THE FUTURE OF HEALTH CARE

Page 3: HCM114 - The future and the theories of Healthcare Management

There is a need to deal with the very rapid change in health care delivery through better management of scarce resources.

The Need For Better Management in Health

Services Delivery

Page 4: HCM114 - The future and the theories of Healthcare Management

Most of health care facilities worldwide are attempting to deal with this change through better management control.

The Need For Better Management in Health

Services Delivery

Page 5: HCM114 - The future and the theories of Healthcare Management

This requires greater empowerment of the health

care worker.

Empowerment can be economic, social, educational,

gender-wise, psychological strength, motivational of target individuals and communities.

Services Delivery

Page 7: HCM114 - The future and the theories of Healthcare Management

Costs for health care insurance are generally exploding while

outcomes for health care interventions have been

decreasing in many countries.

NEED TO FOCUS ON HEALTH CARE OUTCOMES

Page 8: HCM114 - The future and the theories of Healthcare Management

This is mainly caused by serious infrastructure problems in health care delivery that has increased costs while allowing the quality of health care to

diminish.

Efficient production of goods or services has become a very

important function of management.

EMPHASIS ON EFFICIENCY OF SERVICES DELIVERY

Page 11: HCM114 - The future and the theories of Healthcare Management

This emphasis on efficiency has now become a critical concern of those who manage health

care services. (more or less depending on the country)

EMPHASIS ON EFFICIENCY OF SERVICES DELIVERY

Page 12: HCM114 - The future and the theories of Healthcare Management

The question becomes who determines what is important in health services delivery?

DECISION MAKING?

Page 13: HCM114 - The future and the theories of Healthcare Management

The tremendous waste in our current health services delivery systems can only be eliminated

through better management

Building a New Process for health Services Delivery

Page 14: HCM114 - The future and the theories of Healthcare Management

The most important part of eliminating waste in any organization involves the

empowerment of its workers

The secret to success in managing health care facilities

involves listening to your employees

Example: The process audit proposed by Michael Hammer

is started by managers and supported by leaders in the

organization

REENGINEERING HEALTH CARE

>> DISCUSSIONS FORUMS

Page 17: HCM114 - The future and the theories of Healthcare Management

The audit is going to require an evaluation of the process of

delivering health care from the time the patient enters the

facility until they leave.

REENGINEERING HEALTH CARE

Page 18: HCM114 - The future and the theories of Healthcare Management

The audit will also require communication among all team

members with the ultimate goal of improved outcomes for

the patient

DECISION MAKING?

Page 19: HCM114 - The future and the theories of Healthcare Management

The health care sector needs to look at other sectors that have been successful in improving

productivity.

EMPHASIS ON PRODUCTIVITY

Page 20: HCM114 - The future and the theories of Healthcare Management

Imitation needs to be viewed as a driver of innovation not as a force that impedes innovation. Productivity gains in health care are not realized by innovation but by the improvements that result from the innovation. - Shenkar 2010 -

EMPHASIS ON PRODUCTIVITY

Page 21: HCM114 - The future and the theories of Healthcare Management

One of the most important skills required of the new health care manager is to learn what to prioritize on a daily basis. This seems like a very simple task but it can become very complicated and demanding.

THE NEED TO PRIORITIZE

Page 22: HCM114 - The future and the theories of Healthcare Management

Example: Many health care managers spend too much time going to meetings and too little

time being with the patients (Customers).

PATIENTS ARE THE PRIORITY!

THE NEED TO PRIORITIZE

Page 23: HCM114 - The future and the theories of Healthcare Management

Do we need a legislation reform? Why?

If a reform is adopted, it is going to cause massive changes in the

health care delivery system in the country. And in order to respond to this health care organizations need to change the way they do business.

HEALTH CARE REFORM LEGISLATION

Page 24: HCM114 - The future and the theories of Healthcare Management

To prepare better managers for the difficult challenges faced in the

evolving health care sector we need to understand past & current

theories of management.

A management theory: is a collection of observations about how the process of

managing people and things occurs in our world.

MANAGEMENT THEORY

• Scientific management• Bureaucratic management• Human relations Theory• Reengineering Theory

Popular Management Theories

Page 26: HCM114 - The future and the theories of Healthcare Management

The scientific management theory was developed by

Frederick W Taylor - the father of scientific

management-

Scientific Management Theory

Page 27: HCM114 - The future and the theories of Healthcare Management

SMT focusses on the study of the process of work in an attempt to improve output from that work. The goal of this process is to study a job to find the best way to perform that job.

Scientific Management Theory

Page 28: HCM114 - The future and the theories of Healthcare Management

This theory was developed by Max Weber.

The bureaucratic organization has become the dominant model for

organizations throughout the world. This model includes hierarchy of

authority, clear division of labor and formal rules and procedures.

Theory of Bureaucratic Management

Page 29: HCM114 - The future and the theories of Healthcare Management

There needs to be rules and regulations present in a business along with a very clear hierarchy of

authority in order to defeat the competition.Bureaucracy requires the development of highly

trained managers.Management was necessary to make businesses

efficient.Management was very impersonal and should

focus on the goal of efficiency.

Theory of Bureaucratic Management

Page 30: HCM114 - The future and the theories of Healthcare Management

This approach to management ushered in a human side to

management and resulted from a series of experiments conducted at a

Western Electric plant in Illinois in 1924.

A work brought about the theory developed by Abraham Maslow involving a hierarchy of human

needs which was published in 1943.

Human Relations Theory

Page 31: HCM114 - The future and the theories of Healthcare Management

Hammer introduced the concept of the process audit in 1990 focusing

on helping companies plan and execute process-based

transformations of their work process.

(required the total redesign of jobs that allowed decision making to move down to lower level

employees who would be rewarded for the work process and the work outcome.)

Reengineering Theory

Page 32: HCM114 - The future and the theories of Healthcare Management

Bureaucratic organizations are no longer working very well in the turbulent environment found in health care services delivery.

New problems faced by managers in the 21st century require new theories of management.

EMPOWERMENT OF EMPLOYEES LEADERSHIP STYLES

Need for change & innovation

Page 33: HCM114 - The future and the theories of Healthcare Management

Most managers, including those who manage health care facilities,

usually have developed special skills that allowed them to be appointed

to a managerial position. E.g. technical skills, human skills and

conceptual skills. These skills are usually associated with work

expertise or good people skills.
