If you select to be vaccinated at a NSW Health managed clinic from the Vaccine Eligibility Checker, you will be directed to the NSW Health Vaccination Portal. This portal is where you will book your COVID-19 vaccination appointment by completing the following process: 1. Create your account – to do this, you will need an email address. If you do not have one, you can have a family member who has a valid email address book on your behalf or call the NSW Health Vaccination Booking Support Service on 1800 57 11 55. 2. Confirm your identity via a verification email – a verification email is sent to confirm we have the right email address for you. A verification link is included which you must click to proceed with creating the account and logging in. 3. Log in to your account, update your personal information and book your appointment – during the booking process, you will need your Medicare card number, IRN (the number beside your name) and expiry date, plus details of any previous vaccinations (including type and date it was administered). If you do not have a Medicare card, you are still able to register and book your COVID-19 vaccination. Book your COVID-19 vaccination appointment 1. What is the NSW Health Vaccination Portal? If you select to be vaccinated at a NSW Health managed clinic from the Vaccine Eligibility Checker, you will be directed to the NSW Health Vaccination Portal. This portal is where you will book your COVID-19 vaccination appointment by completing the following process:

1. Create your account – to do this, you will need an email address. If you do not have one, you can have afamily member who has a valid email address book on your behalf or call the NSW Health VaccinationBooking Support Service on 1800 57 11 55.

Book your COVID-19 vaccination appointment

1. From the NSW HealthVaccination Portal home page, click Create your account.

The Create your account form will display.

2. Complete the Createyour account form.

You can register without amobile number, howeverthis means you will notreceive SMS remindersfor your appointments. Avalid email address isrequired to complete theverification process.

3. Confirm that you have read the use and disclosure of information statement.

4. Click Submit.

Your request to create an account will be submitted and an email will be sent to the email address you provided to verify your account.

5. Find and open the verification email sent to your inbox and click the Verify email link.

On the portal, a message will display that the registration is complete.

6. Click Login to continue.

The Log in window will display.

7. Enter your user name (email address) and password.

8. Click Log in.

A Two-step Authentication screen will display and you will be sent a 6-digit one-time verification code.

9. Return to your inbox and find the email which contains the verification code (or SMS if you registered a mobile) and enter it in the Two-step Authentication screen on the portal.

10. Enter the verification code and then click Log in.

The NSW Health Vaccination Portal home page will display.

11. Click Enter personal information.

The Enter your personal information form will display.

12. Complete the Enter your personal information form. If you have a Medicare card, you must enter your full name, date of birth and residential address as you have it registered with Medicare.

13. Complete the Vaccination history section. If you have previously received a dose of the COVID-19 vaccine, you will need to provide the vaccine type and the date the dose was administered.

This information is used to determine the next dose that you need to book.

14. If you have already received Doses 1-2 or Doses 1-3 of the vaccine, you will need to complete section a) Booster or Dose 3 details.

If you have not had any doses of the COVID-19 vaccine you will need to complete section b) Preferred vaccine type. The selection you make will determine the list of clinics that you can book your appointment at.

15. Enter your address in the Enter address here field. As you start typing this will bring up matching addresses and you can select the correct address from the results displayed in the list.

This will populate the address fields.

16. If your address doesn’t appear in the list, clear the search and select the I want to enter manually checkbox to complete the address fields manually.

17. Complete the Medicare details section. If you have a Medicare card, please provide your card number, IRN (Individual Reference Number) and card expiry date. If you do not have a Medicare card, you are still able to register and book your COVID-19 vaccination.

18. Complete the Additional Details, Other details and Emergency contact details sections, and then click Submit.

A confirmation will display that you can now book your vaccination appointments.

19. Click Book vaccination.

The Book your vaccination form will display.

20. Confirm your clinic location in the site drop-down.

Important Information

The following applies if you are booking a Booster, Dose 3, or Booster for Immunocompromised appointment:

• If you don’t have a compromised immune system, and you are 16 years and over, your Booster is recommended three months after Dose 2.

• If you do have a compromised immune system, and you are 5 years and over, your Dose 3 is recommended two months after Dose 2. Then, if you are 16 years and over, you can book your Booster for Immunocompromised three months after Dose 3.

• You will need to bring proof of previous COVID-19 vaccinations on the day of your appointment.

21. From the calendar select a) a date that suits for the relevant dose, b) followed by an appointment time from the options available.

22. Where relevant, you can select a different clinic for your second dose.

23. If you need to schedule more than one dose (where you are booking Dose 1 and 2), from the calendar select a) a date that suits for the second dose, b) followed by an appointment time from the options available. The date range available for the second dose is a set number of days apart from the first dose.

24. The Vaccine Screening and Consent section allows you to tell us about any medical conditions before you attend a clinic so that a Clinic Medical Eligibility Reviewer can ensure the vaccination is safe for you.

25. Indicate if you consent to receiving the vaccine.

26. Click Submit.

27. A confirmation page will display with your appointment details.

You will also receive this information in an email, which includes a QR code for check in. You will need to bring this with you to your vaccination appointment, along with your proof of previous COVID-19 vaccinations (if you have had doses administered).
