Problem & Hypothesis
Capturing a child's history is time consuming and difficult to do.
The process is often repeated resulting in inaccurate data.
Problem
Creation of a Digital "Blue Book" where we create and maintain the child's and family's record will reduce the time needed to capture the child's history record and avoid repetition resulting to more accurate and consistent data.
Hypothesis
Solution Overview
• FACS, in partnership with Azeus, designed and developed a web based portal that presents the child’s case information in a simple and attractive manner.
• Data which is held in a Case Management System is pulled into the portal so there is no need for data re-entry.
• Information can be easily shared with the child and partner agencies.
• The child not only views, but also add, his/her own contributions to form a life story book.

The child’s life story contains aggregated information from case records, status updates and diary entries
• The user can also add significant life events to the timeline
The child can upload pictures, videos and documents
Supported file types:
• MS Office documents, spreadsheets and slides
• Text files (plain or with rich text formatting), emails, HTML
• Images (PNG, BMP, JPG, TIFF, GIF)
• Audio-video recording (MP3, MP4, MPEG, AVI)
