Following the trend of low birth rate and an aging population, Japan is faced with a growing need for

persons to provide care for the elderly.More and more foreign residents in Kanagawa Prefecture

have been actively working as caregivers.We would like to introduce care-giving work for the elderly so that you can obtain greater skill and experience, as an

1 What is care-giving for the elderly?

(1) Work Description Caregiving is taking care of elderly persons who are physically limited and unable to perform daily living activities on their own. The caregiver provides assistance for movement, using the toilet, bathing, having meals, etc., while allowing the person to do as much as they can by themselves.

(2) Workplaces

① Residential Care CenterResidential care centers are facilities such as nursing homes where elderly persons move out of their own homes and move into to live at the center. Work at such a facility may include night shift. Most facilities employ a shift system.(Examples of facilities)Facilities which provide long-term care to the elderly covered by public fi nancial aid (special nursing home for the elderly = Tokuyo): Assistance in bathing, toileting, eating and other daily living activities, functional training, health management and medical care is provided. Long-Term Care Health Facility covered by public fi nancial aid (Roken): Facilities which provide care especially for patients in stable conditions, in order that they can return home and lead independent lives. The emphasis in their care is rehabilitation, which is accompanied by management under staff and doctor supervision.Communal Daily Long-Term Care for Patients with Dementia (Group Home): Facilities where elderly patients with dementia receive care for daily activities in a family-like atmosphere, while living a communal life.

Movement Assistance Meal Assistance

② Day Service CenterFacilities which provide care for persons who live in their own homes, who come to visit and use the facility during the day. Users of these facilities spend time in rehabilitation and recreational activities at the center, and return home in the evening.(Examples of facilities)Day Service Center (Outpatient Long-Term Care): Facilities which provide assistance for elderly patients in bathing, meals, recreation and other kinds of service. Day Care Center (Outpatient Rehabilitation): Facilities, such as medical institutions (hospitals) and long-term care health facilities, which provide assistance in bathing, meals, rehabilitation and other kinds of services.

③ Home of the Patient (Home-Visit Nursing Care)Home-visit nursing care is for patients who live in their own homes.Physical care (assistance in movement, meals, bathing, toileting, etc.), living assistance (supporting household chores including cooking, washing, cleaning, etc.), and other kinds of care are provided. A second grade Home Helper License or above is required to work at a home-visit nursing care service provider.

Certified Care Worker“Kaigo Fukushi-shi”

<Practical Training>

Authorized Care Worker“Nintei Kaigo Fukushi-shi”

(tentative name)

Person who has completed beginning training

(Equivalent to Grade 2 Home Helper training)

Care-giving allows you to use your license to the most and advance your career. After certification for a second grade Home Helper license, you can continue on to specialized studies and obtain a license as a certified care worker. Your salary will increase along with qualification allowance, as you work with the licenses such as the Certified Care Worker. Also, your opportunities for work will also increase.

* As of 2013, the second grade Home Helper license will be replaced by the “Basic Training Course for Care Workers”. However, the “Second Grade Home Helper” license obtained prior to revision will still be valid (Equivalent to Grade 2 Home Helper training)

2 Proper workplace etiquette

(1) Dress and Grooming Since this work involves interaction with other people, acceptable grooming which does not offend others is important.

◦� Clothing at the Workplace: Choose clothes that are clean, allow easy movement, and not too colorful. ◦� Makeup: Do not wear excessive makeup, and try to look natural.◦� Hair-style: If your hair is shoulder-length or more, tie it up in

a bun. Do not wear large hair ornaments or clips.◦� Accessories: Accessories are dangerous at work. Remove them

while working.◦� Nails: Fingernails should be cut short. Do not wear fingernail

polish.◦� Other: Do not wear perfume, as it may offend some patients.

(2) As a Member of the Organization◦� “Starting Time” is the time when work begins. You must leave for work early enough

to arrive and prepare for “Starting Time” in advance. ◦� Tardiness and unauthorized absences cause problems for your workmates. If you

are going to arrive late or be absent due to illness, call your workplace as soon as possible in advance. ◦� “Report (HOUKOKU)”, “Inform (RENRAKU)” and “Consult (SOUDAN)” (HO-

REN-SO) to your boss are important.◦� Do not release patient, family or company information to unauthorized persons. ◦� Receiving money or gifts from the patient or other sources is not permitted.

・ I am happy because the patients often express their appreciation for what I

do, and I feel like I am really helping others.

・ My Japanese has improved since I can talk with the patients a lot.

・�At�first,�I�couldn’t�stand�helping�patients�with�meals�and�using�the�toilet.�So�other workers at the facility let me try it little by little. As I became more proficient,�I�could�gain�more�confidence�in�myself.�Then,�it�didn’t�bother�me at all.

・�Some�staff�members�at�the�workplace�kindly�point�out�areas�which�I�needed�to�improve,�while�others�would�never�help�me�out�with�my�duties.�So,�each�time I start working at a new place, I always tell my new workmates “I started�working�here�today,”�and�if�I�don’t�understand�something,�I�try�to�approach and ask other staff members.

・ I had a personal problem with a Japanese workmate. I patiently did what she told me to the best I could. As a result, not only she, but also other workmates accepted me at the end.

Q: Do you have any advice or messages for someone who wants to work as a caregiver?

(According to a series of interview taken by foreign caregivers between January to May, 2012)

・ Basic communication skills are important. Make sure that you are at the same eye level with the patient, and maintain eye contact when you talk with him or her. In this way, you can gain the trust of the patient.

・ Wherever we are from, we can enjoy a friendship with anybody as fellow humans.�People� feel� good�when� they� are� treated�well.�At�first� they�may�reject your help, but as you treat them in a kind manner, they will eventually accept you.

・ I recommend seriously to study Japanese, and remembering polite expressions.�Sincere�apologies�and�expressions�of�gratitude�are�essential�in certain situations.

・ I have heard that backaches are common among caregivers. But you can avoid them by some preventive knowledge.

・ As you gain experience, you can later choose jobs which better meet your wants�and�requirements.�Try�hard�to�study�despite�difficulties.

・ I think anybody who likes elderly people can continue working in a care-giving job.

・�Some� hesitate� to� take� this� job� because� of� a� lack� of� confidence� in� their�Japanese speaking skills. But you can make up for it with diligence and hard�work.�The�most�important�thing�is�sincerity.

・ Considering how you would like to be treated when you get old can help you be successful at this job.

second grade Home Helper license training course for foreigners and Japanese language courses necessary for a care-giving job. It also provides employment consultation and an introduction service for persons who would like to work as caregivers. Consultations available by appointment.

“INFO KANAGAWA”　 Information from the Japanese government and Kanagawa

Prefecture and practical information for daily living, such as education, welfare, sanitation, disaster prevention, etc., are delivered about four times a month through this service. Notification of the “second grade Home Helper license training course” or “Career fair of care-giving jobs” is also provided. The service is free of charge.
