The Environmental Risk Assessment is an initial screening for the potentially most hazardous areas in a home, listed in order by vulnerability.

To use: Replace “examples of problems” with actual findings from the assessment that are specific to the

patient’s home. Replace “examples of recommended interventions” with those specific to the patient’s needs. Once the assessment is completed, give a copy to the patient so he/she can address as many

problems as possible.
