Life Plan Leaving Care Scenario A CYP is able to view their case plan, in an age appropriate format, by visiting the My Plan area. This area holds all the objectives and tasks 2 that the CYP has been asked to complete (related: Interactive Case Planning). Historical versions of their case plan can be accessed through the timeline slider 1 at the top. The CYP can comment 3 on their objectives and tasks, which can feed into the next case plan review. A challenge 5 is a short, sharp mission the CYP can complete, which is relevant to leaving care. The CYP can choose their own challenges 4 to complete based on areas of interest, such as Cooking, Living Skills, Money Management, etc... These challenges can also be added by the case worker during the case plan creation/review (related: Interactive Case Planning). The challenges are designed to empower the CYP and give them flexibility and incentive to engage with their case plan, as they choose. As such, they’re separate from a Case Plan, so they do not require any approval process. These challenges should be able to be curated by the case worker based on the CYP developmental age. They can also feed into rewards for the CYP . Life Plan My Plan

Scenario

A CYP is able to view their case plan, in an age appropriate format, by visiting the My Plan area.

This view can be very simple1, outlining high level areas of improvement for the child, or it can be more complex (see Leaving Care). The My Plan area will evolve in complexity and information as the child ages, slowly incorporating more elements of the mature view2.

Rewards3 can be given to the CYP, which can be seen in the My Plan timeline. These rewards may not be structured, and used at the case workers discretion.

Life Plan After Care

Scenario

Once a Young Person leaves care, the Client Portal is still accessible, but no longer allows the user to input new information or access certain features (such as messaging).

All the information within the Client Portal is able to be exported at the YPs discretion, and the process to access further case information can be initiated from this area4.

The After Care plan is split into a number of areas2, which include specific details on the plan as well as useful information for the YP, such as their current financial entitlements3.

Life Plan Interactive Case Plan/Questionnaires

Scenario

Part of interacting with a case plan is to provide input into case plan reviews. The system will make a variety of inputs from various people available to a case worker when creating a new case plan or reviewing an existing one (related: Interactive Case Planning). One of these inputs in a Questionnaire the CYP can be asked to complete.

The Client Portal will indicate when a questionnaire is available though the notifications as well as the My Plan area1.

Questions are presented in different formats, with older CYPs getting a more mature questionnaire format2 , and younger CYPs getting a simpler more friendly format. Similarly, the style of answers3 available are suited to the CYPs age.
