program in your child’s lunch pail. The YMCA wants to encourage families to pack healthy lunch and substitute sugary treats with more healthy options. If your child needs to have a treat in their lunch pail, please limit it to one a day or (even better), one a week. Children may not bring electronic games, video game units, or electronic devices to the child care center, as these can be distracting, can be stolen or damaged, and can inhibit the YMCA’s overall program, which encourages children to be more active and social. Please have your child leave their cell phones at home. If children bring cell phones to program, they will be asked to leave them in their pocket or bag. Cell phones can easily be stolen or misplaced. You are more than welcome to contact your child directly at the site. We want to provide an environment that is positive and engaging for youth.

Electronic Devices, Game Systems and Cell Phones at Good Times Camp Good Times Campers are encouraged to leave their electronic devices and cellular phones at home or in their bag or pocket. Any electronic devices brought to Camp are brought at your own risk. The YMCA Good Times program hopes to create an environment that is engaging enough to keep pre-teens and teens off their devices. Parents, if you require your child to keep their cell phone on their person, please contact Good Times leadership. If you need to get in contact with your child, we encourage you to contact the Good Times camp leadership. Peanut Butter and Allergies Please inform your Site Director on the first day of program if your child has common or uncommon allergies. Snack menus are posted monthly. Please check the snack menu to ensure that your child can eat the snacks that are being prepared. Peanut Butter is a common allergy, and should be addressed with your Site Director on the first day of program. The YMCA cannot control what children bring for lunch. We will encourage parents not to send peanut butter, but we cannot guarantee that they will not. Our staff will make every effort to ensure tables and counter tops are safe and clean. Our YMCA staff is not trained in how to use an EpiPen. It is the responsibility of the parent to teach and train our child care staff on how to utilize your child’s EpiPen, in the event that it needs to be utilized. Snack A monthly snack menu is posted at each site. Our sites proudly offer snacks that are in line with the YMCA National HEPA Initiative (Healthy Eating and Physical Activity) standards. We provide both a morning and afternoon snack. Birthdays You must clear birthday celebrations with your Site Director. If you would like to have a birthday party for your child, make a request with your Site Director. Please be prepared to tell the Site Director what day and time you would like to have your birthday celebration and what food items will be served. Parents may not bring homemade cakes or food. Food items must be catered or prepackaged. It is our policy to not have more than one birthday a week. Speak to your Site Director, as some birthday celebrations can be combined into one day.

Children with Special Needs The YMCA child care staff strives to respond to the needs of each child in a group care setting, with a ratio of one staff member to 12 children. The YMCA provides opportunities for involvement in large and small groups with a balance of teacher-directed and child-initiated activities. The YMCA child care program is, however, unable to provide one-to-one care for any child except on an intermittent basis, such as injuries, immediate disciplinary issues, and certain personal care needs customarily provided to other children, or when a 1:1 has been provided from an outside agency, such as C.A.R.E.S. or CCRC. The YMCA child care program welcomes all children. To the extent it is reasonably able to do so, the YMCA child care program will provide services to children with disabilities or any special needs in the same manner as services provided for other children of comparable age. It is essential that all pertinent information about the child’s needs be available to staff from the outset of enrollment and that a continuing bond of trust and mutual partnership exists for the benefit of the child. Therefore, a parent has the obligation to disclose significant medical, physical, or behavioral issues at the time of the child’s enrollment and on an ongoing basis. If it is unclear whether the YMCA child care program can reasonably accommodate the unique needs of a child, we will arrive at a final decision by reviewing the circumstances on a case-by-case basis. Movie Policy Feature films and commercial movies will NOT be shown at program. Acceptable movies include home videos which are videos that include your child and their family (such as a child sharing a vacation video or birthday party video). The YMCA believes that children should be active and learn through play and creative outlets. Home videos are required to be pre-approved. Music Policy Music may be played on occasion. The YMCA will only play music that is positive and does not contain foul language. AM / FM radio stations are not permitted to be played at program with the exception of radio stations that are marketed for young children.

Ill Children If your child becomes ill at the YMCA, we will contact you immediately. You or an authorized individual will be required to pick-up your child immediately. Fever: If your child has a fever, he / she cannot attend care. If your child develops a fever at the YMCA, you will be called to pick up your child immediately. Communicable Diseases (i.e. chicken pox, measles): Please let us know if your child contracts a communicable disease. We are required by our health department license to inform other parents. Your child may come back when cleared by a doctor. For chicken pox, all of the spots must be dry. Lice: If your child is found to have lice, you will be called to pick up your child immediately, since they are highly transmittable. Before your child returns to the YMCA his / her hair needs to be free of lice, as well as the eggs. Please thoroughly clean wherever your child puts his / her head – car seats, sofas, towels, pillows, etc. to stop infestation. We will take precautions to control the spread at Camp. Injured Children If your child is injured, our staff is trained to take the appropriate steps to provide first aid. If an emergency occurs, the Site Director or child care staff will immediately contact you or your second emergency contact. If we are unable to reach any of your listed contacts, we will call the paramedics or an ambulance if necessary, to transport your child to the nearest hospital in the company of a YMCA staff member. The YMCA does not carry accident insurance for participants. Expenses incurred in the treatment of injuries will be the responsibility of the family’s own insurance carrier. All injuries will be documented. YMCA Child Abuse Prevention The YMCA maintains a policy of Child Abuse Prevention practices, which include procedures, related to: • Employee reference checking, hiring criteria,

and fingerprinting • Training and supervision requirements for

staff • Staff relationships with children • Unscheduled site visitation by YMCA

supervisory staff and Board of Manager volunteers

YMCA staff and volunteers are not allowed to baby-sit, tutor, socialize, or transport children at any time outside of the YMCA program. (YMCA will take immediate staff and volunteer disciplinary action if a violation occurs.) These policies are enacted to protect parents, children, and YMCA staff members from actual occurrences of child abuse as well as allegations of abuse. For more information contact your Site Director. In the event the YMCA discovers an alleged abuse of a child, the California Child Abuse Reporting Law Penal Code Sections 11165-11174.3., requires, by law, that if a child care service personnel (those responsible for the custodial care of a child) has reasonable cause to believe that a child has been abused, he / she must report that alleged abuse to the Child Abuse Hotline of the Department of Child and Family Services immediately. Your Child’s Registration Paperwork Your child’s file is available for review by the Department of Social Services at all times. Your child may also be interviewed by a state evaluator without prior parent permission. Positive Discipline Policy It is our intent that each child enjoys the activities planned by understanding that she / he is responsible for her / his actions. With prior knowledge of our basic rules of safety and good conduct, each child is made aware of how to exercise self-discipline. We are here to assist her / him and to know that we expect her / him to succeed. YMCA house rules and anti-bullying strategies will be reviewed weekly at the Child care site. We also use positive reinforcement by consistently acknowledging good behavior. The expectations listed below are the general expectations we have for all of our program participants:

• Respect for yourself, for others, and for property.

• Safety first. • Speak for yourself / listen attentively. • Be responsible for your words and actions.

Philosophy The YMCA strives to maintain a positive approach to managing children’s behavior at all times. “Discipline” is the process of teaching self-control

and the ability to live within limitations and agreed upon guidelines. The staff and children at each Child care site establish expected behavior guidelines. Positive behavior is self-rewarding and allows for program activities to run smoothly. When children choose to behave outside of the guidelines, some consequence is required to avoid future problems. The overall safety of all children in the program is our highest priority. Breaking Program Rules • If a child is not following the rules, he / she

will be reminded of the rules. • If the child continues to break the rules, he /

she will be asked to leave that area and find another area to play in.

• If the child still fails to follow the rules, the Teacher will assign an area for the child.

• If the behavior has not improved by this time, the child will be removed from the group and placed directly beside a Teacher or staff member.

Threatening the Safety of the Staff and / or Children in the Program • If a child threatens the safety or health of

anyone in our program by hitting, kicking, biting, pushing, spitting or any other dangerous act they will receive a verbal warning.

• If the behavior continues after the verbal warning has been given, the parent will be called to pick the child up within a half an hour. Credit will not be reimbursed for this day.

• After returning to program, if the child continues this behavior, the parents will be called to pick the child up within a half an hour and the child will be suspended for one (1) program day. Credit or refund will not be reimbursed for these days. A parent conference with the Director will be mandatory at this time.

• The next time the child is required to be picked up for inappropriate behavior, they will again need to be picked up within a half an hour and this time the child will be suspended for three (3) program days. Credit or refund will not be reimbursed for these days.

• Upon returning to program after a three day suspension and the child’s behavior continues, the parent will be called to pick their child up within a half an hour and the child will receive his / her final suspension of

one program week (5 camp days). Credit or refund will not be reimbursed for these days.

• After the final suspension, if the inappropriate behavior has not stopped, the parent will be called to pick the child up within half an hour and the child will be expelled from the program thereafter. There will not be any reimbursements of any kind.

• Drugs, alcohol, lighters, weapons, electronics, cell phones, animals (dead and alive), and other such items, are not allowed at the child care center / program.

There are some cases where a child will be sent home and / or suspended on the first violation. This is left to the Director’s discretion. The Director also reserves the judgment to suspend a child for additional days based on the severity of the incident and the behavior. Suspension from Program If your child is suspended from the program due to behavioral / discipline issues, you have exactly 30 minutes to pick-up your child from the program center; the child can be picked up by either a parent or a parent’s designee (designee listed on the enrollment form authorized for pick-up). Furthermore, an additional day of suspension will be added for every 30-minute period that the child is not picked—up from the site. An additional day of suspension will be added at the 31st minute and an additional day will be added every 30-minutes thereafter, for a maximum of up 3 additional days. Parents will not receive reimbursement, credit or refund for these missed days, including the missed days of the original suspension. It is important for parents to pick-up their child immediately if suspended due to discipline issues. Our goal is to provide your child with a stimulating environment that we keep them engaged in positive behavior. Unfortunately, some severe discipline issues can arise throughout the day that require immediate parent intervention and possibly a suspension from the program for any set amount of days. Enrollments • Please enroll early. Spaces are limited. • The YMCA has a waitlist. If program is full,

please call the front desk and request to be added to the waitlist.

• The YMCA does not prorate for program if you do not attend each day.

Emergency Closing Policy If the host school site or child care center closes at any time (this includes closing early) due to emergencies (power outage, lockdown, natural disaster, etc.), the YMCA program will not be provided. Parents need to listen to local TV and radio stations for closing announcements. Parents will be contacted and are required to pick up their children or make arrangements for an authorized adult to come and pick up their child in case of program closure due to emergencies. The YMCA does not provide refunds, transfers or credits for emergency closings. Field Trips Any planned field trips are not optional. There is no other care available. Field trips are subject to change without notice. Other walking field trips, park days and swim days may also be scheduled. Summer Day camp combines both field trips and / or special visitors, weekly. At times, due to traffic and unforeseen issues, the child care program may return back from field trip after the scheduled return time. It is our goal to keep every child safe and to always depart with every child accounted for. While traveling, the YMCA bus driver will, at all times, adhere to traffic laws. For the safety of all YMCA participants and staff, the YMCA bus driver will not rush back to the site. Please know that the YMCA will do its best to ensure that children are returned from field trips at or before the scheduled time. However, please note that late returns may happen. The YMCA leadership team will also do its best to ensure that someone is at the Child care site to communicate to parents when a field trip is returning late. Swimming Swimming and Non-Swimmers Facts about Swimming/Aquatics during Day Camp:

• Younger campers will enjoy time at the West Valley YMCA Splash Zone weekly.

• Older camps will enjoy time swimming at the West Valley Family YMCA Pool.

The Y’s policy requires that when campers are in the pool, they must be able to stand in the pool, with their feet flat on the bottom of the pool, with the water no higher than their armpits. Most of your younger campers simply do not meet their height requirements so we provide a splash zone for them to enjoy. Parents, if your child’s swim ability is at a high level and they are able to swim in the deep end of the pool, and you permit for them to swim in the deep end, they must pass a YMCA swim test prior to the start of day camp. All parents who have campers who desire to be swim tested, must be swim tested prior to the start of day camp. For your convenience, we have scheduled the following dates for day camper swim testing. Campers will be swim tested in the order that they arrive, so please arrive early. Please anticipate up to 40 – 60 minutes for swim testing, as we do anticipate a wait. Arrive early, if you can. SWIM TEST DATES & LOCATION Location: WEST VALLEY FAMILY YMCA POOL Check in with Swim Test Lifeguard 12:00 pm to 2:00 pm May 23 & May 30 June 6, 13 & 27 Please note that these policies are subject to change at the discretion of the West Valley Family YMCA with 30 days written notice. The policies and procedures in the handbook are in regards to child care and day camps taking place at our licensed child care locations. ACCEPTANCE OF POLICIES AND PROCEDURES By signing below, I acknowledge the following:

• I have received a copy of the 2015 Summer Day Camp Parent Handbook version(s) applicable to the day camp program(s) your child is registered. I

have read and understood the contents and information within the 2015 Summer Day Camp Parent Handbook version(s) that are applicable.

• When signing my child up for day camp, I am required to pay the $50 registration fee for each child I am enrolling in the form of cash or credit. After paying the registration fee, I may register my child for camp weeks using one of the following options: (1) pay for the camp weeks upfront and in advance using cash or credit (2) reserve the camp weeks and walk-in my payment to the branch by the due dates (one week in advance of each week) using cash or credit (3) reserve the camp weeks and set-up an auto-draft deduction from my checking / savings or credit card account to deduct for the week on each due date (one week in advance of each week) or (4) pay week-by-week without a reservation (space is not guaranteed). If the payment is not collected (due to insufficient funds or my failure to walk-in a payment), my child’s enrollment will be cancelled for the specified week and my reservation will be forfeited. I will be subject to a $25 service charge for all returned items due to insufficient funds. I can re-enroll my child if space is available.

• I understand the financial policy for Summer Day Camp 2015, which states as follows:

o TRANSFERS: Families may transfer from one week of camp to another week of camp by request at the welcome center. There is no charge to transfer

funds from one week of camp to another. Funds can be transferred from one child to another at the request of the parent. Transfers are subject to space availability at the camp you are transferring to. We unfortunately cannot process partial transfers. Once the week of camp has begun, we will not transfer any funds for that week or any past weeks. Transfers can be done at our welcome center and must be requested, in-person, before the start of the camp week you are transferring out of.

o CREDITS: The YMCA will not issue credits for missed days of camp or my personal scheduling conflicts. The YMCA will not issue credits for missed days of camp due to the wait period for first time participants.

o REFUNDS: Parents may request a refund for day camp using the refund request form. There is a $25 refund fee for all approved refunds that will be deducted from the total refund amount approved. The $25 refund fee is per week and per camper. If you are requesting a refund for several weeks and it is approved, you will be assessed a $25 refund fee for each week and for each child. The refund request form is exactly what it states, a request. Completion of the refund request form does not guarantee a refund. The payment due date deadline is also the deadline to request a refund for each week. If you are requesting a refund for a week of camp and it is after the payment due date for that week, that refund will not approved under any circumstances. For example, the final payment for Week 1 is due June 1. June 1 is also the deadline to request a refund for Week 1, if you paid in advance.

• I understand that Summer Day Camp closes each day at a specific time. If I pick up my child late, I will be charged a late pick-up fee of $1 per minute, beginning 60 seconds (1 minute) after the program closes. Unless noted, all YMCA child care and day camp program centers close at 6:00 pm.

• I understand that the YMCA does not prorate, issue credits or refunds for missed days of camp or late registration. The YMCA is not responsible for my personal

schedule conflicts. The YMCA does not prorate for wait periods for first time participants.

• I understand that the YMCA will not accept deposits, partial payments, or split payments for Summer Day Camp 2015. The YMCA will allow you to register after the payment due date by walk-in if space is available.

• The YMCA reserves the right to combine or cancel programs due to low enrollments.

• The YMCA reserves the right to limit the age or grade range for any program.

• I understand that if my child is involuntarily removed from the program (suspended or expelled) due to behavior problems, I will not receive a refund, credit or transfer of any kind for any fees paid.

• I understand that if my child is suspended from the program, I have 30-minutes to pick my child up from the YMCA program site, beginning once I am notified. I understand that for every 30 minutes, beginning at the 31st minute, and so forth, that I am late picking up my child, an additional day of suspension will be added to my child’s suspension, up to 3 additional days. I will not receive a refund, credit or transfer of any kind for any fees paid.

• I have read and understood the West Valley Family YMCA’s Trail Period policies and procedures listed on the child care welcome letter. I fully understand all procedures related to the child care trail period refund request.

• I understand that if I have applied for financial assistance, I must wait until my application is approved before enrolling in any day camp programs. If I enroll before my application for financial assistance is approved, the YMCA cannot go back and adjust / reduce fees for camp.

• I understand that I can pay for the day camp program in one or more of the following ways

o Upfront and in advance, using cash or credit card, if space is available (walk-in). o Week-by-Week, by, on, or after the due date, using cash or credit card, if space is available (walk-in). o By or on the due date, using cash or credit card, if you reserved a camp space (walk-in or online). o Through an automatic funds transfer (checking or savings account) or a recurring credit card or debit charge, on the due date, by reserving a camp space

and completing a specialized day camp Payment Authorization form (walk-in)

• I can pay day camp balances online if I reserved a space, via walk-in, in advance.

WEST VALLEY FAMILY YMCA ADMISSIONS AGREEMENT

ADMISSIONS AGREEMENT – Page 1 of 2 ACKNOWLEDGEMENT / AGREEMENT – As the parent or legal guardian of the above named child I understand agree to and / or acknowledge the following:

A. I acknowledge that I have received a copy of the YMCA 2015 Summer Day Camp Handbook and will comply with the policies set forth. I further acknowledge that I have received copies of the following documents required by the State of California Community Care Licensing: “Parents Right (LIC 995)”, “Personal Rights (LIC 613A)”, “Parent Handbook”, “Fee Schedule Page”, “Caregiver Background Check Process (LIC 995E) and “Acknowledgment of Receipt of Licensing Reports”. Some of these documents are located in this registration packet, while others are located in the Parent Handbook.

B. Field trips, either by walking or in YMCA vehicles or chartered buses are a part of the Child Care program

activities. No additional permission slips will be required.

C. Authorization for the YMCA to take photographs, videos, motion pictures and /or sound recordings of the child care participant of members of the participant’s family. I further grant the YMCA permission to use the photographs video, motion pictures or sound recordings in its general publicity materials. See attached waiver.

D. The YMCA staff and volunteers are not allowed to babysit or transport children at any time outside of the YMCA program (The YMCA will take immediate staff and volunteer disciplinary action if a violation occurs).

E. That I am not allowed to leave my child at the YMCA program center unless a YMCA staff is there to receive and supervise my child.

F. That should a person arrive to pick up my child who appears to be under the influence of drugs or alcohol, for the child’s safety, staff may have no recourse but to contact the police. (Please do not put staff in a position where they have to make this judgment call.)

G. That the YMCA is mandated by state law to report any suspected child abuse or neglect to the appropriate authorities for investigation.

H. That per Department of Social Services, Community Care Licensing, Title 22 regulations, and my child’s file is available for review by the Department of Social Services and representatives from these agencies may interview my child without prior parental / guardian permission. In addition Law Enforcement personnel may request the information listed in your file and may interview your child if necessary.

I. That if my child is suspended from the program due to behavioral / discipline issues, I have exactly 30 minutes for my child to be picked-up from the center by either myself, a 2nd parent or guardian or an authorized representative. Furthermore, an additional day of suspension will be added to my child’s behavioral contract for every 30-minute period that I am late in picking up my child. An additional day of suspension will be added at the 31st minute and an additional day will be added every 30-minutes thereafter, for a maximum of up 3 additional days. I will not receive reimbursement, credit or refund for these missed days, including the missed days of the original suspension.

J. That the YMCA may terminate my child’s enrollment for any of the following reasons: • Emergency names and phone numbers are incorrect. • Parent is late picking up child after program center closes. • Non-payment, late payment or ATS reject of fees. • Failure to adhere to the sign in and out policy. • Failure to notify the YMCA that the child is absent. • Child leaving the program center without authorized permission.

• Behavior that is continually disruptive or dangerous to others and / or self. • Behavior that is disruptive to property and / or refusal to replace said property. • Any single incident that is deemed by the program center director to be dangerous, harmful or disruptive. • Harassment, violent behavior or threat of such behaviors against a staff person or other member by

parent / guardian or persons associated to the child (family member, family friend, etc.).

K. That Program participation requires a YMCA school based membership in good standing and that non-payment of membership fees will result in my child not being allowed to participate in the program and could result in legal referral with the additional costs to myself. I further understand there is an administrative processing fee for any payment returned by my bank or credit account.

L. The YMCA and the staff employed by the YMCA will not become involved in custodial disputes between

parent / guardian. If YMCA documents are requested, the court must request them. The staff’s responsibility is to provide a safe environment for children.

M. I understand I am required to give 15 days’ written notice when terminating my child from YMCA child care programs. If 15 days is not provided I will not receive a refund or credit and I will be billed or charged as regularly scheduled. Registration Fees are non-refundable.

N. Pick-Up of Child from Child Care Center: I understand and agree that in the event my child is not called

for by closing, the Child Care Center will immediately make every attempt to contact me, or other persons authorized by me to take my child from the center. If I or my authorized individual cannot be located, or if satisfactory arrangements for calling for the child cannot be made, that the office will continue to try to locate me or another designed person; but if those efforts are unsuccessful within a reasonable period of time (one hour), the appropriate law enforcement and welfare authorities will be contacted, and my child may be released to the custody of those welfare authorities as a possible ward of the Juvenile Court. I hereby release the YMCA, its employees and agents, from all liability for any damage sustained by my child or by me which results directly or indirectly from the procedure outline above. I further agree that if this procedure must be utilized because I fail to pick-up my child on time from the center, or because any person designated by me fails to pick up my child on time from the center, I will indemnify and hold the YMCA Child Care Center harmless for any damage sustained by my child. I hereby release the YMCA, its employees and agents from all liability and responsibility for any damaged sustained y my child after closing time. I understand and agree that if my child is not called for at the designated time, my child and I may be dropped from the program. I have read the foregoing acknowledgement and release. My signature below indicates that I understand and agree to it.

O. My child, listed below has my permission to participate in all activities which are a part of the YMCA Child Care

Program. These activities include water play, use of the playground and apparatus, woodworking, cooking, animal care, messy arts and crafts, adult supervised walks in the immediate neighborhood / community of the YMCA Child Care Center, field trips and other active play experiences typical in a Child Care Program. I, the undersigned, hereby release and discharge the YMCA, officers, employees, agents and servants (herein, collectively referred to as the YMCA). For the purpose of this agreement, liability means all claims, demands, losses, courses of action, suits or judgments of any kind that I, my heir, executors, administrators or assignees may have again the YMCA because of any loss or damage to property that results from any cause other than the negligence of the YMCA. I have no objection to my child being included in photographs, slides or movies taken in the YMCA which might be used for the purposes of interpreting the Child Care Program. I understand that college students may be making observations at the YMCA as a part of class assignments and that any photography or observation will be done only under the supervision of the director. I understand that the discipline methods use with my child shall include warnings, redirection, denial of privileges and logical consequences. Continued behavioral issues may warrant a suspension from the program, as outlined in our Parent Handbook (see Positive Discipline section). I have read, understand and agree to the stipulations and policies in the child care registration packet (also referred to as a day camp registration packet) and policy handbook and realize that abuse of the policies may lead to the termination of my child’s enrollment.
